Los objetivos especfcos de este reporte son:

6) Recomendaciones actuales
AntecedentesVISION2020 es una iniciativa global de la Agencia Internacional para la Prevencin de la Ceguera, cuya misin
es la eliminacin de la ceguera evitable en el ao 2020. Esto requerir el esuerzo coordinado de todos losproesionales de la salud y el compromiso constante de los Ministerios de Salud. Los elementos clave
de VISION2020 son: a) estrategias para el control de enermedades que ms causan ceguera (catarata,
tracoma, oncocercosis, ceguera en nios, errores reractivos y visin baja), b) desarrollo de inraestructura,
provisin y mantenimiento de equipo, c) desarrollo de recursos humanos y d) participacin de la comunidad.
La ceguera secundaria a RP se reconoce como una prioridad para control en Amrica Latina, Europa Oriental
y reas urbanas de Asia.
A nivel global se estima que existen 60,000 nios que son ciegos por RP. Amrica Latina es la regin con el
nmero ms grande (25,000) de casos. Adems, la ceguera secundaria a RP aumentar en India y China
(1/3 de la poblacin del mundo) en tanto mejoren sus sistemas econmicos y se extiendan los serviciosneonatales de cuidados intensivos.
Criterios para programas de tamizaje de RPEn pases industrializados, la poblacin de bebs prematuros en riesgo de presentar RP umbral ha cambiado
con el tiempo, a la echa la retinopata severa del prematuro aecta casi exclusivamente a los bebs sumamente
prematuros (es decir aquellos con peso al nacimiento (PN) igual o menor a 1,000 gramos). En estos pases
se evala slo a los bebs con PN

Indicaciones para el tratamiento de RPBasado en los resultados del ensayo clnico aleatorizado Cryotherapy or ROP iniciado en 1
(CRYO-ROP), la ablacin de la retina avascular peririca usando otocoagulacin con lser y/o crioterap
nios con indicacin de tratamiento para RP mostr una disminucin en la incidencia de ceguera compa
con aquellos no tratados. Un reporte reciente de este estudio mostr que cuando los nios tenan 15 a
casi la mitad de los ojos tratados (44%) tena agudeza visual de 6/60 o peor.
A fnales de 2003, el estudio multicntrico de tratamiento temprano (ET-RP) realizado en EUA docum
que el tratamiento en la ase aguda moderada a severa produca un benefcio signifcativo en los ojos
aleatoriamente se asignaron para recibir tratamiento temprano con lser (por ejemplo en enermedad
umbral de alto riesgo) comparado con el tratamiento tardo (por ejemplo tratamiento slo en enerme
umbral). Los resultados se evaluaron a los 9 meses (agudeza visual, hallazgos estructurales y complicaci
durante el tratamiento). Los hallazgos del estudio ueron que los bebs tratados en etapa pre-umbral te
mejores resultados uncionales y estructurales en comparacin con los bebs tratados en etapa umbral
haba porcentajes ms altos de eectos secundarios oculares y sistmicos en el grupo tratado tempraname
Como resultado de este ensayo se recomienda lo siguiente:
Tratamiento a los bebs con RP pre-umbral tipo 1:
- Zona 1, cualquier RP con enermedad plus
- Zona 1, RP estadio 3 +/ plus
- Zona 2, estadios 2 o 3 con enermedad plus
Incrementar la recuencia de seguimiento de los bebs con RP pre-umbral tipo 2:
- Zona 1, estadios 1 o 2 sin plus
- Zona 2, estadio 3 sin plus
Las implicaciones de este estudio para los programas de RP son que ms bebs necesitarn ser trata
sern tratados en una edad ms temprana y las complicaciones sistmicas como apnea, bradicard
re-intubacin sern ms probables y ms recuentes en los bebs RP pre-umbral tipo 2.
En general, se requiere tratamiento para RP alrededor de las 36-38 semanas de edad postconcepcio
momento en el que muchos de los bebs estn a punto de ser dados de alta del hospital. Es ese
detectar y tratar la retinopata dentro de los dos a tres das despus de su diagnstico o puede dar c
resultado la ceguera. As, cualquier programa de tamizaje debe descubrir y suministrar el tratamiento a e
bebs de manera oportuna.
Importancia del seguimiento de los bebs prematurosLos padres necesitan saber acerca de la posibilidad de prdida visual y la necesidad de seguimie
particularmente una vez que su beb ha sido dado de alta de la unidad de cuidado intensivo o interm
neonatal. El beb necesitar volver despus del tratamiento y continuar un seguimiento a largo p
para valorar errores reractivos, ambliopa y estrabismo. Los bebs prematuros pueden tener deterioro v
secundario a RP as como por lesiones de las vas visuales altas (nervio ptico, corteza visual y reas d
asociacin superiores).

