PETITION FOR APPOINTMENT AS NOTARY PUBLIC

Petitioner, RALPH DAVID D. SO (Petitioner), respectfully states:

1.This is a Petition for Application of Notarial Commission as Notary Public under Rule III of the 2004 Rules on Notarial Practice (Supreme Court Administrative Matter No. 02-8-13 SC), which has been fully read and understood by Petitioner.

2.Petitioner is a Filipino, single, of legal age, over twenty-seven (27) years of age, born on 28 May 1984 in Manila.

3.Petitioner desires to be commissioned as Notary Public in and for Quezon City.

4.Petitioner has been a resident of the Philippines since birth, and presently resides at 7126 Sunny Villas, Pearl St., cor. Camaro, Fairview, Quezon City where he also maintains an office, with telephone number 427-8900, under the jurisdiction of this Honorable Court.6.Petitioner satisfactorily completed the required course of Bachelor of Laws (Ll.B.) at San Beda College of Law, Manila, on April 2010.

7. Respondent passed the 2010 Bar Examinations and has since been admitted to legally practice law in the Philippines, with Roll No. 59532.8.Petitioner is a registered member of the Integrated Bar of the Philippines (IBP) and has duly paid his dues with the IBP for the current year as evidenced by Petitioners Official Receipt No. 859631 dated ________, a photocopy of which is attached as Annex A and made an integral part hereof.

9.Petitioner has also paid his professional taxes to the Manila City Government as evidenced by Official Receipt No. 9703503 dated 27 April 2011, photocopy of which is attached as Annex B and made an integral part hereof. 10.Petitioner is a member of the Philippine Bar in good standing and is duly authorized to practice law under the laws of the Republic of the Philippines, as evidenced by the following:

a.Certificate of Admission to the Philippine Bar dated August 17, 2011, issued by the Office of the Bar Confidant of the Supreme Court, a copy of which is attached hereto as Annex C and made an integral part hereof;

b.Certification duly issued by the Supreme Court dated August 17, 2011, attesting that there is no pending administrative/disbarment case filed against Petitioner, a copy of which is attached hereto as Annex D and made an integral part hereof;

c.Certification from the IBP dated ______ stating that there is no pending administrative/disbarment case against Petitioner, a photocopy of which is attached herewith as Annex E and made an integral part hereof.d.Clearance from the Office of the City Prosecutor, attached hereto as Annex F;e.Clearance from the Regional Trial Court of Quezon City, attached hereto as Annex G;f.Clearance from the Municipal Trial Court of Quezon City, attached hereto as Annex H;g.Certification from the Integrated Bar of the Philippines, Quezon City chapter, attached hereto as Annex I.12.Petitioner attaches herein as Annex J, and makes an integral part hereof, three (3) 2x2 photographs of the Petitioner, taken within thirty (30) days from the date of the filing of the instant Petition.

13.Petitioner further submits his specimen signatures, attached herewith and made an integral part hereof as Annex K. 14.Petitioner also submits true impressions of his dry seal, attached hereto as Annex L and made an integral part hereof.

15.Petitioner respectfully manifests that he possesses all the qualifications and none of the disqualifications for the approval of his notarial commission.

PRAYER

WHEREFORE, it is respectfully prayed that herein Petitioner be granted appointment as Notary Public for and in Quezon City, Philippines.

Other just and equitable reliefs are likewise prayed for.
