measuring 23 x 10 cms should be sent in a large sized cover measuring 38 cm X 28

cm super scribed ‘APPLICATION FOR PARA MEDICAL COURSES’ 2014–2015

SESSION’ addressed to the Secretary, Selection Committee, Kilpauk, Chennai -10

before the last date along with a Demand Draft for 350 /- which is non-refundable

towards the cost of application form drawn in favour of The Secretary, Selection

Committee and payable at Chennai. The name of the candidate and the name of

the course (Paramedical Courses) should be written on the reverse of the

Demand Draft. The template with requisite details available for download should be

printed and pasted on the cover. SC/ SCA and ST candidates who are natives of Tamil

Nadu are exempted from paying the cost of the Application Form.

" AR Number will be assigned by the Selection Committee.

" Candidates can track their applications on the above websites by entering either their

Application number or the Plus Two / Equivalent Examination Register Number of first

appearance with the year.

" Eligible Candidates can download their call letters from the websites www.tnhealth.org/

www.tn.gov.in by entering their AR No allotted by the Selection Committee and their

Date of Birth. They are instructed to attend the counselling as per the date and time

mentioned in the counselling schedule which will also be available on the websites.

Candidates are requested to paste their photograph on the call letter which is to be

attested by a Gazetted Officer. (The photograph must be a copy of the same one

pasted on the application).

GENERAL INSTRUCTIONS:-

1. (a) Candidates seeking admission to Paramedical courses should submit the commonapplication form with necessary enclosures which should reach:-

The Secretary, Selection Committee,No.162, E.V.R Periyar High Road, Kilpauk, Chennai – 10.on or before 19-07-2014 by 5.00 p.m.

(b) The candidates shall ensure that the completed application form with all particulars andenclosures reaches the Selection Committee within the date and time specified.Applications received by Speed post / courier or any other means after the last date &time will not be accepted irrespective of the date of booking.

(c) The Selection Committee shall not be responsible for any delay in the receipt or for theloss in transit of application form etc.,

(d) Request for extension of time for submission of documents called for, after the specifieddate and time will not be entertained.

(e) Any supporting documents received after the due date of application without requireddocuments / particulars will not be entertained.

(f) Candidates seeking admission under Special Category for the paramedical courses(Orthopaedically Physically disabled & Children of Ex-Servicemen ) shall submitthe separate Special Category Form along with the main application in the prescribedformat available along with an additional fee of 100 /- by way of a Demand Draft for eachspecial category enclosing the relevant certificates. If the certificates are not enclosed,the candidates will not be considered under that category without any intimation to thecandidate.

(g) All the Demand draft should be drawn in favour of “The Secretary, Selection Committee”payable at Chennai.

(h) Candidates selected under General Category will not be allowed to claim the very samestatus as the candidates selected under Special Category and vice versa.

(i) No oral enquiry would be entertained.

(j) Photocopy/ Fax copy of application will not be accepted.

(k) Both upper limbs, vision and hearing should be normal for all candidates includingcandidates applying under physically disabled category.

II. ELIGIBILITY:-

2. Candidate should be a citizen of India.

3. NATIVITY:-

i) Candidate should be a Native of Tamil Nadu

ii) Candidates who have studied VIII to XII standard in schools of Tamil Nadu need notenclose the “ Nativity Certificate”

iii) Candidates who are native of Tamil Nadu but studied from VIII to XII Std either partly orcompletely in schools situated outside Tamil Nadu should produce the “NativityCertificate”.

iv) The Certificate of Nativity in Tamil Nadu should be issued by the competent authorityonly in the specified format available in the application form and supported by acopy of Passport/ Ration Card / Birth Certificate.If the copy of the supporting documentis not produced, then the application form will be rejected. Nativity Certificatesubmitted later will be summarily rejected.

v) Applications without a proper Nativity Certificate will not be considered.

vi) Permanent Residence Certificate will not be accepted.

vii) Tamil Nadu native candidates alone will be considered for communal reservations

Note:

a) Nativity Certificate should be produced as per Instructions for Nativity Certificate inAnnexure XIV

b) Producing false nativity certificate if detected at any time later during the course of studywill result in expulsion of the candidate and also initiation of criminal proceedings againstthe candidate as well as the Parents as per law.

