PAST AND PRESENT CHALLENGES OF THE TOTAL REHABILITATION

– The importance of total rehabilitation; – Evolution & history of the MALO protocol; – MALO protocol diagnosis; – What is All-on-4 ®? – All-on-4 ® treatment concept; – All-on-4 ® Standard; – All-on-4 ® Hybrid; – All-on-4 ® Double-Zygomatm; – All-on-4 ®Extreme; – The principles, advantages, objectives and biomechanics of the

All-on-4®. Treatment Concept; – Evolution of this Treatment Concept to overcome past and present

challenges; – New implant products developed to overcome extremely complex

cases and recognize its clinical applications.

This session will briefly review the objectives, as well as the evidence-based principles of the All-on-4® Treatment Concept to restore complete edentulous and failing dentition cases, followed by the discussion of the most recent advancements that this technique has gone through.

The new products developed to overcome the challenges of restoring the atrophic maxillae will also be presented, comprehending a wide range of solutions since zygomatic to short implants, along with other implant design innovations, emphasizing its importance to the success of this treatment and providing clinical examples to its application.

MAURICESALAMA Developer of the latest protocols in reconstructive surgery and bioengineering.Leading Scientific Expert of the DentalXP (Dental Education Site).A member of the multidisciplinary Team Atlanta, together with Dr. Goldstein and Dr. Garber

SYNERGY OF RECONSTRUCTIVE HARD AND SOFT TISSUE SURGERY: THE ROLE OF BIOENGINEERING IN CLINICAL PRACTICE

– Diagnosis and classification of implant recipient sites using CBCT and 3D software.

– Augmentation techniques available at the time of extraction. – Management of the “deficient” site through horizontal and vertical

augmentation techniques. – Evaluation of new techniques and bioengineering technologies for future

applications.

Biologic Modifiers can often be synergistically combined with periodontal plastic surgical techniques, guided bone regeneration and various osseous grafts to effectively establish the optimal foundation for functional and esthetic implant restorations. Vertical soft tissue and interdental papilla enhancement is frequently combined with innovative second-stage periodontal plastic surgery to create an ideal restorative frame.

The three-dimensional reconstruction of the implant receptor site comprises three distinct phases:

– Development of the hard tissues, – Reconstruction of the soft tissue, – Shaping of the tissues through the restorative profil.

DIRK DUDDEСK The author of the most famous dental implant cleanness research in the world. He heads the Clean Implant Foundation, a worldwide initiative whose focus is investigating the quality of dental implants

– The effect of implant composition on the occurrence of peri-implantitis.

– A new standard in implant composition. – Differences in the quality of implants

The surface of dental implants determines the initial phases of the biological response to the implant and affects its ability to integrate into the surrounding tissue.

In four consecutive studies since 2007 more than 300 different, sterile-packaged dental implants were analysed by scanning electron microscopy and elemental analysis (EDS). Nearly 50% of all analysed implants showed a wide variety of organic and/or inorganic impurities containing iron, copper, tin, chromium, tungsten, antimony and nickel. Additional Time-of-Flight Secondary Ion Mass Spectrometry (ToF-SIMS) analysis revealed polysiloxanes, polyethylene and even hazardous surfactants on sterile dental implants.

PERI-IMPLANTITIS AND CLAENESS OF IMPLANTS. RESULTS OF THE LARGEST INDEPENDENT RESEARCH

MARISA RONCATI Оne of the most respected world experts in the non-surgical treatment of peri-implantitis. Author of 3 books on non-surgical and surgical treatment of peri-implantitis

PERI-IMPLANT DISEASES: NON-SURGICAL PERIODONTAL APPROACH

1. Non surgical management of mucositis/peri-implantitis;2. Peri-implant subgingival calculus detection;3. Effective teaching of oral hygiene skills and professional

maintenance care in case of implant supported prosthesis;4. Professional Comprehensive Subgingival Debridement:

– Manual instrumentation with titan universal curet, – Mechanical instrumentation with plastic or modified

ultrasonic inserts, – Air-flow devices;

5. Cause-related laser-assisted nonsurgical periodontal therapy;6. Step by step: Professional prophylaxis appointment in case of peri-

implant tissue health, Nonsurgical protocol in case of mucositis and in case of Peri-implantitis;

7. Antimicrobials and Antibiotics;8. Peri-implant Maintenance.A particularly emerging topic will be addressed: peri-implant diseases.

The non-surgical treatment has proven quite successful in case of mucositis. Vice versa, in case of progressive and irreversible bone loss, the non-surgical solution is not predictable, however, it is recommended as a first therapeutic phase. In some cases, clinically well documented, the non surgical option may have very satisfactory long term outcomes. In other cases can help to prolong significantly the survival rate of severely compromised implants.

It is essential that non-surgical treatment is all the time specific and tailored to the needs of each individual patient. The protocol, particularly composite and differentiated, associated with the use of diode laser, is described and illustrated with clinical images, drawings and videos.

The long-term success of any dental and peri-implant treatment, carried out in a proper manner, depends essentially on the patient’s degree of cooperation, both in adhering to the recommended recall schedule, and in daily proper home care.

LUIGI CANULLO Clinical researcher in the field of etiology and treatment of peri-implantitis. For its research and scientific publications has received many prestigious international awards

PERI-IMPLANTITIS: UPDATED UNDERSTANDING OF TRIGGERING FACTORS AND ETIOLOGY

– Different etiology and risk groups of peri-implant pathology; – An individualized treatment plan addressing specific risks for peri-

implantitis; – The role and efficacy of surgical approaches to peri-implantitis; – Resective approach; – Regenerative approach; – Advantages and disadvantages of clinical activities in order to

avoid peri-implantitis and obtain a long-term stable clinical result.

Within traditional beliefs of peri-implantitis, plaque accumulation is the primary etiologic factor. A more modern approach looking beyond symptomology using stratification of peri-implantitis cases into risk groups has elucidated that, in most of cases, bacterial superinfection seems to be secondary. More important factors for triggering peri-implant disease seem to be deficiencies in implant placement or their prosthetic restoration. Knowledge of these contributing factors will substantially change not only the treatment plan, but also the prognosis of peri-implant disease. The aim of this presentation is to demonstrate the do’s and don’ts of good clinical practice to avoid peri-implant pathology and obtain a longitudinal stable clinical response of the peri-implant tissues.

ALGIRDAS PUISYS MODERATOR OF THE CONGRESS

Leading Implantologist and Periodontologist of Lithuania. Vice-President of the Lithuanian Association of Periodontists

– Dental implants for patients with periodontal disease; – Factors, influencing periimplantits development – Periimplantitis treatment algorithm – when and which treatment

decision to take?; – Non-surgical treatment and its possibilities; – Implantoplasty and bone regeneration possibilities; – Implant extraction; – Prevention of periimplantits.

Nowadays, implantology develops rapidly so we are in a very intriguing stage of discoveries, when new implant placement protocols as well as bone and soft tissue management are being invented or improved existing ones. Even though the new treatment modalities and inventions lead to better prognosis, the main problem remains – possibility of development of periimplantitis.
