Brophy College Preparatory Physical Examination, Informed Consent/HIPAA & Concussion Information Forms/Instructions All incoming freshmen and returning athletes (Fall, Winter, Spring sports) are required to fill out accompanying documents, have a physical examination, and submit, NO LATER THAN JULY 15, 2016 before the start of the 2016-2017 school year. Freshmen are not to begin school without a physical and athletes may not participate without a physical. I/We give our son _______________________________________ permission to participate in interscholastic athletics, physical education, and/or intramurals. I/We realize such activity involves potential for injury, which is inherent in all sports and activities. I/we understand that if I/we have questions pertaining to this risk, I/we will address them to the coach, athletic director, or athletic trainer. I/We realize that even with the best coaching, use of the most advanced protective equipment, and strict observance of rules, injuries are still a possibility. I/we understand that these injuries range from minor to severe: on rare occasions, the injuries may be so severe as to result in total disability, paralysis, quadriplegia, or even death. I/We understand and acknowledge these risks. I/we understand the specific risks associated with the sport or activities in which my son will participate, and I/we have read and understand this warning. I/We also understand we can contact school medical personnel if I/we have further questions or concerns. I/We agree to contact the athletic trainer or other school medical personnel if there are any changes to my son’s health, if pertinent information needs to be communicated for appropriate medical care, or if there are any limitations to my son’s activities. I/we accept the risks as a condition of my son’s participation in sports or activities. I acknowledge that the all information on the physical form is accurate and I will report any changes to Brophy’s health care providers. In the event that I cannot be reached, I, the undersigned parent/guardian of the student above named, do hereby give and grant unto any medical doctor, hospital, or school medical personnel my consent and authorization to render such aide, treatment or care to said student as, in the judgment of said doctor, hospital, or school medical personnel may be required, on an emergency basis, in the event said student should be injured or stricken ill while involved in activity by the above named school. I understand that I have certain rights to privacy regarding my protected health information. These rights are given to me under the Health Insurance Portability and Accountability Act (HIPAA). I understand that by signing this consent I authorize you to use and disclose my protected health information to carry out:

• Treatment by Brophy healthcare providers • Responsible and prudent care in medical emergencies or for the medical management of acute or chronic medical conditions

I also understand that I may contact the Brophy administration or healthcare providers if I have questions or concerns with this Notice of Privacy Practices, and I understand that Brophy reserves the right to change the terms of this notice from time to time. I understand that I have the right to request restrictions on how my protected health information is used and disclosed to carry out treatment and health care, but Brophy policy doe s not have to agree to these requested restrictions. However, if there is an agreement, Brophy is bound to comply with this restriction. I understand that I may revoke this consent, in writing, at any time. However, any use or disclosure that occurred prior to the date I revoke this consent is not affected.

• I understand I have the opportunity to ask questions about concussions or concussion management to Brophy’s healthcare providers. • I have fully disclosed to the staff any prior medical conditions and will also disclose any future conditions. • There is a possibility that my/my son’s participation in sport and school activity may result in a head injury and/or concussion. In rare cases, these

concussions can cause permanent brain damage, and even death. • A concussion is a brain injury, which my son/I are/am responsible for reporting to the coach, athletic trainer, or team physician. • A concussion can affect my/my son’s ability to perform everyday activities, and affect his reaction time, balance, sleep, and classroom performance. • Some of the symptoms of concussion may be noticed right away while other symptoms can show up hours or days after the injury. • If I suspect a teammate has a concussion, I am responsible for reporting the injury to the school staff. • (Parent/Guardian) If I suspect my son has a concussion, I am responsible for reporting the injury to the school staff. • (Parent/Guardian) I will not expect or encourage my son to return to play in a game or practice if he has received a blow to the head or body that results

in concussion related symptoms. • (Parent/Guardian) I acknowledge that my son will not return to play in a game or practice until his symptoms have resolved AND he has clearance to do

so by a qualified health care professional. • Following concussion the brain needs time to heal and you are much more likely to have a repeat concussion or further damage if you return to play

before your symptoms resolve. • Based on the incidence of concussion as published by the CDC, the following sports have been identified as high risk for concussion; baseball, basketball,

diving, football, pole vaulting, soccer, softball, spiritline and wrestling.
