I grant permission for my child to leave DIAs campus after each examination session.

I acknowledge that Dubai International Academy is not responsible for my child once he/she leaves

the campus.

I understand that I am responsible for transporting my child to AND from school as per their

examination schedule.

Procedure for Granting Leave

1.

Student and parent to read, print and sign the above Permission Leave Form.

2.

Permission Leave Form will be given to Mr. Alvaro, Mr. Luce or Ms. Hager either on the day

of the examination (DP2) or to their homeroom teacher (DP1).

3. Upon turning the Form in, students will receive an Exit Pass with their picture. Students are

to show this to security in order to leave campus. In no case will students be allowed to

leave DIAs campus without an Exit Pass, regardless of circumstance.
