0. Introduction to the HRA report

The Health Risk Assessment (HRA) report is used by PDO to process and store health related

information.

The report starts with a description of the scope of the HRA and the persons involved in sections

1 and 2. The remedial actions from the HRA are presented in report section 3, and are sorted

by priority. Section 4 provides information from the detailed appraisals. The focus here is on

High and Medium RAM rated risks. Section 5 lists the status of the Low RAM rated risks that

were reviewed during the HRA. Section 6 provides an overview of generally applicable controls

and recovery measures. The report concludes with definitions and a diagram of the RAM risk

matrix in appendices 1 and 2. Appendix 3 contains an overview of the detailed risk appraisals

and their page numbers.

1. HRA Scope

Crude oil production flows from (gas lifted and ESP) wells from various fields. The production is gathered and sent via bulk and test lines to the LPS. At LPS, the production fluids flow through six bulk separators V-2611/21/31/51/61/99 to separate gas from a mixed oil / water stream. Crude oil and water from the bulk separators flow to the dehydration tanks T-2613/23/38 to separate water from oil before export. The separated crude oil is pumped to Fahud North Oman crude stabilisation (NOCS) plant. Recovered natural gas liquid (NGL) is spiked into this dehydrated oil (to Fahud). The gas from the bulk separators is compressed to approx. 7200 kPa. The Hydrogen Sulphide (H2S) content of the gas is reduced in the Amine GSU unit from 500 ppm to less than 5 ppm and the gas dew point ( C) is reduced by refrigeration and diethylene glycol (DEG concentrate 85% lean and 7 % rich) injection in Gas Conditioning units for “Sweet” and “Sour” gas. The gas is used for gas lift and export to Yibal. Condensate from the lift gas compressor scrubbers and the gas conditioning unit is partially stabilised in the NGL recovery system. AGMP AP Gas recovery compressor was installed to minimize AP gas flaring. The AP gas flared via AP flare is recovered through Vapour Recovery Unit (VRU) to recover and utilizing AP gas, and thereby meeting PDO flare emission policy. The LPS provides water for injection wells to sustain the injection gradient and to maintain the

reservoir pressure. Water is supplied from the FARS aquifer to water injection facility in LPS

where it is de-aerated and pumped through high pressure Fars water ring main system.

Produced water from the dehydration tanks is used for dehydration water re-injection for wells

at A-North, Lekhwair B & C. FARS water is used for injection at Ufuq, Malaan, Dulaihma and

LEK –E.

Principal Production Operator and helper Accountabilities are:

Monitor all process conditions and alarms via DCS in CCR.

Perform regular facility surveillance and inspections incorporating the following: a) Routine inspections of escape routes to ensure their integrity and to ensure

they are free of obstruction. Report any damage/obstruction in SAP and to ONO/14L / 15L

b) Visual inspections of tank bund walls. Report any damage/defects in SAP and to ONE/2 ONO/46L.

Perform first line maintenance of sight glasses/level indicators.

Ensure all personnel are provided with adequate and appropriate PPE in accordance with procedures/requirements and PTW system including personal H2S detectors. Ensure adequate SCBA is supplied for rescue teams in event of incident.

Monitor fired heater start-up sequence to ensure safeguarding system is functioning as per design intent

Maintain access control requirements for all facilities including vehicle specification and ignition control. Restrict vehicle/personnel access to site through PTW system.

Maintain the overrides log book for the station.

Ensure head count of personnel is conducted at muster areas in the event of station evacuation

Report missing persons to LECB and initiate search and rescue in accordance with company ERP .

Perform spot checks on PTW holder cards to ensure permit conditions are implemented.

Stop all work not complying with PTW forms

Verify through spot checks that PTW forms are completed correctly.

Ensure equipment is mothballed in accordance with Company procedures.

Verify that all as-built drawings of facilities are up-to-date through ongoing contract with EMC provider.

Ensure that as part of Facility Change Procedure all POMs are updated to reflect changes to operations and facilities.

Ensure all modifications go through PDO FCP authorisation process.

Ensure that appropriate technical safety and discipline engineer input is obtained for all FCP including ensuring appropriate HEMP processes are followed. (Eg design review, HAZOP/IPF etc

All FCPs should be evaluated to determine the impact of proposed changes on the HSE Case.

Ensure that FCP procedures include updating POMs, MCPs and as built P&IDs.

Verify that all FCPs are subject to multi-disciplinary review.

Verify that all relevant discipline authorities (up to CFDH) sign off on FCP appropriate to level (M/I/E).

Principal Production Coordinator and Area Supervisor Accountabilities are:

Review the LO/LC register for its accuracy and completeness.