Capacidad actual (infraestructura, recursos humanos y equipo)1. Anlisis de la situacin: Algunos pases tienen una idea clara de la extensin de sus programas de
tamizaje y el grado en el que la necesidad se cubre. Un anlisis debe emprenderse en cada poblacin para
proporcionar inormacin sobre el nmero de unidades en el pas; el nmero y proporcin de supervivencia
de bebs pre-trmino en riesgo por grupo de peso al nacimiento; la capacidad de los programas de tamizaje
actuales, si existen, y qu tanto cubren las necesidades; y el grado de entrenamiento de otalmlogos para
el diagnstico y tratamiento de RP. Esta inormacin es esencial para la planeacin.
2. Incremento de cobertura: Hay muchos desaos en el incremento de cobertura de programas de RP. La
alta de reembolso fnanciero por el tiempo gastado por otalmlogos en el programa es un actor importante.
Adems, deben desarrollarse sociedades con las agencias gubernamentales y rurales. Los pases de
Amrica Latina con programas bien establecidos pueden ayudar a otros pases donde los programas apenas
comienzan.
3. Desarrollo de pautas: Las pautas regionales necesitan ser desarrolladas por neonatlogos y otalmlogos
bajo la ayuda del IAPB, PAHO, PAAO y SIBEN las cuales pueden ser usadas (despus de modifcaciones, si se
requiere) no slo para la aplicacin del programa, sino tambin para la movilizacin de recursos y apoyo.
4. Conciencia y apoyo: Los padres, agencias gubernamentales y proveedores de cuidado de salud deben
tomar conciencia del serio problema que implica la RP en Amrica Latina. Esto puede lograrse a travs
de publicaciones, anuncios de servicios pblicos y otros medios. El material educativo est disponible en
muchos pases y debe realizarse una mejor diusin de ste.
El currculo de enermeras, pediatras, obstetras y neonatlogos debe incluir la inormacin sobre su papel en
la prevencin primaria de la ceguera secundaria a RP. Estas responsabilidades incluyen:
a) Desarrollo de normas mnimas aceptables para la monitorizacin de oxgenob) Inormacin dada a los padres por el neonatlogo con respecto a RP
c) Identifcar y anotar a los bebs que necesitan el examen (quin y cmo)
d) Cuidado durante la revisin en cuidados intermedios e intensivos
e) Asegurarse que los bebs dados de alta y tratados asistan a revisiones
) Cuidado durante el tratamiento de lser o crioterapia
g) Revisin de bebs prematuros por pediatras
h) Recoleccin de datos, supervisin y evaluacin de los programas
El currculo de estudiantes de medicina, residentes y otalmlogos generales debe incluir la inormacin
bsica sobre RP incluyendo el reconocimiento de la enermedad y su tratamiento. En particular, otalmlogosque sirven como los evaluadores y/o tratantes deben estar amiliarizados con el proceso de la enermedad,
tamizaje y tratamiento de RP. Las responsabilidades de los evaluadores y tratantes incluyen:
a) Proporcionar inormacin a los padres
b) Desarrollar criterios de tamizaje en colaboracin con los neonatlogos
c) Determinar la recuencia de exmenes y asegurar el adecuado seguimiento
d) Indicaciones y consentimiento para el tratamiento
e) Mtodo de tratamiento y seguimiento despus del tratamiento
) Reerir a los servicios de visin baja a los nios con el deterioro visual por RP