4. AGE LIMIT : - FOR B.Sc., (NURSING) COURSE:-

Candidates should have completed 17 years of age and maximum age is 30 years as on 31st

December, 2014. For SC / SCA / ST candidates maximum age is raised to 35 years. Evidenceshould be produced.

FOR ALL OTHER COURSES:-

Candidates should have completed the age of Seventeen years on or before 31st December2014. Evidence should be produced.

5. EDUCATIONAL QUALIFICATIONS: -

Candidates should have passed in all the subjects of the qualifying Examination of the HigherSecondary Course conducted by the Tamil Nadu State Board or any other equivalent Board ina single sitting in the following group of subjects with minimum eligible marks.

(a) Physics, Chemistry, Botany and Zoology.(or)

(b) Physics, Chemistry, Biology or Mathematics with any other subject.

A failure in examination/ reappearance/ first appearance in compartment in oneor more subjects will be treated as an attempt.

6. MINIMUM ELIGIBILITY MARKS: -

i. B.Pharm. /BASLP Courses:

For OC / BC / BCM / MBC / DC Candidates : -

Individual pass in Physics, Chemistry, Biology or Botany and Zoology or Mathematicsboth in Theory and Practical with a minimum of 35% of marks and with not less than 40%aggregate marks in the above subjects.

For SC / SCA / ST Candidates: -

Individual pass in Physics, Chemistry, Biology or Botany and Zoology (or) Mathematicsboth in Theory and Practical with a minimum of 35% of marks and with not less than 35%aggregate marks in the above subjects.

English:- A Pass in English with a minimum of 35% marks is mandatory.

ii). B.Sc. Nursing Course:

For OC / BC / BCM / MBC / DC Candidates :-

Individual pass in Physics, Chemistry, Biology or Botany and Zoology both in Theory andPractical with a minimum of 35% of marks and with not less than 45% aggregate marksin the above subjects.

For SC / SCA / ST Candidates:-

Individual pass in Physics, Chemistry, Biology or Botany and Zoology both in Theory andPractical with a minimum of 35% of marks and with not less than 40 % aggregate marksin the above subjects.

English: A Pass in English with a minimum of 35% marks is mandatory.

(iii) B.P.T., /B.Sc.Radiology& Imaging Technology / B.Sc. Radio – Therapy Technology/B.Sc., Cardio Pulmonary Perfusion Technology and B.O.T :

All Categories:

Individual pass in Physics, Chemistry, Biology or Botany and Zoology (both in Theoryand Practical) with a minimum of 35% of marks in each of the above subjects is mandatory.

English: A Pass in English with a minimum of 35% marks is mandatory.

NOTE:-

(i) Only the marks obtained in the science subject in the HSC qualifying examination as awhole in the first appearance will alone be taken into consideration for the allotment ofseats to the candidates as per the G.O. Ms No. 184/HE (J2)/Dept, dated:09/06/2005 andG.O.(St)No.143/HE(J2)/Dept., dated : 06/05/2008.

(ii) Candidates who have passed any other equivalent examinations in the first attemptwith same group of subjects mentioned above should produce the eligibility certificatefrom the Tamil Nadu Dr. MGR Medical University before admission to the course.

(iii) In the case of candidates who have furnished the mark sheets of the qualifying examination,only in GRADES, the minimum marks of the grades alone will be taken into considerationwhile determining the marks in the select Science subjects of Physics, Chemistry andBiology or Botany and Zoology.

(iv) Candidates who have submitted mark sheets with GRADES should obtain the statementof marks indicating the actual marks secured from the examination authority and attacha certified copy as an enclosure to the application.