Ensure LO/LC register is updated to reflect current operating conditions

Establish and provide speed limit sign boards inside process facility and hydrocarbon area.

Review request for operation variance waiver.

Verify that all operation variance waivers are approved by Appropriate authority (e.g. CFDH).

Monitor operation variance waiver to ensure it does not extend beyond prescribed time limits.

Make safe well location and prepare handover certificate for well engineering.

Ensure surface equipment is isolated and depressured prior to handover to well engineering.

Ensure operations are performed within the operating envelope of the facilities in accordance with design specification.

Review, evaluate and approve requests for temporary overrides of overpressure instruments and ESD valves.

Ensure that clean-up and remediation plans of Hydrocarbon spillage are developed for credible scenarios.

Ensure LPG loading procedures are kept up-to-date and followed for Lekhwair operations.

Ensure all personnel are aware of the procedure for reporting oil spills in the field. All oil spills should be reported to the LEBC in accordance with company procedures

Ensure weather reports are communicated to all stations and are displayed for employee information

Ensure that all employees are familiar with POMs and that tasks are carried out as per job specific procedures

Ensure that all personnel have undertaken PTW training and passed the area interview prior to undertaking activities under PTW system, inclusive of Confined Space Entry.

Ensure that PDO personnel and visitors complete the Site induction.

Verify that PTW applicant/holder are authorised for the specific activity prior to permitting operations to commence.

Provide adequate levels of supervision for HSE Critical Activities. Ensure that all less than competent personnel (i.e. trainees, new employees) receive adequate induction training and coaching before and/or while undertaking HSE Critical Activities. Ensure all contractor operations are suitably supervised and monitored by PDO staff.

Verify through PTW authorization process that an adequate lifting plan has been prepared

Verify through site supervision and spot checks that the job HSE Plan for heavy lifting over hydrocarbon inventories has been implemented

Appendix 1: Definitions Acute Health Effect Health Effect that occurs immediately or a short time (seconds to hours) following exposure, such as heat stroke, chemical burns and asphyxiation. Agent See Hazard ALARP - As Low As Reasonably Applicable The point at which the cost (in time, money and effort) of further Risk reduction is grossly disproportionate to the Risk reduction achieved. Barrier A Risk Control or a Recovery Measure. Barriers provide the means of preventing an Event or Incident, or of mitigating the Consequences. A Barrier can be an item of equipment or a human intervention, and can also be a Control on an Escalation Factor. Chronic Health Effect Health Effect that occurs gradually over a long period of time (months or years), generally following repeated and prolonged exposure. In specific cases a short term exposure may result in a chronic health effect. Consequence An Effect on people, assets, the environment or reputation as a result of a Hazard being released. Control A means of preventing an Incident. Controls can be engineering, procedural or behavioral. Critical Activity An activity necessary for the development, implementation or maintenance of a Barrier established for managing Hazards with RAM red Risks. Effect The Consequence of the release of a Hazard. Escalation Factor A situation, condition or circumstance that may lead to the partial or full failure of a Barrier. Event An Incident that occurs when a Hazard is released. Exposure Limit The airborne concentration of chemical agents and levels of physical agents to which nearly all workers may be repeatedly exposed day after day without adverse Effect. Exposure Rating The chance of over-exposure to a health hazard when compared against agreed control standards. The exposure rating is evaluated as Very Low, Low, Medium or High. Hazard An agent with the potential to cause harm to people, damage to Assets, or an Impact on the environment or reputation. Health Hazard A factor of a physical, chemical, biological, ergonomic or psychological nature with the potential to cause harm to the health of people. HRA - Health Risk Assessment A process of identifying, evaluating, controlling and managing Health Risks, associated with work to prevent acute and chronic health Effects.

Incident An unplanned Event or chain of Events that has, or could have, resulted in injury or illness or damage to Assets, the environment or reputation. Likelihood The probability that a specified Consequence will happen. OEL – Occupational Exposure Limit The airborne concentration of chemical agents and levels of physical agents to which nearly all workers may be repeatedly exposed day after day without adverse effect. RAM - Risk Assessment Matrix A tool used to assess the Consequences and Risks of Hazards to HSSE. The results are referred to as a RAM Severity rating and a RAM Risk rating. Recovery Measure A Barrier that reduces the Consequences of the release of a Hazard as a result of an Incident. Recovery Measures can be engineering, procedural or behavioral measures. Remedial Action Plan RAP - A Plan to implement corrective action items. Risk The combination of the Consequence of a specific Hazard being released and the Likelihood of it happening. Risk Assessment The process of identifying the Consequences of the Worst Case Credible Scenario arising from the release of a Hazard, and estimating the Likelihood of that scenario. Severity A level of Consequence on the RAM on a scale of 0 to 5. TBD To be determined.
