Please follow the simple steps below to ensurethe safe delivery of your home movies toIcehouse Pictures.

Organize.Locate and organize all of your home movies and individually number each �lm reel and videotape in chronological order using a marker. Also be sure that the subject matter is clearly written on each item submitted.

Please do not use post-it notes or notes with tape as they tend to fall o� during shipping.

Any unlabeled �lms or videotapes will be given a generic title on the completed DVD.

Package.1. Use a strong, durable box to package your home movies.

2. Carefully wrap your �lms and videotapes with bubble wrap.

3. Complete the customer info card and enclose it within the box.

4. A�x the attached shipping label to the outside of the box.

Ship.Ship your order with the carrier of your choice and obtain a tracking number so you can track the shipment.

Once your package has arrived, a representative from Icehouse Pictures will contact you.

INFORMATION ACCURACY:It is the responsibility of the client to ensure accuracy prior to submission. In the event information was submitted incorrectly

on the order form it is the �nancial responsibility of the client to purchase another copy with the correct information. Material deemed obscene, illegal, or criminal in nature will be refused or reported to authorities.

LIMITATION OF LIABILITY:When submitting videotapes, �lms or any other form of media to us for duplication or for any other reason, client acknowledges

and agrees that Icehouse Pictures and any individual or business a�liated with Icehouse Pictures shall not be liable for any loss or damage to such materials following receipt, during processing of such materials and during transit. Client expressly understand and agree

that Icehouse Pictures shall not be liable for any direct, indirect, incidental, special, consequential or exemplary damages, including butnot limited to, damages to, theft or loss of the content you provide, damage to DVD players, computers, or other equipment, loss of pro�ts,

goodwill, use, data or other intangible losses, resulting from, but not limited to, product defects, accident, misuse, acts of God or other circumstances. Our liability will be limited to a full refund of any monies paid to us by you.

DVD COMPATIBILITY:I understand that it is my own responsibility to verify whether or not the intended DVD player is capable of playing DVD-R discs.

Icehouse Pictures is responsible for the digitizing of the product and will demonstrate on Icehouse Pictures DVD Players that the service has been performed correctly. This display/demonstration veri�es completion of the agreed services. Icehouse Pictures is not responsible,

if in the event that the customers own DVD equipment is not capable of displaying the �nished product. Demonstration of �nished product con�rms that your DVD product was successfully digitized.

COPYRIGHT LIABILITY:Customer warrants that the videotape, �lm, photos, or other media submitted to Icehouse Pictures for processing

is not protected by a third party copyright owner or if protected, the customer has obtained permission of such third party copyright holder to make copies. Customer also agrees to protect and indemnify Icehouse Pictures (or it’s assignees) from any costs

related to copyright infringement.