5. Evaluacin de programas actuales: Cada pas debe determinar la inormacin mnima necesaria
supervisar los programas actuales en trminos de cobertura adecuada y calidad. Una orma estandariz
para reportar el banco de datos usados acilitar este proceso. Tal banco de datos servira como
mecanismo para compartir, discutir problemas y sus posibles soluciones.
6. Manejo de visin baja para los bebs prematuros. Hay servicios de visin baja inadecua
actualmente para nios en Amrica Latina. Los padres son importantes para el xito de la rehabilitavisual de los nios y necesitan ser incluidos como un componente importante, y otros proesionales
ejemplo maestros) tambin necesitan estar involucrados. Muchos inantes prematuros que tienen dete
visual tienen mltiples limitaciones asociadas (discapacidad sensorial, motora y cognitiva) que aumenta
retraso de desarrollo.
Recomendaciones actuales:1) Recopilacin de datos para establecer el estado actual y monitorizacin del progreso
a) Para cada ciudad:
- Nmero de recin nacidos

experimentado. El otalmlogo tratante de RP debe haber observado al menos 10 a 15 tratamientos y
debe comenzar a proveer el tratamiento bajo supervisin experta.
4) Inormacin para los padres de bebs de alto riesgo
La siguiente inormacin debe proporcionarse a todos los padres de bebs en riesgo de RP:
- Descripcin del proceso de maduracin de la retina- Riesgos de la maduracin de la retina en prematurez, enermedades y terapias
- Posibilidad de prevenir la ceguera con diagnstico y tratamiento oportunos, y sus consecuencias
- El tratamiento, incluso cuando es administrado oportuna y apropiadamente, no siempre exitoso
- Importancia de exmenes oportunos, aun cuando el beb se da de alta
5) Criterios de tamizaje sugerido
Los siguientes son los criterios de tamizaje que se sugieren:
- Peso al nacimiento < 1,750 gr y/o edad gestacional < 32 semanas
- Peso al nacimiento >1,750 gr a opinin del neonatlogo
Sin embargo, como el riesgo de RP en cualquier UCIN se relaciona con los resultados de la misma unidad
y niveles de cuidado, cada UCIN puede adaptar su propio criterio de tamizaje. Esto debe hacerse con base
en los estudios longitudinales de por lo menos un ao en los que existan detalles de la edad gestacional, el
peso al nacimiento y los resultados otalmolgicos de bebs vistos en tales unidades.
Es responsabilidad del neonatlogo identifcar a los bebs que deben ser examinados, llevar un registro diario
para determinar cuando se debe realizar la exploracin y notifcar al otalmlogo la necesidad de tal examen
para valorar RP de manera oportuna.
6) Tiempo y recuencia de realizacin de exmenesCuando una estimacin fable de edad gestacional no est disponible, el primer examen debe ser 4-6
semanas despus del nacimiento.
Para los bebs con una edad gestacional estimada fable de por lo menos 28 semanas, los exmenes
deben empezar a las 4-6 semanas despus del nacimiento. Cuando el cuidado neonatal mejora y los bebs
prematuros sobreviven, estas pautas pueden ser modifcadas.

Exmenes subsecuentes:
- Si la retina es inmadura y no hay RP el prximo examen debe realizarse a las 2 o 3 semanas
- Si hay RP en zona 3 el prximo examen debe realizarse a las 2 semanas
- Si hay RP en zonas 1 o 2 el prximo examen debe realizarse cada semana o cada 3-4 das dependie
de la ase de la enermedad y la apariencia de los vasos del polo posterior
- Los exmenes deben continuar hasta que la retina est totalmente vascularizada (dentro de 1 dim
del disco de la ora serrata) o haya retrocedido
Es responsabilidad del otalmlogo decidir cundo realizar el prximo examen e inormar al neonatlog
el beb est hospitalizado o a los amiliares si es ambulatorio, para documentar la echa para la prx
visita.
Si, en la opinin del neonatlogo, el inante es demasiado inestable para llevar a cabo el examen de o
de ojo en el momento sugerido por el otalmlogo, la razn del retraso debe documentarse en el exped
del inante.
7) Procedimiento para el examen otalmolgicoAntes de iniciar la exploracin para deteccin de RP, los neonatlogos deben verifcar el estado cardiovasc
y respiratorio del beb en el UCIN para evitar problemas durante la examinacin.
- Dilatacin pupilar: ciclopentolato 0.5% combinado con enilerina al 2.5% o tropicamida 0.5%. Toda
gotas midriticas deben instilarse por lo menos 30 minutos o 1 hora antes del examen.
- Examen de la retina: Se recomienda el uso de otalmoscopio indirecto con lupa de 28 o 30 D. Instila
de anestsico tpico. El examen debe realizarse por un otalmlogo experimentado en el diagnstic
RP. El examinador debe examinar el polo posterior primero para buscar seales de dilatacin vasc
y/o tortuosidad (enermedad pre-plus o plus), examinar la retina nasal primero y posteriormentemporal para determinar la zona de vascularizacin y el estadio de retinopata presente.
8) Lugar de la examinacin
Si el beb todava est en la UCIN, debe examinarse en la UCIN a la edad postnatal apropiada, ya sea
el beb est en incubadora o con ventilador. Evitar el transporte innecesario del beb que pudiera aume
la morbilidad. Despus de que se da de alta el beb debe tener seguimiento ambulatorio en el consu
del otalmlogo.
9) Aseguramiento del seguimiento de los bebs
En la UCIN se deben registrar direcciones detalladas y nmeros de telono (incluyendo aqullos de padotros parientes) a fn de ser contactados cilmente. Cada esuerzo debe hacerse para asegurar el seguim
oportuno. Esto puede incluir llamar por telono o solicitar al personal asistente o a los trabajadores soc
que contacten a las amilias.
10) Indicaciones del tratamiento
El consentimiento inormado escrito debe ser obtenido de los padres usando una hoja de inormacin
de entender.

