    MONROE COMMUNITY COLLEGE CHILD CARE CENTER

    PARENT AGREEMENT

    STUDENTS

    . REGISTRATION: Students are registered according to academic semesters. Registration can be completed onby submitting all signed registration forms and the $30 registration fee. Spaces are reserved in the Center only wpayment of this non-refundable fee. An interview by the Child Care Center director or designee is required. Attime of enrollment, a one-week non-refundable tuition deposit will be charged in advance and applied to the finaweek of tuition.

    . TUITION: Child Care fees are based upon the age group in which the child is enrolled and his/her childcareschedule. Parents are required to pay for the full plan for which they have registered, even if the child is absentfrom the Center. Parents are responsible for payment through the end of the semester including exam week. In event that the Center closes for weather or an emergency, there will be no charge to parents. The MCC Child CaCenter reserves the right to increase tuition with 30 days-advanced notice in writing.

    . PAYMENT DAY: Payment is due weekly on the first weekday that the child attends.

    . LATE PAYMENT PENALTY: If payment is not made by Thursday of each week a late fee will be added to yoaccount. The MCC Child Care Center reserves the right to discontinue services to any family that is in arrears o

    that has repeated occasions of paying late. When payment becomes two full calendar weeks behind, the child mnot return to the Center until payment has been made. The slot will be held for only one week. If the student faito make payments on this balance in a timely manner, collection costs equal to one half of the outstanding balanamount will be added to the account. A hold will be placed on the MCC records, if a student accrues an unpaibalance with the center.

    . RETURNED CHECK CHARGE: There will be a $30 charge for any check returned for insufficient funds.

    . OPERATING SCHEDULE: The MCC Child Care Center is open from 7:00 a.m. to 5:30 p.m. Monday througFriday year round. Hours of operation and / or major legal holiday closings are subject to change with a 30-dayadvanced notice given to the parents.

    . LATE PICK-UP: The Center closes at 5:30 P.M. Pick-up time and closing time are strictly observed. A $5 latecharge will be added for every 15 minutes (or any part thereof) that the parent picks up the child after the Centercloses.

    . HOLIDAYS: The Center will be closed for the major legal holidays: New Years Day, Memorial Day, the entireweek of July 4th, Labor Day, Thanksgiving Day, and the day after, the entire week of Christmas, Martin LutherKing Day, and two days for staff training (TBA). Tuition will be charged for ALL holidays and staff training daThere will be no charge for the additional days in the week of July 4 th or Christmas week. The center will close p.m. the day before Christmas.

    . WITHDRAWAL FROM THE CENTER: Parents may withdraw their children from the Center or reduce theirschedule by giving two weeks-advanced notice in writing. Parents must pay for these two weeks in full whethernot their child attends. If a child is unable to adjust in a reasonable period of time, we reserve the right to removthe child from the program.

    0. SICKNESS: By state regulations, children many not come to the Center if they are sick. Children who becomewhile at the Center will be placed in isolation briefly while the parent is contacted to have the child picked up assoon as possible. Parents must call the office by 9:30 a.m. if your child is going to be absent.

    1. PHYSICAL: Prior to attending the Center all children are required by New York State regulations to have aphysical report signed by a physician on file in the Center. Parents are required to have medical forms for theirchildren updated yearly and as new immunizations are given. Children under age two need to have medical repoupdated whenever they visit the physician.

    2. MEDICATIONS: Medication may only be given when: the medication is in the original labeled container, theCenter is provided with a set of written instructions signed by a physician; and the parent signs medicationinstructions. This policy also applies to all over the counter medications. All medication must be dropped off inthe office, and can only be administered by NYS trained staff.

    3. NAPS: Children under age five are required to have a rest period daily if they attend the Center full days. TheCenter will provide a clean mat, cot, or crib for every child. Parents must provide a labeled crib sheet and blankwhich parent must take home weekly to be washed.

