Bysigningthiscontractyouagreetothefollowingtermsandconditions:

1. I,theowner,willundernocircumstanceabandonmypet.IagreethatIwillpickuptheanimal(s)onthepre-arrangedpickupdate.Dropoffcanbepre-arrangedaswell,thiswillserveasthepickupdate.IfIamunabletodoso,IwillmakeeveryefforttocontactAdoptMeCanada/CrazyCatLadypetsitting(hereinknownasthesitter)toarrangeanalternatepickupdate.24hoursnoticeisrequiredtoextendastay.

2. Iamawarethatafter30dayspastthepickupdateIhavenotclaimedormadecontactinregardstocollectingmyanimalfromthesitterthatIwillbeheldliabletopaya$1000surrenderfeeplusalllegalfees,expensesforcareandmedicalneedsincurredbymypetduringtheirstay.Iacknowledgethat

mymailingaddressprovidediscorrect.Onthe15thdayafterthepickupanoticewillbesenttotheowneradvisingthemattheanimalisstillinthecareofthesitterandmustbepickedupassoonaspossible.

3. Iagreetohavemyanimal(s)checkedoveratcheckintoensuretherearenomedicalissues,openwoundsorothervisibleinjuriestotheanimal.Itisthediscretionofthesittertoconfirmthatanimalisingoodhealth,unlessotherwiseadvisedandpre-arranged.

4. Iagreetocoverallmedicalexpensesincurredduringmyanimalsstay.Thesitterwillmakebesteffortstocontactboththeownerandtheemergencycontacttoapproveanyveterinaryvisitsrequired.IfthereisseriousinjuryortheimmediateneedtomedicalattentionIagreetohavetheanimalseenand

treatedbyavetofmychoosingatmyexpense._________________(Initialhere)5. 6. Iagreetoreimbursethesitterforanyandallexpensesincurredduringmypetsstay.Thisincludesfood,veterinaryvisits,medication,andanyothersuppliesneededthatIhavenotprovided.

7. Iwillnotholdthesitterresponsibleforanyinjuriesordamagesasaresultofmyanimal.

8. Intheeventofthedeathoftheanimal,Iwillnotholdthesitterliable.9. Ifmypetrequiresanyspecialcare,Iagreetoprovidethatinformationinwriting.

10.Iagreetoprovideanyandalllifesustainingsupplies,suchasmedicationsandothertreatmentstothesitteratthetimeofcheckin,aswellasascheduleforanymedicationstobeadministered.

Allmedicationsandsupplementsmustbeprovidedbyowner,pleaseprovideuswithaschedule.Pleasebeadvisedthatanadequatesupplymustbeprovidedtolast

thedurationofthestay.ForSubcutaneousfluids,allsuppliesmustbeprovided.Thisincludes:fluids,fluidlineorsyringe,andanyotherrelateditems.

PaymentTerms1. Servicefeesaredueattimeofcheckin,withtheexceptionoflongterm

stays.Forlongtermstays,thefirstmonthsservicefeeisdueuponcheckin

2. IfwearepickingupyouranimalwerequirefullpaymentfortheirstayviaEMT(Emailmoneytransfer)priortopickup.Forlong-termstays,werequireservicefreesforthefirstmonthtobeprepaid.

3. Printedinvoiceswillbegivenuponreturn/pickupoftheanimal.Monthlyinvoicescanbeemailedtotheowneruponrequest.

4. Paymentmustbemadeinfullforshorttermstays.5. PaymentwillbeacceptedviaEMTorcashonly.6. Ownerwillpayallmedicalcostsifillnessoccursorinjury.Ownerswillbe

contactedpriortovetvisitsunlessthereisasevereemergency,whichrequiresimmediatemedicalattention.

7. Ifownerdefaultsonpayment,theanimalwillbekeptincarefor30days.After30daystheownershiprightsareforfeitedifpaymentisnotreceived.Ownerswillbechargedfortheextradays

8. Servicefeesforlongtermstaysaredueonthe30thofeachmonth.Outstandingbalanceswillbechargedinterestattherateof20%perannum.

9. Forlong-termstays,ownerswillbebilledonamonthlybasis.
