Participation is easy — simply visit onecommunity.com to set up your Passport to Wellness (PTW) Portal. From this portal you can track and review your health-related activities. You can use the site to complete personal health challenges, sync apps and devices, track activity (steps, nutrition, sleep, etc.), browse a database of recipes and workout videos, complete a meal planner and more. Enroll and Earn Your Discounts To receive discounts on your St. Luke’s monthly medical premiums (any plan option) for the remainder of 2018 and in 2019, benefit- eligible employees and covered spouses must complete the steps above by August 31, 2018. Your new OneCommunity portal at onecommunity.com features year-round activity tracking, device syncing, workout videos, a recipe database, deals at your local grocery store and more. Even better, you will use the site to participate in individual and group challenges to earn points all year long!

Menu for Success Over six weeks, Menu for Success provides simple ways to eat more vegetables, fruits and whole grains, and less of everything else. Each week offers information about a different nutritional focus, a challenge and a healthy recipe to try.

Step Up Your Game Ever wonder what your favorite athletes go through on game day? Step Up Your Game challenges participants to achieve step counts equal to the distance professional athletes travel over an average quarter, half or full game. Each week is themed to a different sport.

Every Move Matters Great for those looking for an active challenge, Every Move Matters provides education about new exercises and quick daily workout routines. Learn to move more and move correctly, and set yourself up for better health in the future!

Dashing Through the Holidays This four-week challenge helps you fend off holiday weight gain with healthy holiday eating strategies. Each week focuses on a different strategy, and includes tips and tricks to enjoy a healthy holiday with family and friends.

STEP 1: REGISTER & COMPLETE THE SURVEY

2. Follow the registration instructions.When prompted, enter yourprogram’s Invitation Code:

PTW2018Returning participants – you willneed to choose ‘Join A NewProgram’ in order to be taken toyour New Program.

3. You’re all set! Now you can participatein the health survey and upcomingscreenings.

STEP 2: GET SCREENED

• Submit your PhysicianProvided Lab Results (formcan be downloaded fromOneCommunity’s “My Wellness”page) OR

• In April, St. Luke’s Des Peres

Hospital employees willbe screened as part of theonboarding process. ThePassport to Wellness team willbe on site during the

orientation sessions.

• Biometric Screeningsinclude blood pressure,total cholesterol, glucose,HDL, waist circumference andweight. If fasting (preferred),triglycerides and LDL are alsoincluded. Each participantreceives a results consultation.