    4. CLOTHING: Parents are required to provide at least one complete change of clothes to be left at the Center incase of accidents. These clothes must be labeled and need to be changed as the child grows and as the seasonschange. All children must have warm outdoor clothing and boots in winter, as they will be going outside mostdays. Sunscreen must be provided in warm weather.

    5. MEALS: Meals are planned by a dietician to be nutritional and well balanced. Children will be served the

    approved center meals (breakfast, lunch, and an afternoon snack). Food substitutions for personal, religious, ormedical reasons may be accommodated. If resultant meal patterns or serving sizes will not meet the childsnutritional needs, a medical statement must be obtained documenting the appropriateness of the variation. Efforwill be made to provide substitutions through our food service provider. If suitable substitutes are not availableparents must provide appropriate substitutes. Our center participates in the Child and Adult Care Food ProgramThis program has a mandated form that must be filled out by every family for EVERY CONTRACT YEAR,regardless of whether you are eligible or not for free or reduce meals. The financial information you put on thesforms is confidential. If your child is going to be absent or will not stay for lunch, please call the center before9:30 a.m., otherwise you will be charged $2.75.

    6. ARRIVAL AND PICK-UP: Upon arrival parents must take their child to the sink and wash their hands. Parenare required to walk their children into their classroom, sign them in, and turn them over to a caregiver. At the eof the day, the children must be signed out and will be released ONLY to someone carrying a pick-up card andproper identification. The person picking up the child must be at least 16 years old. Parents should have their csay goodbye to their teacher.

    7. INFANT CARE: Parents must provide a crib-sized blanket, diapers, and several changes of clothes. Parents wibe asked to complete an Infant Schedule form at parent orientation. All infant materials must be labeled includinbottles.

    ARRIVAL AND PICK UP REQUIREMENTS: Parents are required to walk their children into their classroom. Your child must be signednd out of the center daily. Your child will only be released to those persons whose names are on the registration papers.     Center has no legal authority to prevent any parent access to their child unless we have received a copy of a court order stating otherwise. Yre required to let MCC Child Care Center know at the time of dropping off your child if the person who will be picking up you child isomeone other than the regular people in the permission form.     MEDICATION: The nurse or MAT Trained Staff may give medication to my child only if: you must submit a childs medical examinationorm prior to or at the time of admission. MCC Child Care Center Staff cannot admitter any medication to your child without a written con

    or your childs physician stating the childs name, dosage, medication, and date. You must also provide a medication form provide by us eime your child is on medication. If your child is on a long-term medication, one medication form may be given that provides a beginning nding date for the medication this should be done every six months.

    MEDICAL EMERGENCIES: You need to provide us will al special information regarding DIETS, HABITS, and ALL PERTINENTHEALTH CONCERNS, on the registration date. If your responsibility to provide us with any new information such home address, job chawork number, home phone number, or change in medical insurance. This is essential in case of an emergency. We need to be in constantontact with you at all times. Voice mail should not be the only number that is available to the center. If you are not going to be at school

    work a second number will be provided to the center where you can be reached. If you can not be reached, the emergency person that Iesignate will be called. In case of an accident or injury, you understand that emergency medical care may be given in the event that you, ohe person designated, cannot be contacted.

    ICKNESS: Just for your information, children are taken outside year round unless the weather is raining, too windy, or extremely cold.lease be sure that your child has proper outdoor clothing (boots, hats, mittens, snow pants). Fresh air and exercise is germ free and healthor your child. It is also a daily requirement of New York State Licensing regulations. It is myth that cold air makes your child moreusceptible to illness. Children do not get sick due to being outside in the cold. Children become sick due to germs. If you child is wellnough to attend the center, they should be well enough to play outdoors. Sometimes parents request that their child be keep indoors withnother classroom, however, we have to adhere to legal rations; because our center is usually full, we are sometimes unable to do this. It islso upsetting and difficult for the child whose parent does not want them outdoors, to understand that they are not being punished when pln an unfamiliar room and not outdoors with their friends.