El tratamiento se indica para la enermedad pre-umbral tipo 1 (defnicin de ET-RP) dentro de 48 horas,
incluye:
- Zona 1 cualquier estadio
- Zona 2 estadio 2 + plus
- Zona 2 estadio 3
El mtodo de tratamiento actualmente recomendado es ablacin peririca de 360 grados en la retinaavascular usando un lser indirecto y/o crioterapia.
El paciente debe prepararse y supervisarse durante el procedimiento entero por la enermera neonatal,
neonatlogo y/o anestesilogo. El procedimiento debe hacerse en una sala de operaciones o en el lugar de
cuidado apropiado. La opcin de sedacin, analgsico o anestsico general depender de las posibilidades
de cada servicio. La recuperacin post-tratamiento debe hacerse en la unidad de cuidado intensivo neonatal.
Las medicaciones oculares postoperatorias podran incluir las combinaciones de antibiticos y esteroides
durante una semana a 10 das.
Tratamiento de RPEl tratamiento para la RP tipo 1 consiste en ablacin de la retina del avascular anterior a la regin de la
enermedad activa para evitar la progresin de RP. Deben evitarse la cresta y la fbroprolieracin extravascular
en la unin de retina avascular y vascular.
El tratamiento con lser y crioterapia pueden ser procedimientos dolorosos as que los dos deben realizarse
bajo sedacin o anestesia general, asegurando una buena analgesia. Debe evitarse el estrs causado por el
dolor, as como las complicaciones sistmicas. Cuando un anestesista no est disponible, un neonatlogo
experimentado debe dar el apoyo. Si el tratamiento no est realizndose en la unidad neonatal, deben estar
disponibles los medios para la ventilacin artifcial y el equipo de resurreccin. La midriasis debe lograrse
antes de la crioterapia o lser.
A. Tcnica de Crioterapia1) Se puede usar una sonda para crioterapia de retina
2) Indentacin del ojo sin presionarlo en exceso
3) El punto fnal de una aplicacin adecuada es el blanqueamiento espontneo de la retina. Las aplicaciones
de crioterapia deben ser del espacio de una mancha una de la otra ya que tienden a expandirse
4) La crio-sonda debe retirarse del ojo en intervalos peridicos para asegurar perusin de la retina y el
disco ptico
5) El tratamiento de ambos ojos generalmente toma entre 45-90 minutos

