LIFESTYLE CHANGE PROGRAM AT MIDHA MEDICAL CLINIC

CDC estimates that 1 in 3 adults (more than 79 MILLION Americans age 20 years and older) have Prediabetes and most (about 90 percent) don't know they have it. Without interventions, 15-30 percent of these men and women will develop Type 2 Diabetes within 5 years. Centers for Disease Control and Prevention (CDC) also estimates that by the year 2050, 1 in 3 people could have DIABETES.The National Diabetes Prevention Program brings communities an effective lifestyle change program proven to prevent or delay the onset of type 2 diabetes by 58 percent.

At Midha Medical Clinic, our Diabetes Prevention Program has received CDC's Preliminary Recognition. This designation is reserved for those programs that are effectively delivering a quality, evidence based program. “As the Director of Division of Diabetes Translation, it is immensely gratifying to see the science of diabetes prevention being implemented to improve the public's health. In order to qualify for the program, the participant must

be

At least 18 years of age

Be Overweight (BMI ≥ 25/ BMI ≥ 23 for Asians)

Have established risk factors for developing type 2

diabetes

Be diagnosed with prediabetes within the past

year or previously diagnosed with gestational

diabetes mellitus (GDM) while pregnant.

Prediabetes Fact: Older Adults (> 60 years age) with

prediabetes can reduce their risk of getting diabetes upto

71 percent by participating in this program and

achieving 5 to 7 percent weight loss.

Participants at high risk for type 2 diabetes meet in groups

with our trained and Certied Lifestyle Coach for 16

weeks and 6 monthly follow up sessions to learn ways to

incorporate healthy eating and moderate physical activity

as well as problem solving, stress reduction and coping

skills into their daily lives.

Participants work with the Lifestyle Coach and the group

members to identify and overcome barriers to making

sustainable lifestyle changes.

The program empowers participants to take charge of

their health and well being.

EVIDENCE OF SUCCESS OF THE LIFESTYLE CHANGE PROGRAM

An NIH (National Institute of Health) led CDC supported 1research study showed that weight loss of 5-7 percent of

body weight achieved by lowering fat intake and increasing physical activity to at least 150 minutes per week reduced the risk of developing type 2 diabetes by 58 percent in people at high risk for the disease.1Knowler WC, Barrett-Connor E, Fowler SE, et al. EVIDENCE OF SUCCESS FROM OUR PARTICIPANTS

Our Previous Participants Lost 23, 20, 12, 10 Pounds In 16 To 30 Weeks, Imrpoved Their Overall Health And Some Of Them Dropped Their A1c Number Back To Normal Again ( No Longer Prediabetic After Participating In This Program).
