Dr Karen Kearley I combine my clinical work with teaching and research in the Department of Primary Care in Oxford. My interests include Cognitive Behavioural Therapy (CBT) and Evidence Based Practice. Dr Andy Chivers I am interested in obstetrics, psychiatry, minor surgery and epidemiology. Dr Helen Steel I have recently moved back to Jericho from the North Oxford Medical Centre. I am the college doctor for Somerville and Keble Colleges. They also carry out detailed reviews for asthma, diabetes, hypertension, heart disease and other long standing conditions. Please let the receptionist know your reason for seeing the Nurse when you book your appointment, as this will determine the length of your appointment. Accolades and Complaints: It is always nice to receive accolades as we try very hard to care for all our patients. However if you feel you have cause for complaint please put this in writing to the Practice Manager or one of the Partners. Receptionists …deal with enquiries, make appointments and help the surgeries run as smoothly as possible. They also prepare prescriptions and take calls for home visits. They act on the doctors’ instructions when dealing with requests for appointments and visits. They are there to help you and answer your queries whenever possible. Please treat them with courtesy. Health Visitors The Health Visitor team work closely with the whole team and are concerned with health promotion both individually and within groups. They see all families with young children and advise on normal health and development. They can also be contacted to discuss the health of other age groups including the elderly. Their telephone number is: 01865 903077 (answer phone) District Nurses Several nurses work with our practice and are responsible for the nursing care of patients confined to their homes. You can refer yourself or be referred by a GP, social worker or the hospital. Their telephone number for any queries: 01865 903650 (answer phone if not in the office) Midwives A team of midwives is attached to the Practice. They run their own clinics at the Health Centre (only on a Wednesday). When unavailable on a Wednesday please contact Blenheim Midwives direct on: 01865 220457 Named GP Following publication of the new GP Contract 2015/2016, all patients have now been allocated a named GP. For most patients this will be their usual GP. If a patient wishes to change their named GP, they should inform reception. Clinics and Services available at the Practice Antenatal Care This is shared between the GPs, midwives and Health Visitors. Cervical Smears These are routinely offered every 3 years to women 25-50 years of age, and then every 5 years to 65 years of age. There is a national recall system to remind you when your next test is due. Child Health Clinic This is run by the Nurses and Health Visitors and is held at the following time: Every Monday (Health Visitor drop-in), 10.00am – 12 noon This clinic is an opportunity to talk about general development and any other queries you have about your childs welfare. Drugs and alcohol clinics Together with specialist addiction councillors Anna Simpson and Michael Robertson, we provide support for patients who have drug or alcohol problems.

Management of Longstanding Conditions Our specialist nurses Di Croft and Kathryn Balmford offer reviews for asthma, diabetes, heart disease and hypertension in their own clinics. We advise patients with these conditions to take advantage of these reviews annually. Please let the receptionist know your reason for seeing the Nurse when you book, as this will determine the length of your appointment. Counselling and Psychological Services We have an in-house counsellor. If you would like to see him please discuss a referral with the GP first. GPs can also make referrals for longer term treatment should this be necessary. Other Services Certain services are not covered by the NHS but can be provided at cost. These include signing of passport applications, private medical insurance, sports and driving medicals, fitness to travel certificates, yellow fever injections and private sick notes. Please ask at reception for the current fees which are set by the British Medical Association. How to register with the practice You will need a permanent address within our practice area, photo ID, proof of address and be entitled to NHS treatment. Please ask the receptionist for a registration form which should be completed in full. New patients with children under 5 years of age will be asked to complete forms for the Health Visitor. You may request which partner you have as your named (registered) doctor but please note you can see any of the doctors within the practice. We do recommend that you see the same doctor for any particular episode of illness. In an emergency it may not be possible to see your usual doctor. On occasions when the surgery is busy, routine appointments may not be available for several days. However, urgent medical cases will always be dealt with on the same day. You may be offered a telephone appointment if appropriate. Please let the receptionist know as early as possible during the day if you need an urgent appointment. Booking appointments We routinely offer 10 minute appointments. Please make sure you ask for separate appointments for each member of the family who need to be seen. Please also make a double appointment if you have several problems for the doctor to deal with. This helps our surgeries run on time and reduces stress for patients and staff alike. If you require a special service such as a minor operation, an insurance examination or, if you have a medical form that needs completing, please inform the receptionist. If you are unable to keep your appointment please cancel it as soon as possible, so that it can be offered to another patient. GP appointments are usually available between 8.30 - 12 and 3 – 5:30 p.m. Mon-Fri 6.30-7.30pm on Tuesday evenings and 8.30-11.30 on Saturdays. Home visits The doctors will make home visits if you are unable to attend the surgery for medical reasons. If possible please phone for a visit request before 11am as it really helps the doctors plan their rounds. Please give the receptionist full details of the situation so that the most urgent cases can be seen first. The doctor may telephone to discuss your illness before they arrange a time to visit. This is normally done between the hours of 12 – 15:00.