Complicaciones potenciales:
- Sistmico: son comunes apnea, bradicardia y desaturacin de oxgeno. Estas complicaciones pue
ocurrir durante los primeros 3 das despus del tratamiento y pueden requerir de ventilacin mecnica.
- Oculares: hematoma subconjuntival, edema de prpados, laceracin conjuntival y hemorragia vtrea.
B. Fotocoagulacin con lser1. Lser de diodo (el ms usado)
- Lser de sistema Indirecto
- 200-500 miliwatts y 0.2-0.5 seg. La retina avascular debe tratarse con una distancia entre disparo
no ms de medio disparo
Complicaciones: como resultado directo de la otocoagulacin se puede desarrollar catarata en 1-2%
El tratamiento de RP muy posterior es generalmente ms cil con otocoagulacin lser.
2. Cuidado post-operatorio
- La medicacin post-operatoria incluye ungento o gotas dos veces al da durante 3-5 das. La ciclop
es opcional.
- El ondo de ojo debe revisarse 5-7 das despus del tratamiento. Si la enermedad plus todava
presente entonces la perieria debe examinarse para determinar si hay o no reas no tratadas. Si
entonces debe aplicarse ms tratamiento.
- Despus de 1 semana se debe reexaminar al beb. Las reas no tratadas deben buscarse, as c
signos de regresin o progresin de RP y/o de enermedad plus. La observacin cuidadosa pa
progresin a desprendimiento de retina se realizar en las semanas posteriores.
- La retina peririca debe retratarse con lser o crioterapia si hay seales de progresin. En gener
retratamiento slo es necesario en las reas no tratadas.
Ojos con DR parciales (estadios IVa y IVb): la ciruga vitreorretiniana puede indicarse en los estadio
o b, pero la decisin para operar y la naturaleza de la intervencin quirrgica necesita ser basado en
valoracin cuidadosa del ojo, y del nio, por un cirujano experimentado, en colaboracin con el anestesi
y pediatra.
No hay consenso acerca del tiempo de tratamiento, ni evidencia de los ensayos clnicos acerca del nm
de intervenciones ptimas.
Estadio V: la ciruga vitreorretiniana compleja no es recomendada actualmente ya que los resulta
uncionales son extremadamente pobres en resultados anatmicos exitosos.

Background
VISION2020 is a global initiative o the International Agency or the Prevention o Blindness, whose missionis the elimination o avoidable blindness by the year 2020. This will require the co-ordinated eort o all stake
holders, and ongoing commitment rom Ministries o Health. The key elements o VISION2020 are:a) strategies
or the control o the major blinding eye diseases (cataract, trachoma, onchocerciasis, blindness in children,
reractive errors, and low vision), b) inrastructure development and supplying and maintaining equipment,
c) human resource development, and d) community participation. Blindness rom ROP is recognised as a
priority or control in Latin America, Eastern Europe, and urban areas o Asia.
Globally there are estimated to be 60,000 children who are blind rom ROP Latin America being the region
with the largest number (25,000). In addition, blindness due to ROP is likely to increase in India and China
(home to 1/3 o the worlds population) as their economies improve, and neonatal intensive care servicesexpand.
Criteria for ROP screening programmesIn industrialized countries, the population o premature babies at risk o threshold ROP has changed over
time, and nowadays severe ROP almost exclusively aects extremely premature babies (i.e. those with birth
weights (BW)

Indications for treatment of sight-threatening ROPBased on results rom the cryotherapy or ROP randomized clinical trial initiated in 1986 (CRYO-ROP), abl
o the peripheral avascular retina using laser photocoagulation and/or cryotherapy when sight-threate
ROP develops was shown to decrease the incidence o blinding disease by more than 43% compare
eyes that go untreated. A recent report rom this study when the children were 15 years old, showed
despite the advantage o improved retinal outcomes, almost hal o the treated eyes (44%) had visual a
o 6/60 or worse.
In late 2003, the multicentre early treatment trial (ET-ROP) conducted in the US documented that treatme
moderately severe acute phase ROP resulted in a signifcant beneft to eyes that were randomly allocate
early laser treatment (i.e. treatment when high risk prethreshold disease was present) compared to del
treatment (i.e. treatment was only given i threshold disease developed and the eyes that regressed wit
treatment were simply observed). The outcomes were assessed at 9 months: i.e. visual acuity, struc
fndings, and complications during treatment. The fndings o the trial were that babies treated earlier had b
unctional and structural outcomes than babies treated at threshold, but there were higher rates o ocular
systemic side eects in the group treated earlier. As a result o this trial the ollowing are recommended:
Treatment or babies with Type 1 pre-threshold ROP:
Zone 1, any ROP with plus disease
Zone 1, Stage 3 ROP +/ plus
Zone 2, Stages 2 or 3 with plus disease
Increased requency o ollow up o babies with Type 2 pre-threshold ROP:
Zone 1, Stages 1 or 2 without plus
Zone 2, Stage 3 without plus
The implications o this trial or ROP programs are that more babies will need to be treated; their eyes
be treated when the babies are at an earlier postmenstrual age, and systemic complications such as ap
bradycardia and reintubation are more likely, and more requent ollow up will be required or babies with T
2 pre-threshold ROP.
In general, ROP requiring treatment occurs around 36-38 weeks postconceptional age, a time when m
o the babies are nearing discharge. It is essential to detect and treat retinopathy o this severity within
to three days or blindness may result. Thus, any screening program undertaken must detect and pro
treatment or these babies in a timely manner.
Need for follow up of premature babiesParents need to know about the possibility o blindness rom ROP and the need or ollow-up, particu
once their baby has been discharged rom the neonatal intensive or intermediate care unit. The baby will n

such as reractive errors, amblyopia, and strabismus. Premature babies may have visual impairment
ROP as well as rom lesions o the higher visual pathways (optic nerve, visual cortex, and higher associ
areas).

