Check List for PHCs

1) Number of patients used the out-patient services in the past quarter:

a. How many of them are from SC, ST, and other backward classes?

b. How many of them are women?

2) Availability of Medicines in the PHC:

d. Is the facility for internal examination for gynaecological conditions

available at the PHC?

e. Is the treatment for gynaecological disorders like leucorrhoea,

menstrual disorders available at the PHC?

a. Was there an outbreak of any of the following diseases in the PHC area in

the last three years?

i. Malaria

iii. Gastroenteritis (diarrhoea and vomiting)

b. If yes, did the PHC staff respond immediately to stop the further spread of

the epidemic?

c. What steps did the PHC staff take?

d. How is the behaviour of PHC staff with the patient?

i. Courteous

e. Is there corruption in terms of charging extra money for any of the service

provided ?

f. Does the doctor do private practice during or after the duty hours?

g. Are there instances where patients from a particular social background (SC,

ST, minorities, villagers) have faced derogatory or discriminatory behaviour

or service of poorer quality?

h. Have patients with specific health problems (HIV/AIDS, leprosy suffered

discrimination in any form? Such issues may be recorded in the form of

specific instances.

i. Are women patients interviewed in an environment that ensures privacy

and dignity?

women attendant and procedures conducted under conditions that ensure

privacy?