Telephone advice All the doctors are happy to provide telephone advice but prefer not to be interrupted during surgery time unless it is urgent. If the doctor is unavailable the receptionist will take your number and get the doctor to phone you back. Injuries and accidents Many minor injuries can be dealt with by the practice nurse or a GP. Patients with more serious illnesses (e.g. serious burns, deep cuts and possible major fractures) should go directly to the Accident and Emergency department at the John Radcliffe Hospital, if the injury is less than 24 hours old. Repeat prescriptions: You should allow 48 hours for your prescription to be prepared. If you would like the prescription returned to you by post, please provide a stamped self-addressed envelope and allow slightly longer for its return. Please inform one of the receptionists if you would like your prescriptions to be sent electronically to your nominated pharmacy. We are unable to take requests for repeat prescriptions over the phone as this can lead to errors and also blocks our telephone lines. Repeat prescriptions can be requested via our website. Please allow 48 hours for this to be processed before collection. Patient Access It is now possible to book appointments and request prescriptions online. Please ask one of the receptionists for details. Blood tests Every week day morning there is a phlebotomy service available for blood tests at Jericho Health Centre. These clinics run between 8.30am – 12.30pm. Please ask the receptionist if you are unfamiliar with how to have a blood test. The clinics are pre-bookable; although we reserve a few appointments for patients who come on the day. Contraception We offer the full range of contraceptive services. Dr Walker and Dr Luckhurst fit contraceptive coils and implants. Please make an appointment with one of them if you wish to discuss these options. Fittings are usually arranged on a Monday, Wednesday or Thursday morning. Out of Hours services If you have a medical emergency outside of our opening hours you can telephone 111. 111 is the number to call when a medical situation is less of an emergency than 999. Alternatively you can call 01865 429993 and you will be connected to OXEMS – Oxford Emergency Medical Service. The answering service will take your details and the details of the emergency and will arrange for a doctor to contact you either by telephone, an emergency appointment at the Out of Hours Centre or, if necessary, a home visit. This service is for emergencies – please do not call for routine enquiries or with matters that can wait until our surgery is next open.

Confidentiality All information about patients is treated as confidential and will not be disclosed to a third party (i.e. insurance firms) without your written consent. You may request to see any such information before it is then disclosed. Disabled access The health centre is a ground floor premises with wheelchair access. Medical students We sometimes have medical students from Oxford University attached to the practice as part of their training. You will be informed on the days when students are attending. Please tell the receptionist if you do not wish to see a medical student during your consultation. Keble & Somerville College Students We run daily surgeries with the College Nurse at both colleges for students studying at either Keble or Somerville. The GP college surgeries are held weekly at Jericho Health Centre. Patient responsibilities All members of our team aim to treat you with respect and expect you to do the same. We will not accept any violent, abusive or threatening behaviour from patients. Patients or visitors may be asked to leave the premises if their behaviour is deemed inappropriate. Such behaviour may result in a patient being removed from our list or the police being called. Patient Participation Group

The practice has an active Patient Participation Group (PPG), which meets quarterly. Its aim is to involve

patients in the development of services provided by the practice and to influence the provision of local health

services. Any patient can join the PPG. We understand that not everyone can get to meetings, but would like