Current capacity (infrastructure, human resources, and equipment)1) Situation analysis: Some countries, but not all, have a clear idea o the extent o their current screening
programs, and the degree to which the need is currently being met. An analysis should be undertaken in
each country to provide inormation on the number o units in the country, and the provider or those units;
the number and survival rate o preterm babies at risk by birth weight group, the extent to which the current
screening program, i any, is meeting this need, and the current status o ophthalmologists adequately trained
or ROP screening and treatment. This inormation is essential or planning.
2) Increasing coverage: There are many challenges to increasing coverage o ROP programs. Lack o
fnancial reimbursement or time spent by ophthalmologists on the program is a major actor. In addition,
partnerships with in-country governmental agencies must be developed. Countries in Latin America with well
established programs can assist those countries where programs have only just started.
3) Developing guidelines: Regional guidelines need to be developed by neonatologists and ophthalmologists,
under the auspices o the IAPB, PAHO, PAAO and SIBEN, which can be used (ater modifcation, i required)
not only or program implementation, but also or advocacy and to mobilise resources.
4) Awareness and advocacy: Parents, governmental agencies and health care providers must be advised
o the serious o the ROP issue in Latin America. This can be achieved through publications, public service
announcements and other means. Extensive educational material is available in several countries and it should
be more widely dispersed.
The curriculae o nurses, pediatricians, obstetricians and neonatologists must include inormation about their
role in primary prevention o blindness rom ROP. These responsibilities include:
a. Developing minimal acceptable standards or oxygen monitoring
b. Inormation given to parents by neonatologists with respect to ROPc. Identiying and recording o babies needing examination (who and how)
d. Care during examinations in intensive and intermediate care
e. Ensuring discharged and treated babies attend ollow-up
. Care during laser/cryo treatment
g. Follow-up o premature babies by pediatricians
h. Data to be collected or monitoring and evaluating programs
The curriculae o medical students, residents and general ophthalmologists must include basic inormation
about ROP including recognition o the disorder and its treatment. In particular, ophthalmologists who serve
as screeners and/or treaters must be amiliar with the disease process and screening and treatment o ROP.The responsibilities o screeners/treaters include:
a. Providing inormation or parents
b. Developing screening criteria in collaboration with neonatologists
c. Determining requency o examinations, and ensuring adequate ollow-up
d. Indications and consent or treatment
e. Method o treatment and ollow up ater treatment
. Reerral to low vision services or children with visual impairment rom ROP

5) Evaluation o current programs: Each country must determine the minimal inormation needed to mo
current programs in terms o adequacy o coverage and quality. A standard reporting orm and database
region wide will acilitate this process. Such a database or website would serve as a mechanism or sha
ideas across borders and discussing problems and their possible solutions.
6) Low vision care or premature babies. Currently there are inadequate low vision services or chil
who have the potential to beneft in Latin America. Parents are key to the success o visual rehabilitatiochildren and need to be included as a key component, and other proessionals (e.g. teachers) also nee
be involved. Many premature inants who have visual impairment have associated multiple other disab
(sensory, motor and cognitive developmental delays) which compound their developmental delay.
Current recommendations:1) Data collection or establishing current status and monitoring progress
a. For each country:
- Number o newborns < 2,000 gms birth weight- Rate o premature babies surviving with < 2,000 gms birth weight
- Number o Neonatal Intensive and Intermediate Care Units
- Number o Units with ROP screening
- Number o Units that provide treatment or ROP
- Number o premature babies screened
- Number o premature babies treated
- Birth weight and gestational age o babies treated
b. For each unit:
- Date o birth
- Sex
- Birth weight
- Gestational age
- Days on oxygen and ventilation
- Date o frst eye examination
- Date o last eye examination
- Stage o ROP
- Treatment given (laser, cryo, VR surgery), and outcome o treatment
- Subsequent ophthalmologic checkups: at 6 and 12 months
- I patient dropped out o program
2) Neonatal care
Minimal acceptable standards or monitoring o oxygen:
- Personnel trained in procedure o administration o oxygen
- Adequate equipment must be available (e.g. pulse oximeter) to monitor all babies receiving supplem
oxygen
- Adequate blenders and heater humidifers must be available
- Maintain oxygen saturation between 88% and 92% and monitor arterial oxygen between 70 and 80 mm

