Billing and Documentation Tips

Half of Americans suffer from one or more chronic diseases. Keep in mind that ALL chronic diseases/conditions should be assessed and documented in the patient’s medical record at least ONCE EVERY YEAR. Definition of Chronic: Generally defined as a condition or disease lasting more than 3 MONTHS. Is of slow duration and slow progression. Chronic diseases include, but not limited to: diabetes, asthma, chronic lung

diseases, hypertension, congestive heart failure, coronary artery disease, substance abuse disorders, HIV/AIDS, depression, and other mental health conditions.

Conversely, acute diseases are defined as a condition with an abrupt or rapid onset.

Tips for Coding/Billing for Chronic Diseases: Code and bill chronic disease(s) as often as the patient receives treatment and care for that condition(s). Follow the guidelines listed below: Do not code suspected diagnoses in the outpatient setting. Code only the

symptom, complaint, condition, or problem reported. Medical records, not claim forms, should reflect the services provided for “rule out” purposes.

Providing Services to Children in Foster Care

There are approximately 14,000 children in foster care in Michigan. At no fault to their own, these children are often removed from their homes due to abuse and/or neglect and placed in foster homes or group homes. Children in foster care are vulnerable and at high risk for chronic physical, emotional/ behavioral, and developmental conditions. These conditions can exist prior to the child entering foster care and are often intensified with adjustment to placement outside of their home. In addition, these children are often subjected to multiple placements that could interrupt services re-sulting in gaps of care and services. Often there is a fragmented approach in coordination with these children, which only emphasizes the importance to ensure that children in foster care receive the best care in medical services to address all needs. A multidisciplinary approach of health care professionals, foster parents, community welfare agencies, mental health professionals, and health insurance agencies, can assist children in foster care in receiving the necessary care.

Once a child enters foster care, they become either a temporary ward of the court or a permanent ward of the court. Each foster child is assigned a Department of Human Services Worker (DHS) who serves as the responsible party. DHS collaborates with the foster parents, private agency (if involved), and the birth parents to assist in coordinating services for foster children. Each DHS county office has a Health Liaison Officer (HLO) who is the identified point person that oversees the delivery of health services in the county and works with the Medicaid Health plan.

When providing services to children in foster care, there are certain guidelines that must be followed. It is important that these guidelines be followed to protect these foster children.

The health plan must obtain permission from the DHS worker, the responsible party, in order to speak with the foster parents and/or private agency worker.

Once permission has been obtained, the health plan can work with the foster parent in coor-dinating services for the foster child through a variety of services including case management, cus-tomer service, and quality, etc. At UnitedHealthcare Community Plan, there is a designated Foster Care Liaison that works with the HLO, DHS workers, health care professionals, community service agencies, and internal staff to assist with coordinating services and to ensure that these guidelines are being followed. GET THE FACTS: ASTHMA VS. COPD

Asthma and COPD (including chronic bronchitis & emphysema) are both life-long (chronic) inflammatory diseases involving the respiratory tract and cause airway limitation. Both conditions are usually characterized by mucus and bronchoconstriction. THESE DISEASES TYPICALLY ARE UNDERDIAGNOSED, MISDIAGNOSED AND UNDERTREATED. Both diseases share symptoms and treatments yet are completely different. Reference the chart below to determine which respiratory disease to diagnose the patient with and the best treatment to provide to the patient.

The State of Michigan puts an emphasis on the importance of monitoring health services for foster children to assist in continuity of care. The State’s current foster care policies promote the oversight of these services and provide general health requirements for all children in foster care to ensure that:

Each child must receive the age specific well-child exam within 30 days of initially entering foster care.

Primary Care Physicians must complete ALL the pages of the designated well-child exam form. Well-child exam forms consist of a physical exam, immunization review, developmental surveillance/validated developmental screening, dental, and mental health screening.

These well-child exams should be completed in accordance with the Early Periodic Screening,

Diagnosis, and Treatment (EPSDT) Program Requirements. Depending on the age of the child, children may require more than one exam annually.

The well-child exam forms and policies can be found on the DHS website, www.michigan.gov/dhs. Updates to these policies can be found on the Michigan Department of Community Health (MDHC) website, www.michigan.gov/mdhc.

By health care professionals working together, children in foster care can receive the essential medical care to address their needs. By following the established guidelines, foster children can be protected by eliminating gaps in services and increasing continuity in care.
