NORTHEAST OHIO MEDICAL UNIVERSITY PHASE 1 CAPP REQUIREMENTS

Effective March 1, 2018

Applies to: Bacc/M.D. & Post-Bacc/M.D. program students admitted to undergraduate partner schools in Academic Year 2015 or greater with expected NEOMED promotion in Academic Year 2017 or greater

1 The Phase I Committee on Academic and Professional Progress (CAPP)

1.1 What is CAPP? • CAPP is a Committee made up of NEOMED basic science and clinical faculty, medical students,

plus undergraduate program coordinators and NEOMED administrative officers as non-voting members.

o A similar committee exists to monitor students already enrolled at NEOMED. • CAPP reviews students in all partnership programs to determine eligibility for continuation in

the program, including promotion to the medical school phase. o CAPP monitors: academic performance and intellectual & behavioral readiness

(“Professionalism”)

1.1.1 What Types of Things Can CAPP Decide? • Promotion/provisional promotion-moving to Phase 2 of the program (NEOMED) • CAPP can grant requests for continuation in Phase 1/retention in the program. This means

advancing in the undergrad portion of the program in the following year. • To retain a student for an additional year in Phase I (even if not requested by student). This can

be based on student request or mandated/required by CAPP for academic or professionalism concerns.

• Grant a request for an enrichment year. • Dismissal from the program/rescind provisional offer of admission. • Other actions as needed (e.g. professional development).

1.1.2 Review The status of each student enrolled in the Baccalaureate or Post-Baccalaureate phase of the combined Bacc/M.D. or Post-Bacc/M.D. degree program (Phase 1) will be reviewed at least annually by the Phase 1 CAPP to determine whether or not each student will be permitted to continue to progress in the Bacc/M.D. or Post-Bacc/M.D. degree program, and under what conditions. This review consists of the following:

1. Evaluation of academic performance as measured by the grades received in all courses and scores from the Medical College Admission Test (when appropriate);

2. Assessment of each student's individual readiness for medical studies as assessed by ratings

and interpretive comments of faculty and other appropriate university and College of Medicine personnel; and

3. Consideration of information contained on the student's Petition for Review of Progress and any other relevant information, such as, but not limited to: Individualized Course Plans (ICPs), student disciplinary action, criminal background checks, medical reports, academic integrity violations, or unprofessional behavior.

1.2 General Guidelines and Definitions of Terms Possession of minimally acceptable academic credentials in no way guarantees promotion or retention in Phase 1.

Expectation of Appropriate Personal and Professional Development In addition to the acquisition of the required levels of skill and knowledge, as demonstrated through academic markers of GPA/BCPM/MCAT, the College of Medicine requires candidates for promotion to demonstrate the following:

1. accomplishment in integrity, self-awareness and interpersonal skills; 2. commitment to excellence; and 3. responsibility for one's self and others in a manner that is developmentally commensurate

with a newly matriculating medical student. This approach reflects the concern of the College to admit medical students who demonstrate the appropriate development of human qualities and commitment to professional standards as well as technical competence. Evaluation of each student's progress includes all of the above aspects. Failure to meet any of the standards of the program may result in remedial or other actions, including revoking your provisional admission to the program.

Academic year Defined as fall and spring semesters; in the event a student engages in summer coursework, this coursework will be calculated as part of the overall GPA/BCPM. Class Status Definitions – Bacc/M.D. B3 is defined as a student’s first academic year in the Bacc/M.D. Partnership program B4 is defined as a student’s second academic year in the Bacc/M.D. Partnership program. B5 is defined as an added (third) academic year in the Bacc/M.D. Partnership program Class Status Definitions – Post-Bacc/M.D. PB1 is defined as a student’s first academic year in the Post-Bacc/M.D. Partnership program. PB2 is defined as a student’s second academic year in the Post-Bacc/M.D. Partnership program. PB3 is defined as an added (third) academic year in the Post-Bacc/M.D. Partnership program.

