must purchase the qualifying Pinnacle product listed on this

form. Your purchase must be made between November 25,

2009 and December 3, 2009 through Frys Electronics.

Additionally, the materials and information listed below are

required prior to rebate payment. Failure to submit any of

the following items will delay or prevent rebate payment.
