There is currently work in children’s services to develop high quality person centred transition plans that focus on outcomes and cover health, housing, employment and community participation.

• 60 education staff now trained to facilitate Person Centred School Reviews

• Developed joint PCP network between Adult and Children Services

• Year 7 leaflet to introduce transition planning