(v) Normalization method will be adopted for calculating the minimum eligibility mark for thecandidates who have passed any other equivalent examination.

7. i. Candidates who have passed H.Sc., in vocational stream are not eligible to apply to ParaMedical Courses.

ii. Married candidates are also eligible for admission for B.Sc. (Nursing) Course. All theselected candidates should stay compulsorily in the Hostel during the entire period ofstudy.

iii. The ratio between men and women shall be 1 : 9 respectively in the selection ofcandidates.

8. Normalization of Marks:-

i) As per the Tamil Nadu Act No 3 of 2007, dt 05.03.2007 and G.O( Ms) No 77 HE(J2) Dept,dt: 05.04.2007, the marks obtained by the students in the relevant subjects in the qualifyingexamination conducted by various Boards or Authority shall be equated with the marksobtained by the students in the same subjects in the qualifying examination conductedby the State Board (Government of Tamil Nadu) by adopting the method of normalization.

Explanation:-

Under the method of normalization, the highest mark obtained by the students of variousBoards in each subject will be equated to the highest mark obtained by the students of StateBoard in that subject and the relative marks obtained by other students in that subject will bedetermined accordingly.

Illustration:-

If the highest marks secured by the student of State Board in Physics is 100 and the highestmark secured by a student of any other Board in the same subject is 90, both the highestmarks will be considered to be equal to 100. If a student of the other Board secures 60 marksin Physics when the first mark in Physics in the same board is 90, the 60 marks will beconsidered to be equal to 66.66 marks as arrived at below:-

100 x 60 —————— = 66.66%

90(ii) After normalization of marks in the relevant subjects in the qualifying examination conducted

by different Boards, the qualified students of different Boards will be merged into acommon merit list.

In case of candidates who have qualified from National boards such as CBSE and ISCEetc. the highest marks obtained by the candidates at the National Level alone will betaken into consideration for normalization.

III. PROCEDURE FOR FILLING AND SUBMISSION OF APPLICATION : -

9 a) Before filling up the application form and OMR sheet/Scrutiny form, read all the instructionscarefully.

b) Each application form bears a unique number which is printed on the Application form andthe OMR. Candidates are requested to check the number and ensure that thesame number is printed on the Application form, OMR, Outer and Inner Covers.In case any discrepancy is noted, it should be immediately brought to the notice of theSelection Committee before the last date for the receipt of applications. However theserial number of the OMR will be considered as the final number for correspondencepurposes.

10 (i) Use one common application form for all Paramedical Courses. No candidate shouldsubmit more than one application.

(ii) If a candidate applies for Paramedical Courses in forms other than one prescribed forthe academic year i.e., 2014-2015 session, the application will be summarily rejected..

11. Fill up the application form and OMR sheets legibly, neatly and correctly. The information willbe processed by using a computer. Any mistake committed by the candidate will prejudicethe selection.

12. Do not fold OMR sheets under any circumstance. Making a hole, using a pin or a stapler onthe OMR sheets should be avoided. Cutting, overwriting and erasing in the Application formand in the OMR Sheet or Scrutiny form (for Downloaded applications)should be strictly avoided.

13. Application with incomplete or insufficient particulars or without enclosures or those receivedafter the last date mentioned will be summarily rejected without any intimation to the candidate.

14. Filled up application without the signature of the candidate will be rejected.

15. The Government Orders issued and to be issued from time to time pertaining to any of thematters contained in the prospectus should be read as part and parcel of this prospectus andsuch terms and conditions in the Government Order are deemed to have been incorporated inthis prospectus.

16. (i) Candidates should affix recent passport size photographs taken with name and date(as shown in the Instructions-Annexure XII) in the places provided in the applicationform and the OMR.The photograph pasted in the common application form alone shouldbe duly attested by Group A or Group B officer of the Government of Tamil Nadu or CentralGovernment or the Head of the Institution last studied

(ii) The photograph pasted in the OMR need not be attested.