Phase I Definition The period between when a candidate is granted provisional admission into NEOMED and the time of approval for matriculation into NEOMED as an M1. During Phase I the student is enrolled at the partner institution.

Phase II Definition The period of time after promotion to NEOMED as an M1 student, inclusive of the student’s enrollment at NEOMED.

Expectation of Enrollment Status and Degree – Bacc/M.D. Students are required to be enrolled at the Baccalaureate University for fall and spring semesters. Students receiving transfer credit are required to complete a minimum of 30 credit hours at the Baccalaureate University. Students must complete all Bacc/M.D. required courses by the spring semester of B4, including – but not limited to – pre-medical sciences (BCPM, (Biology, Chemistry, Physics, Math)). Students must complete a bachelor’s degree within two (2) academic years of beginning Phase 1, that is, by the end of the spring semester of B4, unless granted an extension by the CAPP committee (see section 1.6 Phase I Enrollment Extension). Expectation of Enrollment Status – Post-Bacc/M.D. Students are required to be enrolled at the Post-Baccalaureate University for fall and spring semesters; students must take at least one 3-credit hour science course and one Urban Health course per semester. Additional seminars and workshops may be required in any given semester. Students must complete as many Post-Bacc/M.D. courses as possible by spring semester of PB1 or as pre-approved by Phase I CAPP. Students must complete all Post-Bacc/M.D. required courses by the spring semester of PB2, including – but not limited to – pre-medical sciences (BCPM, (Biology, Chemistry, Physics, Math)). Students must complete at least 30 credit hours at the Post-Baccalaureate University by the end of the spring semester of PB2. Grade Minimums • All required courses must be passed with a “C-” or better to receive credit toward promotion. • All courses that are taken “Credit/No Credit” or “Pass/Fail” must receive a grade of “Credit” or

“Pass” to receive credit toward promotion. • A "D" or "F" in any BCPM (Biology, Chemistry, Physics, Math) course at any time may be grounds

for dismissal from the program and shall be sufficient grounds for denial of promotion. • A “D” or “F” in any course required by your undergraduate program must be remediated.

Remediated courses will be included in the Overall and BCPM GPA calculations. • All required courses, including transferred courses, must be completed no more than seven

years prior to Spring of B4/PB2. • A “D” or “F” in any concentration course at any time may be grounds for a dismissal from the

program and shall be sufficient grounds for denial of promotion. • Required BCPM and concentration courses transferred from another institution will be subject

to the same minimum grade requirements. Failure to meet grade and course requirements will place the student's participation in the M.D. Partnership Program in jeopardy.

GPA Calculation Methods • B3/PB1 Overall GPA and BCPM GPA (Biology, Chemistry, Physics, Math) includes transferred

courses, and fall and spring semesters. Overall GPA includes all required and electivecoursework for which the partner university gives credit from admission to the programthrough Spring Semester of B4/PB2. It excludes courses that were (a) not completed duringand (b) are not required as part of the M.D. Partnership program or the student’s bachelor’sdegree.

• BCPM GPA is computed by the method employed by the AMCAS system of the Associationof American Medical Colleges (e.g., in the case of repeated course work, both the originaland the repeated grade are included in GPA/BCPM calculations). Post-Bacc students mayselect which portion(s) of their previous coursework they wish to apply toward promotionrequirements, on approval by the program. All courses used will be included in thecalculations.

Phase 1 CAPP Role Specific to the Partner Institutions The role of the Bacc or Post-Bacc partner will be to monitor, support and guide the Bacc/M.D. or Post-Bacc/M.D. Partnership students during their undergraduate/Post-Baccalaureate studies, so they can successfully matriculate to NEOMED College of Medicine. Documentation of Bacc or Post-Bacc student academic and professional progress will be maintained by the Bacc or Post-Bacc partner and provided to the Phase 1 CAPP Committee.

1.3 Requirements for Retention (Continuation in Phase 1) A student’s provisional admission can be revoked if they do not meet the continuation requirements for their program.