3) Training o ophthalmologists or screening and treatment
- Training or a general ophthalmologist who already knows how to perorm indirect ophthalmoscopy should
take place at least twice a week or 3 6 months, examining at least 100 cases with an experienced
screener. The ophthalmologist treating ROP should have observed at least 10-15 treatments and should
begin to provide treatment under expert supervision.
4) Inormation or parents o at-risk babiesThe ollowing inormation should be provided to all parents o babies at risk o ROP:
- A description o the maturing process o the retina
- Risks to the maturing retina rom prematurity, illnesses and therapies.
- Possibility o preventing blindness with timely diagnosis and treatment, and what this will entail
- Treatment, even when administered in a timely and appropriately manner, not always successul
- Importance o timely examinations, even i baby is discharged rom NICU.
5) Screening criteria suggested
The ollowing criteria are suggested:
- Birth weight: 1,750 gms and/or gestational age 32 weeks.- Birth weight >1,750 gms at the discretion o the neonatologist
However, as the risk o ROP in any NICUs is related to case mix as well as neonatal outcomes and levels o
care, NICUs may want to set their own criteria. This should be done on the basis o longitudinal studies o at
least one year in which details o the BW, GA and eye fndings o consecutive babies are recorded.
It is the responsibility o the neonatologist to identiy those babies who should be examined, to keep a diary
or determining when the frst examinations are needed, and to notiy the ophthalmologist o the need or ROP
screening examinations in a timely manner.
6) Timing and requency o screening examinations
When a reliable estimate o gestational age is not available, the frst examination should be 4-6 weeks ater birth.
For babies with a reliable estimated gestational age o at least 28 weeks, examinations should begin at 4-6
weeks ater birth. As neonatal care improves and increasingly premature babies survive, these guidelines may
need to be modifed (see below).

Subsequent examinations:
- I the retina is immature and there is no ROP, the next examination should be at 2 - 3 weeks
- I there is ROP in zone 3, the next examination should be at 2 weeks
- I there is ROP zone 1 or 2 the next examination should be at 1 week, or at 3-4 days depending on
stage o disease and the appearance o the posterior pole vessels
- Examinations should continue until the retina is ully vascularized (within 1 disc diameter o the
serrata) or the ROP has regressed
It is the responsibility o the ophthalmologist to decide when the next examination should be, to inorm
neonatologist caring or the baby i the child is an inpatient or the mother/parents i the child is an outpat
and to document the date or the next visit.
I, in the opinion o the neonatologist, the inant is too unstable to undergo the eye examination at the int
suggested by the ophthalmologist, the reason or the delay should be documented in the inants chart.
7) Procedure o the eye examination
Beore scheduling an ROP screening examination, the neonatologist should check that the baby on the Nis stable enough to be examined. A neonatologist must be available during screening in case a baby deve
cardiovascular or respiratory problems.
- Pupil Dilatation: Cyclopentolate 0.5 % combined with phenylephrine 2.5% or Tropicamide 0.5%
mydriatic eyedrops should be instilled at least 30 minutes or 1 hour prior to examination.
- Retinal examination: Indirect ophthalmoscopy with a 28 or 30 D lens is recommended, ater ully dil
the pupils. Instillation o topical anesthetic is strongly recommended i a lid speculum is being used
examination should be perormed by an ophthalmologist experienced in diagnosis o ROP. The exam
should frst examine the posterior pole, to look or signs o vascular dilation and/or tortuosity (pre-or plus disease), examine frst the nasal retina and then the temporal retina to determine the zon
vascularization and stage o retinopathy i present.
8) Location o examination
I the baby is still on the NICU, the baby should be examined in the NICU at the appropriate postnatal
regardless o whether the baby is in an incubator / being ventilated. Avoid unnecessary transport o the b
as this may increase morbidity o the examination.
Ater discharge the baby should be ollowed up as an outpatient either at the NICU or in the ophthalmolog
ofce.
9) Ensuring ollow-up o babies at risk
The NICU should collect detailed addresses and telephone numbers (including those o parents and o
relatives) or each baby so they can be easily contacted or ollow up examinations.
Every eort should be made to ensure timely ollow up. This might include telephoning, or by reques
personnel health assistant, or social workers to contact the amily