(iii) The Registration number allotted by the Directorate of Government Examination/concernedBoards should be indicated along with the year of +2 examination / equivalent in theapplication.

17. All the entries in the application form and OMR sheets should be written legibly by the candidatein his/her own handwriting. The candidate should ensure that all information including themarks in the qualifying examination furnished by him/her in the application and the enclosuresare correct. The candidate is informed that if after proper scrutiny of his/her application, themarks furnished by him/her in the application are not correct or any other information is foundto be incorrect, then;

a. He/She will forfeit the admission no matter at what stage of the course, he / she will be atthat time.

b. He/She will be debarred from pursuing any other course for a period of three years;

c. Legal action will be in initiated against the candidate and the parents/guardian for furnishingincorrect information;

d. Criminal action will be initiated against him/her for furnishing false Mark Sheets / falseCommunity Certificate/ false nativity certificate.

18. No document enclosed with the application will be returned. Candidates are instructed toenclose only the attested photocopies of the following certificates along with the application.Attestation should have been made by Group A or Group B Officer of the State Government orCentral Government or by the Head of the Institution where the candidate last studied.

(a) Mark sheet of H.S.C. (Academic) examination (both sides of mark sheet should bephotocopied) or any other equivalent examination of the first appearance.

(b) Transfer certificate obtained after the completion of H.S.C. (Academic) or equivalentcourses.

(c) Nativity certificate (if applicable )

(d) Permanent Community Certificate Card (if applicable)

(e) X standard Mark Sheet .

(f) Certificate for proof of study from VIII standard to XII standard.

(g) H.S.C Hall Ticket

(h) First graduate certificate (if applicable). Certificate submitted along with application alonewill be considered. If submitted later to the college authorities it will be summarilyrejected (Annexure XV- a & b)

(i) Special category form with Demand Draft(s) and relevant certificates (if applicable).

Note:

i) The candidates belonging to Scheduled Tribe should produce Community Certificateobtained from a Competent Authority not below the rank of a Revenue Divisional Officer. Ifhe/she claims ST status in common application without producing any evidence or claimingthat ST status is under process of scrutiny will be treated as open category candidate

ii) Producing a false community, Nativity or any other certificate if detected later at anypoint of time during the course of study will result in expulsion of the candidate andalso criminal proceedings will be initiated against the candidate as well as the parentsas per provision law.

(iii). As number of bogus community certificates are being encountered off late and in order tocurb such practices, the authentication of the above certificate has to be verified andconfirmed by the concerned authority, before the individual completes 1st year Para Medicalcourse. The concerned candidate and parents should take necessary steps to thateffect. Unless proof of authenticity of community certificate is received, the candidate willnot be allowed to continue the 2nd year.

(iv) Community Certificates obtained from other states will not be considered and suchcandidates will be treated as open category.

IV METHOD OF SELECTION AND ADMISSION:-

19 (i) Ranking will be made on merit on the basis of marks obtained by the eligible candidatesin the prescribed science subjects in the qualifying examinations reduced to the base of200 marks. The marks obtained through different Boards will be taken into account afternormalization.

Percentage in Botany and Zoology - Taken Together (Z)(after normalization)

Percentage in Mathematics (after normalization) (W)

Weighted total marks for a maximum of 200. Either (X+Y) OR

(Z+Y) OR (W+Y)

The percentage and the weighted total should be accurate to two decimals.

ii) Ranking of the candidates will be done based on the total marks obtained in theScience Subjects after normalization of marks computed to a maximum of 200.

iii) Only the marks obtained in the Science subjects in the H.S.C qualifyingexamination as a whole in the first appearance will alone be taken intoconsideration for the allotment of seats to the candidates and the improvementmarks in +2 examination of any year will not be considered. Also the marksobtained through part by part appearance in +2 examination will not be consideredincluding for natives of Tamil Nadu who have passed any other equivalent examinationfrom other Boards/States.

iv) a) If claiming of improvement marks for selection to Para Medical Courses is detectedat any point of time, it will lead to cancellation of selection and removal of his/ hername from rank list.

b) Any application submitted without the latest attested pohoto copy of the firstappearance mark sheet of the qualifying examination will summarily be rejected.