Post-Baccalaureate/M.D. Requirements for Continuation (Good Standing in Provisional Admission Program)

3.30 3.30 Individual Course Plan (ICP) that identifies GPA benchmarks that may differ from the 3.25 PB1 requirement. For instance, a student transferring in a low GPA may have to reach a GPA of higher than a 3.25 in order to matriculate to NEOMED at the end of PB2. The ICP must be facilitated by the Partnership Student Coordinator/Advisor and approved by the Phase I CAPP CSU Subcommittee before the student enrolls in the M.D. Partnership.

1.4 Requirement for Promotion to Phase 2

Promotion to studies leading to the Doctor of Medicine degree from the College of Medicine at NEOMED is contingent upon petition by the student and upon a positive recommendation of the NEOMED’s Phase I CAPP. Promotion constitutes a decision of the final offer of admission to NEOMED as an M1.

Decisions of the Phase I CAPP are based upon fulfillment of all the following criteria:

1.6 Phase 1 Enrollment Extension The M.D. Partnership experience is designed so that students complete all course requirements for promotion within the prescribed Phase I timeline. Students may petition to extend Phase I of the program by one additional year or be referred by the Phase I CAPP to complete an additional year to remediate an unmet requirement for promotion (see Section 1.4 for promotion requirements). All student-initiated extensions need prior approval of the Phase I CAPP.

1.7 Voluntary Withdrawal Students who decide that they no longer want to participate in the pre-medical M.D. program must notify the Phase I coordinator at the pre-medical partner campus, who in turn will notify NEOMED.

2 PHASE I CAPP MEETINGS

2.1 When Does CAPP Meet? • CAPP meets several times during each academic year for:

o Professionalism cases (as needed) oro Other business of the Committee – reviewing standards, processes, etc.

• CAPP has a progression meeting at the end of each academic year. This is typically in June (oncegrades are in) over the course of one or two days.

o CAPP reviews all students in a partnership program. CAPP determines eligibility to continue in the program, promotions to medical

school, etc.

2.2 What If I Meet All Expectations? • You typically do not have to attend a meeting.• All students are reviewed annually at the June meeting for formal approval of their request

(continuation/retention in undergrad portion, promote to NEOMED, etc.).o You must submit your annual “Petition for Review”. This is your chance to request

continuation or promotion. You will be reviewed based on the requirements of therequest you make on this form. If your original request needs to be changed, you maysubmit an addendum. Your undergraduate coordinator will assist you in this process.

o Academic performance information is forwarded by your undergraduate coordinatorafter final grades are received, including your MCAT scores.

2.3 Why Might a Student Be Referred for Individual Review? • Professionalism:

o Conduct or other issues beyond academic criteria. The case will go through your home university judicial/conduct process first (if

applicable). A professionalism referral may result in an invitation to meet with CAPP at

NEOMED to discuss the case in more detail. Further sanctions may be placed upon you by CAPP.

• Academic:o Students who do not meet the criteria for retention or promotion

2.3.1 What Happens If I’m Referred For Individual Review? • Individual Review means that CAPP will review your file because you are not meeting some

requirement(s) of the program. Individual Review does not necessarily mean that you mustattend a meeting.

o Professionalism cases are referred to the co-chairs for determination of the severity ofthe infraction. If the co-chairs deem the case to be severe enough in nature, you will beinvited to appear before the Committee. If they decide that the case is less severe innature, you may not have to appear before the Committee. In either case,documentation is placed in your student file and you will be monitored for theremainder of your time in the program.

o At the year-end meeting (June), CAPP will invite students to appear in person if: revoking provisional admission to medical school is a possible outcome; there is a professionalism concern (see above); or an undergraduate coordinator requests that a student attend the meeting.

o Second year students who do not meet the promotion standards as listed in section 1.4may be granted a third year (B5 or PB3). Students in this situation are not automaticallyinvited but may request to attend the meeting through their undergraduatecoordinator. NEOMED must receive this request from the coordinator no later than 10business days before the meeting.

o Leave of Absence request students may be invited to attend depending on thecircumstances. This is determined by the co-chairs.

