There is currently work in children’s services to develop high quality person centred transition plans that focus on outcomes and cover health, housing, employment and community participation.
Where this works well the local health economy and local authority children’s services have
•A joined up clear vision for young people•Clarity about who should attend reviews•Who should bring health information together•Who should liaise with adult health services•And how the young person and their family will: know what will happenHow it will happenAnd who will do what

• Person Centred Planning Team, Transition Team and Education have all worked together to ensure Person Centred School Reviews
• 60 education staff now trained to facilitate Person Centred School Reviews
• Developed joint PCP network between Adult and Children Services
• Year 7 leaflet to introduce transition planning