Note:-

In respect of the candidates who have taken the subjects of Physics, Chemistry,Biology and Mathematics, the marks obtained in Physics, Chemistry and Biologywill be taken into account for arriving the General merit list and for those candidateswho have taken Physics,Chemistry, Mathematics along with other subjects (otherthan Biology, Botany and Zoology) the marks obtained in Physics, Chemistry andMathematics will be taken into account for arriving the merit list for B. Pharmacyand BASLP courses only.

20. Determination of Rank of Candidates obtaining equal Marks in all paramedicalcourses (Inter-se Merit)

In cases where two or more candidates obtain equal aggregate for 200, the rank ofsuch candidates shall be determined on the basis of the following order namely:-

a) Percentage in Biology or Botany and Zoology taken together in +2 Examination.

b) Percentage in Chemistry in +2 examination.

c) Percentage in Fourth Optional subject in +2 Examination.

d) Date of Birth – Weightage given to seniority in age.

e) Computerised Random number assigned. Higher value of random number will betaken into consideration.

V. MERIT LIST

21. The Secretary, Selection Committee, will publish the tentative merit list in the website:www.tnhealth.org / www.tn.gov.in

22. The candidates will be called for counselling as per their merit and they may exercisetheir option as per their eligibility norms mentioned in the Prospectus

VI. COUNSELLING PROCEDURE- SINGLE WINDOW SYSTEM:

23. The selection will be made on merit basis and by Counselling applying the rule of reservationby the Government of Tamil Nadu as follows:-

Open Competition – 31%

Backward Class – 30%

Most Back ward Class – 20%

Scheduled Caste – 18%

Scheduled Tribe – 1%

Within 30 % reservation for BC, 3.5 % reservation will be provided for BC Muslims and16% of seats out of 18 % quota earmarked to Scheduled Caste shall be allocated tothe Arunthathiyar Community.

BC, BCM, MBC / DNC, SC , SCA & ST candidates are eligible for Selection underOpen Competition as per merit in addition to the reservation made for them.

a) Any remaining unfilled seats for want of adequate number of eligible candidates belongingto SCA community shall be filled by candidates belonging to SC community other thanArunthathiyar as per merit and vice versa. (GO (Ms) 55 P & AR Departmentdated:08.04.2010)

b) Any remaining reserved seats in ST Quota for want of adequate number of eligible candidatesbelonging to ST community shall be allotted to candidates belonging to SC Communityas per merit. (GO (Ms) No.77 Health & Family Welfare Department (MCA1) dated24.02.2011)

c) Any remaining unfilled seats in BCM Community for want of adequate number of eligiblecandidates after exhausting the merit list shall be filled up on merit basis by candidatesbelonging to BC community without exceeding the quota for BC community as awhole(G.O.Ms.No.30/H&FW Dept(MCA1)/dated 18/01/2012)

d) Even after filling up of the required seats reserved for Arunthathiyars on preferential basis,if more number of qualified Arunthathiyars are available, such excess number of candidatesof Arunthathiyars shall be entitled to compete with the Scheduled Castes other thanArunthathiyars in the inter-se merit among them (As per G.O.Ms.No.65, dated:27.07.2009of P &AR(K) Department).

24 Counselling Schedule will be available on the website. Candidates who havenot received the call letter in time can also attend the counselling session as perschedule and as per their merit with call letters downloaded from the website.

25 (a) Counselling for the Special Categories will be done before the General category

(b) There will be no reallotment / change of college allotted for the Special Category

26. (i) Option once exercised during counselling will be final and cannot bechanged.