• If it is determined that you need to be invited to appear before the Committee, an invitationletter is sent on behalf of the Committee.

o You should meet with your coordinator/advisor to discuss the process and your options.o You may choose to attend or not.

You do not have to attend the meeting if you are invited. If you choose not toattend, there are implications if the Committee reaches a decision for dismissal.

• You must submit the “Student Interview Form” by the deadline. This provides more informationto the Committee. This form will be sent with your invitation letter.

You should attach any supporting or relevant documentation that is specific toyour situation.

2.4 What happens at a Phase I CAPP meeting?

2.4.1 If You Choose to Attend: During the Meeting • The Committee members will introduce themselves.• You will be given the chance to make your opening statement (2-3 minutes).

o It is ok to write it in advance and read it.• Be prepared for the Committee members to ask you questions and discuss your situation for as

long as is needed to become well informed.• After the questions, you will be given the opportunity to make a closing statement.• You will then be excused while the Committee deliberates.• You may bring a support person with you.

o It can be Faculty/staff/student from NEOMED or your current partner University. Familymembers or lawyers are not allowed in the meeting. You can have others with you while

you wait to be escorted to the meeting room, but they cannot accompany you to the actual meeting.

o This person may be there to help you feel more comfortable. They do not have to talk,but the Committee may ask them questions.

• As CAPP is a formal College Committee, we suggest those in attendance present professionalattire.

2.4.2 If You Choose Not to Attend • The Committee will deliberate on the basis of the information that is available to them from

your student file (including material provided by current institution, court documents, etc.) andany additional items you provided with your Student Interview Form.

• If the Committee chooses to dismiss you from the program, and you elected not to attend themeeting, you forfeit your right to appeal.

2.5 What to expect after the meeting • All students that are reviewed will receive a decision letter whether they attended the meeting

or not.• Official decisions are sent within 5 business days of the meeting.

o CAPP decisions are sent via email.o Decisions which include dismissal/rescission of provisional admission to medical school

will also be communicated via certified mail.• All communications will include the Committee’s decision and information on next steps,

including the appeals process (when applicable) and any deadlines.

2.5.1 If You Are Dismissed / Conditional Offer Admission Revoked • You may be eligible to continue at your current institution – talk to your campus coordinator. Be

sure to inquire whether the decision will be noted on your official transcript or not.• You may have the opportunity to file an appeal to Executive Review CAPP under specific

conditions.o The conditions are limited to:

evidence of procedural defect or error in the committee’s operations or significant and compelling information becomes available (that was not known

to you at the time of the meeting).o You will receive a form with the dismissal decision letter, including a deadline by which

to appeal.

2.5.2 If You Are Being Retained for a B5 or PB3 year • Register for Fall classes at your current university.• Talk with your coordinator/advisor about conditions for promotion as outlined in section 1.4

and strategies to make the most of this opportunity.

2.5.3 If You Are Being Promoted • Complete any remaining summer courses successfully.• Submit registration/other pre-matriculation requirements to NEOMED by established deadlines.

If you have any questions, start with your current campus coordinator. They know your school’s program the best and are best equipped to assist you. If they do not know, or if they think it is a question for NEOMED, they will ask us.

3 STUDENT RESPONSIBILITIES

3.1 Petition for Review (See Appendix A for example) • All students must complete the Petition for Review Form annually.• Your Petition Options are:

o To request Phase I Continuation/retention. If you want to request to remain in the program, this is the option you will choose. If you are in your 2nd year, this is something that you should discuss with your campus coordinator. You are eligible for this if you are in your 2nd year and need or want a 3rd year in the undergraduate phase for whatever reason. This might be academic or it might be personal.

o To request promotion. If you are in your 2nd or 3rd year (B4, B5, PB2, or PB3), have met all criteria and feel ready to move up to medical school, that is the time to request promotion.

o To withdraw from the program.o If you are in your 3rd year (B5 or PB3), your options are to request promotion or

withdraw, you cannot request to be retained for a 4th year.

