HMO planHealth Net HMO (Health MaintenanceOrganization) Plan is a network of contractedphysicians throughout the state of California offeringhealth care benefits. With an HMO, you select aprimary care physician from Health Net’s HMOnetwork. Your primary care physician is responsiblefor coordinating your care, and he or she willauthorize a referral to a specialist in his or herparticipating physician group. While you may notselect a specialist as your primary care physician,female members can always self-refer to an OB/GYNwithin their primary care physician’s physician group.

With an HMO plan, your primary care physicianmust be contacted before you receive initialtreatment through a hospital or from a specialist,except for emergencies, OB/GYN visits, mentalhealth care and chiropractic care. To view thespecialists affiliated with a particular physiciangroup, go to www.healthnet.com/homedepot orcall Health Net’s Customer Contact Center at1-800-847-3991.

Point of Service(POS) planYour POS plan offers you the flexibility to choosehow to receive health care services. Services arecovered at three levels, so it’s like having three plansin one! Each covered member of your family canuse any level at any time – it’s your choice.

SELECT I level – HMO features comprehensivehealth care services with low out-of-pocket costs;care must be coordinated through your primarycare physician.

SELECT II level – PPO allows you to self-refer toany physician in the Health Net PPO providernetwork at a higher copayment than SELECT I.

SELECT III level – Out-of-Network allows you tosee any licensed provider in the United States forcovered services, but you will typically have higherout-of-pocket costs.

HOW THE PLANS WORK

HMO ADVANTAGES INCLUDE:• No paperwork or claim forms

• Set copayments for office visits andprenatal, postnatal and newborn care

• Preventive care and health education

You may self-refer to anylicensed physician in-networkor out-of-network when usingSELECT level II or III. Just keepin mind that if you do not gothrough your primary carephysician, you will pay higherout-of-pocket costs. To get thehighest level of benefits, careshould be accessed throughSELECT level I, which works justlike the HMO plans describedin this brochure.

Helpful things to knowWhat is a Primary Care Physician?A primary care physician is a Health Net contractedphysician who coordinates all of your care. Primary carephysicians include general and family practitioners,internists and pediatricians.

Who needs to choose a primary care physician?Whether you enroll in an HMO or the POS plan,everyone must select a primary care physician. (Note:If you don’t select a primary care physician, one will beassigned to you by Health Net.)

• For those selecting an HMO plan, your primary carephysician is responsible for coordinating all care.

• For those choosing POS, to receive the highest levelof benefits under the SELECT I level, you must seeyour Primary Care Physician. However, you also havethe choice to see specialists directly – at reducedbenefit levels – by accessing care through SELECT IIor III levels.

How do I choose a primary care physician?To pick a primary care physician, simply choose aphysician from the Health Net HMO Network.

For the most up-to-date network listings, go towww.healthnet.com/homedepot > Doctor Search. Be sureto call the office to make sure that the doctor is acceptingnew patients. Also, consider the list of specialists withinyour primary care physician’s physician group beforeselecting a primary care physician.

Do all my family members have to pick the sameprimary care physician?No. Each family member can pick a different primarycare physician. This helps to make sure each familymember’s specific preferences and needs are met.

Can I change my primary care physicianor physician group?Yes. You can change your primary care physician orphysician group as often as once per month. Call thededicated Customer Contact Center for The HomeDepot associates at 1-800-847-3991 by the 15th ofthe month for the change to be effective the 1st of thefollowing month.

Where can I get prescriptions filled?Health Net contracts with most major supermarket-based pharmacies, privately owned pharmacies inCalifornia, and all major chains throughout the UnitedStates. To find a Health Net pharmacy, log on towww.healthnet.com > Start Here > View PharmacyInformation >Select a Region > Find A Pharmacy.

How do I get my prescription at the best price?1. Ask your doctor or pharmacist if your prescribedmedication is on our RDL, or if there is an alternativethat is covered.

2. If it is a brand name drug, ask if there is a genericalternative you can use.

3.Sign up for the Prescriptions by Mail program for yourongoing (maintenance) prescriptions.

4.Purchase covered drugs at a participating pharmacy.

Getting care and using your ID cardABOUT YOUR HEALTH NET ID CARD

Keep your ID card with you at all times, as it containsinformation you’ll need to visit your doctor or in case ofemergency. Be sure to show your ID card each time youaccess care.

Review your ID card carefully. Your primary carephysician’s name and phone number is listed on yourcard. Whether you enroll in an HMO or POS plan, youwill receive just one card to use in all situations. Pleasemake every effort to enroll in a timely manner so yourcards will not be delayed.

PHYSICIAN VISITS

Simply present your ID card and pay the appropriatecopayment to obtain services from your physician. ForHMO and SELECT I level POS benefits, always contactyour primary care physician before seeing a specialist.

PRESCRIPTION COVERAGE

To fill a prescription, simply present your ID card at aparticipating pharmacy and pay the appropriatecopayment.

MENTAL HEALTH BENEFITS –HMO AND POS

CHIROPRACTIC BENEFITS –30 VISITS PER YEAR!

Simply visit any chiropractor participating in theHealth Net Chiropractic Network (through AmericanSpecialty Health [ASH] Networks). Present your ID cardand pay the appropriate copayment. “Transition of care” is Health Net’s commitment tohelping you transition your medical care without a gapin service when you are switching plans, doctors ormedical groups.

With transition of care, Health Net works to ensureyour care is the most important priority during timesof change. When you need to change doctors contactHealth Net’s Customer Contact Center at1-800-847-3991 as early as possible, prior toenrollment, to be sure we know about your situation.

What do I do in case of emergency?• If the emergency is life-threatening (you are in dangerof losing your life and require immediate care) youshould call 911. Contact your primary care physicianor medical group within 48 hours of receiving care, asthey will need to authorize any necessary follow-up care.

• If the emergency is not life-threatening, contact yourprimary care physician or medical group office. Writedown the name of the person you spoke with and getinstructions on how to receive immediate care.

What should I do after I have used emergency services?Members must always contact their ParticipatingPhysician Group and/or Member’s Primary CarePhysician as soon as possible whenever emergencyservices have been received. Follow-up care must beperformed or authorized by the member’s physiciangroup or it will not be covered after the medical problemceases to be an emergency.