(b) Due to unforeseen reasons, if a candidate is not able to attend the counselling on thespecified date and time, he/she can attend on a subsequent time/date during thatcounselling phase and allowed to take the course and college available at the time ofcounselling when he/she actually attends.

(or)

He / She can authorize his / her parent on his / her behalf and the parent’s decision willbe binding on the candidate. The authorization letter must be in the prescribed format inthe Annexure XIII.

(c) If the candidate is absent during the 1st phase of counselling, for which he / she iscalled cannot attend the subsequent phase of Counselling.

27. One of the Parents/ Guardian will be allowed to attend the counselling. They shouldmake their own arrangements for attending the counselling session. Individualintimation will be sent for regular counselling through call letter.

28. a. All communications will be sent only to the mailing address given in the application andself addressed envelopes (Size 23 X 10 cms). Candidates are requested to intimate anychange of address immediately. The intimation / call letters will be sent either by RegisteredPost, Speed Post, Courier Service or by e.Post whichever is feasible. SelectionCommittee is not responsible for any postal delay or non receipt of call letters.

b. Any change or modification and relevant information pertaining to this admission processwill be made available immediately on the websites. (www.tnhealth.org / www.tn.gov.in.)The candidates are instructed frequently to visit the website from the date of applicationtill the end of counselling without fail. Please do not wait for counselling information to bereceived by post, as all information including the date of counselling will be put up in thewebsites. Call letters can also be downloaded from the websites. Candidates willbe permitted for counselling accordingly.

c. All candidates attending the counselling for Para Medical Degree Courses willhave to remit a non-refundable amount of Rs.200/- by means of Demand Draftdrawn in favour of “The Secretary, Selection Committee” as processing fee.

29. Those who get Retotalled / Revalued marks can also attend the counselling on the dayand session according to their new marks. If the candidate produces the Retotalled /Revaluation marks after the counselling is over and within the cut off date then his/hername, will be considered in the existing vacancies available and he/she cannot claim anypreferential rights for allotment according to merit under Retotalling /Revalued marks.Candidates who submit the Revaluation / Retotalling marks after the administrative cut-offdates fixed by Government will not be considered for selection.

30. The vacancies arising after the first phase of counselling and also to the new colleges, ifany added will be filled up from the waiting list through counselling, if necessary.

31. The movement of candidate from one category to another category (from self financing toGovernment colleges ) and from one college to another college (only for Governmentcolleges) will be based on the merit following the rule of reservation.

The date of such counselling will be published in the website and also will be informed tothe Deans of respective Government Colleges. No individual intimation will be sent to thecandidates

32. If a candidate chooses a Government Quota seat in Self-Financing College categorywhen seats are available in Government colleges, he/she has to give a declaration to theeffect that he/she will not be permitted to claim the Govt. seats later during re-allotment /subsequent phase of counselling

33. Candidates can exercise their option for the course and the college of their own choiceat the time of counselling, and allotment will be subjected to availability of seats.Individual request for transfer of college or change of course will not beentertained after counselling is over

34. Mutual transfer will not be permitted.

If adequate number of eligible candidates are not available under Special Category, theearmarked seats will be reverted back to general category.

a) 3% of the total number of seats available in Government Colleges are reserved for theOrthopaedically Physically Disabled candidates. In the first instance, in the reservationof seats, candidates with disability of lower limbs between 50% to 70% shall be consideredand in case candidates are not available in that category, then candidates with disabilityof lower limbs between 40% to 50% may be considered. The other conditions for admissioninto Paramedical course will be applicable as in the case of the general category.

The other conditions for admission to Para Medical Courses will be applicable as in the caseof General Category. The candidates are instructed to produce a certificate from the DistrictMedical Board of the area concerned constituted for this purpose assessing the natureand the extent of physical disability in the format prescribed in the Annexure III A & B.

The Medical Certificate issued by the District Medical Board of the area concerned forthis purpose is subject to the confirmation of the Expert Committee constituted by theDirector of Medical Education at the time of counselling.