• Deadline: April 15, 2019o The form will be emailed to you.o You must submit the completed form to your coordinator.o In the event your circumstances change after you submit the form, you do have the

opportunity to submit an addendum. Your campus coordinator can tell you more about that if you believe it may be relevant for you. Complete Addendum if information changes between April and June.

3.2 AMCAS • You must complete the AMCAS application if you are requesting promotion.• Deadline: April 15, 2019• The AMCAS application fee is waived for all partnership program students who apply before the

deadline.• Be aware, it is time consuming and you will need to allow several hours to complete it.

o You can work on your application in multiple sessions.

3.2.1 Completing your AMCAS Application Information on where to go to complete your application and resources to help you with the process will be sent to you annually at the start of the process. The AMCAS application and the Petition for Review are done at the same time.

• Complete all required fields of the AMCAS application, including:o Biographic informationo Entering coursework

• Coursework must be entered from all colleges.• It includes current and planned coursework.

It is helpful for you to have an unofficial copy of your transcript in handfor reference when completing the application.

3.3 NEOMED Accounts Everyone in their 2nd (B4 or PB2) or 3rd (B5 or PB3) year will receive an email from NEOMED that has your login information for your NEOMED accounts – Banner Self-Service (our web portal) and email. These will be necessary to complete some of the tasks before you matriculate, such as the financial aid application, which must be done before you complete your Petition for Review. Receiving this information does not guarantee that you will matriculate to NEOMED the following year, but rather it helps you complete some of the early matriculation tasks should you be granted promotion by the Committee.

Individualized Couse Plans

All Post-Baccalaureate Students are required to have an up-to-date Individualized Course Plan (ICP) on file with the Partnership for Urban Health advising office at Cleveland State University. The ICP will be

reviewed annually as part of the Phase I CAPP continuation and promotion process for post-baccalaureate students.

Upon acceptance of the offer of admission, all post-baccalaureate students are required to meet with their Partnership for Urban Health academic advisor at Cleveland State prior to initial course registration well in advance of starting their first semester in Phase I. A copy of the ICP will be sent by the academic advisor to the NEOMED faculty coordinator no later than June 1st prior to the start of the fall term of initial enrollment for approval. Any subsequent proposed changes to a student’s ICP must be sent to the NEOMED faculty coordinator for approval prior to a student enacting any class schedule changes. Any unplanned deviation from an ICP may result in an invitation to the Phase I CAPP and may result in loss of provisional admission.

Students must receive a letter grade in at least one science (3 credit hours or more) course and one (1) Urban Health Concentration course (or other approved course) at CSU for fall and spring semesters.

GPA Selection Options and Requirements

Post-Baccalaureate GPA Calculation Method

• Overall GPA and BCPM GPA (Biology, Chemistry, Physics, Math) includes transferred courses, and fall and spring semesters. Any summer grades will be included in Overall GPA and/or BCPM GPA calculation during PB2 CAPP review.

• Overall GPA includes all required and elective coursework for which the partner university gives credit from admission to the program through Spring Semester of PB2. It excludes courses that were (a) not completed during and (b) are not required as part of the M.D. Partnership program or the student’s bachelor’s degree.

• BCPM GPA is computed by the method employed by the AMCAS system of the Association of American Medical Colleges.

• In the case of repeated course work, both the original and the repeated grade are included in GPA/BCPM calculations. Institutional course forgiveness policies shall not apply. Post-Bacc students may select which portion(s) of their previous coursework they wish to apply toward promotion requirements, on approval by the program.

• Post-Baccalaureate students transferring course credit from another institution will have an Individual Course Plan (ICP) that identifies GPA benchmarks that may differ from the 3.25 PB1 requirement. For instance, a student transferring in a low GPA may have to reach a GPA of higher than a 3.25 in order to matriculate to NEOMED at the end of PB2. The ICP will be facilitated by the Partnership Student Coordinator/Advisor and approved by the Phase I CAPP CSU Subcommittee before the student enrolls in the M.D. Partnership.