10) Treatment indications
Written, inormed consent should be obtained rom the parents / guardian using an inormation sheet that is
easy to understand.
Treatment is indicated or Type 1 pre -threshold disease (ET-ROP defnition) within 48 hours and includes:
- Zone 1 any stage
- Zone 2 Stage 2 + plus
- Zone 2 Stage 3The current recommended method o treatment is 360-degree ablation o the peripheral avascular retina,
treating anterior to any ROP, using an indirect laser and/or cryotherapy.
The patient should be prepared and monitored during the entire procedure by the neonatal nurse, neonatologist
and/or anesthesiologist. The procedure should be done in an operating room or appropriate care sector. The
choice o sedation, analgesic or general anesthetic will depend on the possibilities o each service. Post-
treatment recovery should be done in the neonatal intensive care unit. Postoperative ocular medications
might include steroid/antibiotic combinations or a week to 10 days.
Treatment of ROPTreatment or Type 1 ROP consists o ablation o the avascular retina anterior to the region o the active disease
to arrest the progression o ROP. The ridge and extravascular fbroprolieration at the junction between the
avascular and vascularized retinal should be avoided.
Both cryo and laser treatment may be painul procedures, thus, both should be perormed under either
sedation or ull anesthesia, ensuring a good analgesia. The stress caused by pain should be avoided as
systemic complications may ensue. When an anesthetist is not available, an experienced neonatologist should
give support. I the treatment is not being perormed at the neonatal unit, acilities or artifcial ventilation,
resuscitation equipment and intravenous line must be available.
Mydriasis should be achieved prior to cryotherapy or laser.
A. Cryotherapy technique1) A retina probe or modifed probe maybe used.
2) Indent the eye without pressure, excess o pressure is to be avoided.
3) The end point or adequate treatment using cryotherapy is a sudden whitening o the retina.. Cryo-
applications should be 1 spot apart rom each other as they tend to expand.
4) The cryo-probe should be withdrawn rom the eye at periodic intervals to ensure perusion o the retina
and the disc.
5) Treatment o both eyes generally takes between 45-90 minutes.
Potential complications:
Systemic: Apnea, bradycardia, oxygen desaturation are common. These complications can occur during the
frst 3 days ater treatment and require mechanical ventilation.
Eye: Subconjunctival haematoma, swollen o the eyelids, conjuctival laceration, vitreous haemorrhage.

B. Laser Photo-coagulation1. Diode Laser (used extensively)
- Laser Indirect delivery system.
- 200-500 milliwatts, and 0.2-0.5 seconds. The avascular retina should be treated with a distancebetween laser spots o no more than hal o spot
Complications: As a direct result o Laser photocoagulation cataract may develop in 1-2%.
Treatment o very posterior ROP is generally easier with laser photocoagulation.
2. Postoperative care
- Post operative medication includes ointment or drops twice daily or 3-5 days. Cycloplegia is optio
- The undus should be re-examined 5-7 days ollowing treatment. I plus disease is still present, thenperiphery must be examined to determine whether or not there are signifcant untreated areas. I t
are, then more treatment should be applied.
- Ater treatment the baby should be re-examined at 1 week. Untreated areas should be looked or, as
as signs o regression / progression o ROP and/or o plus disease. Careul observation or progres
to retinal detachment is needed in the postoperative weeks.
- The peripheral retina should be retreated with laser or cryotherapy i there are signs o progressio
general, retreatment o skip areas is needed only i there is progression.
Eyes with partial retinal detachments (stage IVa and stage IVb): Vitreoretinal surgery may be indic
or Stage IVa or b, but the decision to operate, and the nature o the surgical intervention, needs to be b
on a careul assessment o the eye, and o the child, by an experienced vitreoretinal surgeon, in collabor
with the anaesthetist, and paediatrician.
There are no internationally agreed guidelines concerning the timing o treatment, nor evidence rom cli
trials as to which are the optimum interventions.
Stage V: Complex vitreoretinal surgery is not recommended at present, as the unctional results are gene
extremely poor even in anatomically successul results.