The Candidates seeking admission under this category should produce a full sizephotograph exhibiting the deformity.

The Certificate must have been obtained within three months prior to submitting theapplication for seeking admission under this category. If the Certificate has been obtainedearlier, the application will be rejected.

Note :

(1) The selected candidates for Para Medical Courses will be instructed to undergo secondMedical Examination by a Medical Board constituted for the purpose by the Director ofMedical Education to ascertain and confirm their nature and extent of physical disability.

(2) Candidates with any other disability other than the locomotory disability of the Lower limbwill not be considered for admission into Para Medical Courses under the Special Category.Such applications will be summarily rejected without any intimation to the candidate.

(3) The decision of the Director of Medical Education in this regard will be final

(4) Candidates who previously applied for Paramedical Courses under OrthopaedicallyPhysically Disabled Category and were rejected by Special Medical Board constituted bythe Director of Medical Education for this purpose are not eligible to apply under thiscategory. Students of SC / ST community, whose Parents/ Guardian’s annual income is less than 2,50,000 (Rupees Two Lakhs and Fifty Thousand Only ) are exempted from payment of all

compulsory, non- refundable fee ( Enrollment/ Registration, Tuition, Games, Union, Library,Magazine, Medical Examination and such other fee compulsorily payable by the scholar to theInstitution or University / Board) and also exempted from all compulsory Non refundable feefixed by the Fee Committee. The fee, so exempted shall be claimed by the respective Institutionsunder the Scholarship Programme of the Government of India. ( As per G.O.(Ms) No 6 AD andTW Dept. dated 09.01.2012 and G.O.(Ms) No.51, Adi Dravidar and Tribal Welfare Department,dated 7.8.2013 ).

40. The selection and admission of candidates of other Universities / Boards will be provisionalsubject to approval of the qualifying examination by the Tamil Nadu Dr.M.G.R. Medical Universityunder which the candidate gets admitted. In case, the recognition is not accorded for theadmission of the candidate on any ground, the provisional admission will not entitle the candidatefor the continuance of the course and the admission will be cancelled.

41. The Selected Candidates should produce all the original documents at the time of admission tothe Deans/Principals of respective Colleges concerned. Selection will be cancelled if thecandidates do not produce the original documents at the time of admission.

42. The candidates will be required to undergo Medical Examination at the College to which theyare allotted before the Medical Board on the date notified by the authorities of the college.Only those candidates found medically fit will be admitted and be permitted to pay the fee.

43. Candidates selected for admission and his / her parent / guardian will have to sign disciplinedeclaration form in Annexure – IX, X and XI. Failure to sign the declaration will result in thecancellation of selection.

44 (a) As per the Tamil Nadu Dr.MGR Medical University regulations all the Paramedical Coursesexcept B.Sc. Nursing Course, no admission shall be made after 30th September 2014 forthe academic year.

(b) As per Indian Nursing Council, the last date for admission to B.Sc.Nursing Course is 31st

October 2014

45. Candidates can type the necessary certificates in the prescribed format separately andthey are requested to attach the same along with the application.

46. All selection and admission for Para Medical Courses are only provisional. The Selectionand admission is liable to be cancelled at any time as and when the eligibility conditionsare found to be defective or incorrect or there has been any suppression or misrepresentationof facts. The Selection Committee reserves the right to cancel the allotment of ParaMedical courses and Colleges.

47. Candidate selected for admission to Para Medical Courses are not guaranteed anyappointment in Government Service.

48. COMMUNICATION :-

a) All intimation for communication to the candidates will be sent only to the mailing addressgiven by the candidate at the time of application. The Intimation / Call Letters will either besent by Registered Post or Speed Post or Courier Service or e.Post or they can be downloadedfrom the Websites www.tnhealth.org & www.tn.gov.in. as per instructions on Page 2 of thisProspectus whichever is feasible depending on the availability of time before counselling.Selection Committee will not be held responsible for Postal Delay and non viewing of websites.

b) Candidates should inform the change of address, immediately to the Selection Committee.If the change of address is not intimated to the Selection Committee in time, the SelectionCommittee will not be responsible for delay in communication or its consequences.

