Is this really a thing? Does drug-induced nutrientdepletion actually exist? Actually, it does occur.Certain prescription medications can deplete yourbody of important vitamins. This is not somethingthat comes up during the usual dialog about med-ication side effects. Luckily, this is changing. YourPeoples Drug Mart Pharmacist is always learningand passing this informationalong. This month we shallhighlight some of the most im-portant drug-induced vitamindepletions, so you and yourpharmacist can work together to help you stay healthy. How can a prescription med-

ication reduce vitamins in the body? There are afew ways this can happen. The first is that certainmedications might decrease the absorption ofnutrients from your diet. Some medications areintended to reduce stomach acid production. Vitamins and minerals that require stomach acidfor absorption might be affected. Certain nutrientsmight be removed from the body as a side effectof a medication. Diuretics (water pills) make thekidneys work better at removing sodium from thebody, but this action also removes certain vitamins. Other medications might change yourbody's metabolism and increase the utilization ofcertain vitamins. Some of these drug and nutrientinteractions might not be monitored. This is whyit is important to ask your Peoples Drug MartPharmacist about your medications.

Many people who have high blood pressure aretaking a diuretic (water pill) of some sort. Thesemedications work very well and generally have fewside effects. Your doctor and Peoples Pharmacistare aware of these medications ability to alter theamount of potassium in your body. However,some diuretics can also deplete your body of mag-

nesium, calcium and thiamin(vitamin B1). A lack of magne-sium might lead to muscleweakness, stiffness, constipa-tion and osteoporosis. Calciumis very important for bonehealth. Thiamin is very impor-tant for heart function and en-

ergy production. If you are taking a diuretic, thismight be a good time to talk to your Peoples Phar-macist to see if this depletion might affect you.Sometimes all it takes is to take extra vitamin B1and ensure you are getting enough magnesiumand calcium in your diet.Another group of medications that can cause

nutrient depletion are those which reduce the pro-duction of stomach acid. These medications in-clude the proton pump inhibitors (PPI) and H2receptor blockers. Ask your Peoples Pharmacist ifyou are taking these medications and if it could af-fect your nutritional status. While these drugs workwell to reduce the symptoms associated with ex-cessive stomach acids, certain nutrients requirestomach acid for absorption. The most notable arethe minerals calcium, iron and zinc. Normally,

Chronic bronchitis and emphysema are together called chronic obstructive pulmonarydisease, or COPD. This is a chronic conditionwhere not enough air enters or leaves thelungs.In chronic bronchitis, the airways (or

bronchi) that connect thewindpipe and the lungs be-come inflamed and swollen.The airways become narrowand are clogged up withthick mucus, called phlegm.Chronic bronchitis may befound together with emphy-sema, in which destructive changes of the airsacs in the lungs cause them to become larger,reducing the surface area where oxygen exchange takes place. Both diseases make it difficult to breathe.It's a very common condition, especially

among people who smoke. More than 700,000Canadians have COPD. In the United States,about 12 million are affected with this condi-tion. COPD is the fourth leading cause of deathin both Canada and the United States. Morewomen than men have COPD, except in theage category above 75.CausesSmoking is the main cause of COPD. It causes

the airways to produce excess mucus that linesthe walls of the airways, making the air passagesvery narrow. This makes it easier to get abronchial infection. An infection can causeeven more damage to the airways by causingmore mucus production.

Less common causes ofCOPD include a rare geneticdisorder called alpha-1 antit-rypsin deficiency, air pollution,exposure to occupationaldusts and chemicals, and frequent lower respiratory infections during childhood.

Symptoms and ComplicationsPeople with chronic bronchitis may cough up

phlegm almost every day.It is common for someone with chronic

bronchitis to persistently cough and wheezewhen breathing. It is also common to feel shortof breath. Low oxygen in the blood due to thedecreased ability to diffuse oxygen across theair sacs may cause the lips or fingernails to become bluish in colour.COPD can lead to heart failure, as the heart

has to work harder to pump blood into thelungs. When the heart fails to pump bloodproperly, it collects in the blood vessels of thelegs and ankles and causes them to swell - this

is called edema.Sometimes you may become housebound

because of breathing difficulties, even whendoing simple tasks such as getting dressed orwashing.

If you have COPD, it is likely that you maycatch one or two infections every winter. Youmay also occasionally cough up blood. If thishappens, it may be a sign of a more seriousproblem and it's important to see your doctor.Making the DiagnosisYour doctor will test to see how much air youcan forcefully exhale in one second. These pul-monary, or lung breathing, tests are simple andpainless. If you exhale less than normal, yourairways are inflamed, in spasm, or clogged upwith mucus. If this persists, then you may have COPD.

Treatment and PreventionEven with treatment, COPD often becomes

progressively worse. The only treatment to slowthe progression of COPD is stopping smoking.Lung function deteriorates with age, and ithappens much faster if you're a smoker.

Your doctor may prescribe medications calledshort-acting bronchodilators, including salbu-tamol*, ipratropium bromide, or a combinationof the two, which relax and widen the bronchiand help relieve shortness of breath.

If symptoms are persistent, treatment withlong-acting bronchodilators such astiotropium, salmeterol, or formoterol can beadded. If there is any inflammation present (notas common in COPD as in asthma), your doctormay suggest that you try inhaled or oral corticosteroids to help with breathing.

There are also medications available that com-bine long-acting bronchodilators with inhaledcorticosteroids. Your doctor may also prescribeantibiotics for you to keep at home in case abacterial lung infection develops.

Since influenza (the flu) may make COPDsymptoms worse and can lead to respiratory failure, it is recommended that people withCOPD receive the annual flu vaccine. Some peo-ple with COPD may also benefit from receivinga pneumococcal vaccine to lower their risk ofgetting pneumonia (lung infection), which canalso lead to complications. Talk to your doctor about receiving these vaccines.

Inhaling oxygen from oxygen cylinders or anoxygen concentrator for at least 15 hours a daymay also be helpful for some people withCOPD. Finally, drinking plenty of fluids through-

out the day can help loosen phlegm buildup.Exercise with or without a formal physiother-

apy program can improve a person's quality oflife and activities. A healthy nutritional intake isimportant, as weight loss due to the increasedwork of breathing presents a serious sign of advancing COPD. Lung reduction surgery orlung transplantation can also be considered inextreme cases.

stomach acid converts these minerals into a formthat is easily absorbed from the stomach. It is gen-erally known that long term use of these medica-tions, especially PPIs, can cause an increased risk ofosteoporosis. One vitamin that does require stom-ach acid for absorption is vitamin B12. Long termvitamin B12 deficiency can lead to neurologicaldamage and anemia. If you are taking these med-ications you might want to ensure that you arealso taking extra of these important vitamins andminerals. In the case of calcium and zinc, choosethe citrate from as it does not require stomachacid for absorption.One drug that can also affect the absorption of

vitamin B12 is metformin. Metformin is a pill thathelps to treat diabetes. It appears this medicationblocks your bodies ability to absorb vitamin B12from the intestines. If you are taking metformin,you might want to take extra doses of this impor-tant vitamin. Doses of 1000mcg a day are safe formost people. Ask your physician to check your vitamin B12 levels from time to time. A lack of thisvitamin can cause nerve problems. Certain medications that help to treat epileptic

seizures can also reduce the amount of importantvitamins in your body. The drugs phenytoin, car-bamazepine, phenobarbital and primidone couldcause your liver to metabolize more vitamin Dthanusual. This reduction in vitamin D levels can reduce calcium absorption and lead to a greaterrisk of osteoporosis. It might be a wise idea to takeextra vitamin D if you are using these medications.These anti-seizure medications can also reduce theamount of folic acid in the body. This could leadto an increased risk of anemia. You should checkwith your doctor or Peoples Pharmacist before tak-ing extra folic acid. Being a naturally curious person has served me well overthe years. Curiosity has engaged me in lifelong learning ad-ventures that included taking on university at the age of 50.In spite of huge doubts as to the probability of the educa-tional adventure turning out well, I am happy to report thatwhat began as a 1 year trial period, turned out to be a 4 yearride resulting in a Social Work degree. It was a bumpy ride,but achieving Dean’s list status upon graduation far ex-ceeded any expectations I had originally imagined. Year onewas the hardest� relearning how to learn was like walkingthrough quick setting cement. Studying was tedious and ittook a tremendous amount of time and effort. Perseverancewas key and by year four the neurons were firing muchquicker and my cognitive functions were noticeably fitter.Use it or lose it had pretty much been lip service to me butexperiential learning now solidifies it as truth. My brain hasbeen forever altered and I mean that in the best of ways.

Neuroscience has become fascinated with the brain andthe aging baby boomer population is good incentive for re-searchers to study ways to keep cognitive functions healthyas we age. We used to believe that the brain was certain todeteriorate once we turned the corner into middle age butscience is now touting that brain plasticity means the agingprocess can go either way. The brain is just like any othermuscle in the body� it will atrophy without use and it will ex-pand through exercise. Plasticity is not age dependent but itdoes take more of a purposeful focus as the shadow of ouryouthful years lengthens. While enrolling in university is cer-tainly a sure fire way to forge some new neural pathways,there are other ways to exercise the brain to ensure optimumcognitive health.

1. Be a student for life and never stop seeking out new ex-periences and knowledge. You may want to take up a newinstrument or hobby, keep a good book handy or join a clubor an organization that challenges you to new ways of think-ing. You want to fully engage and exercise the brain in a va-riety of new activities.

2. If games are your thing there are benefits to solving jig-

saw puzzles, Sudoku and crosswords. As much as technol-ogy may have a negative impact on our mindfulness andfocus, many online programs are geared at exercises thathelp to strengthen brain functioning and speed.

3. Building models, knitting or playing an instrumentstrengthens the part of the brain involved in fine motor skills.Work with your hands or practice using your non-dominanthand when doing some everyday activities like brushing yourteeth and eating. The extra benefit of eating with your non-dominant hand is that it forces you to eat slower which isfabulous for your digestive processes.

4. Actively practice using your memory. Often what wethink of as a failing memory is just a result of sleepwalkingthrough the day or relying too much on external memorieslike cell phones and computers. Engage the memory as yougo about your day. Rather than just observing the worldaround you - participate in it. What is the name of that tree,flower or bird you observed? Make it your mission to com-mit new names and numbers to memory. Repetition andpractice are important.

5. Engage all the senses and you will engage more of thebrain. Be fully present with your activities and take time toregister all sights, smells, sounds and sensations. When wasthe last time you really sat down to truly savour a meal orstop to take in the wholeness of a flower?

6. Your brain works while you sleep so make sure yoursleep habits are optimal. One of the best things I learnedwhile I was in school is that taking a nap after studying orlearning a new skill can do wonders for helping the brainconsolidate new information.

As with any pursuit of optimal health, success comes whenwe incorporate a variety of strategies. Challenging the brainis as important as getting rid of junk food, feeding the brainnutritionally sound food and getting a daily dose of oxygenthrough moderate exercise. The good news for most of us isthat it is never too late to put on your thinking cap and giveyour brain a good workout.