49. IX. Hepatitis – B Vaccination :-

All candidates selected for admission to first year para medical course should take theHepatitis – B vaccine before admission and to produce a certificate as in Annexure –VI

X. Ragging :-

50. The candidates who join Paramedical courses should not indulge or participate in anykind of ragging throughout their study period. If they are found to have indulged in raggingat any time, the candidate will be removed from the roll of the Institution at whatever stageof study and criminal action will be taken against the candidate

51. Candidates applied for Paramedical courses are deemed to have read the contents inthe prospectus and agreed with all the conditions / clauses and will not have the right tochallenge any of the clauses contained in the prospectus

ANNEXURE XIV (a)

INSTRUCTIONS FOR NATIVITY CERTIFICATE :

1. This Certificate should be issued by an officer of the Revenue Department not below the

rank of Tahsildar in the Taluk concerned.

2. This Certificate should not be issued by the Special Tahsildars, Deputy Tahsildars such

as Loans, Land Acquisition, Election, Excise and HQs, Deputy Tahsildars, Special Deputy

Collectors, Assistant Commissioner of Agricultural Income, Excise, Elections etc.

3. The Certificate should bear the stamp of the office of the Officer signing the Certificate.

4. Any foreign national irrespective of the period of study in the State will not become

eligible to apply for this course. No certificate of Nativity in the State of Tamil Nadu,

should therefore be issued to Nationals of other countries for this purpose.

5. The certifying officers should insist upon clear proof of the Nativity of the Parent or Guardian

of the candidate and satisfy themselves on the genuineness of the residential qualification.

They should be particularly careful in the case of candidates whose mother tongue is not

Tamil and whose place of birth is other than Tamil Nadu and who have studied VIII to XII

standards outside Tamil Nadu.

6. Residence Certificate will not be considered as Nativity Certificate.

7. The guidelines prescribed for the issue of Nativity Certificate and they are as follows as

per Letter No.RA.V(B)16932/2000, dated 3-4-2000 of the Special Commissioner and

Commissioner of Revenue Administration, Chepauk, Chennai-5.

(i) The parents/guardians of the applicants/students or the applicants themselves

should have permanently resided continuously for a period of five years in Tamil

Nadu.

(ii) Permanently residing for a period of five years should be supported by documentary

evidence.

(iii) The family ration card, Electoral Roll, Census List if taken recently, documents

like sale deed, tax receipt etc. relating to the property owned by either of the

parents or by the applicant may be verified.

(iv) The Transfer Certificate issued by the School authorities where the applicant had

studied last may be verified to know whether he/ she was in the State for five years.

(v) Enquiry in the village / place of residence of the neighbour / Village Administrative

Officers regarding continuous residing.

(vi) To ensure that wrong or incorrect address had not been furnished to obtain the

certificate.

(vii) The birth place, place of residence of the parent / father, permanent assets, mother

tongue, place of education, place of marriage of the applicant / parents, the period

of stay in and outside Tamil Nadu can also be considered before issuing certificate.

INSTRUCTIONS FOR FILLING UP THE APPLICATION FOR PARAMEDICAL COURSESADMISSION 2014 – 2015

1. APPLICATION CONSIST OF TWO PARTS: -Part 1 : Application Form Part 2 : OMR SheetThe Application Form contains 24 items. Sub group of the items of the application isreproduced in the OMR Sheet.

2. METHODS OF ENTERING PARTICULARS IN THE APPLICATION: -

Particulars required by an item are to be entered in the Application in long hand or in codedform depending on the item. “Address for Communication “ for example, is written in longhand. For certain items, like “ Medium of Instruction “, several alternative responses arelisted in tabular form. Each alternative is assigned a code. You must encircle the codewhich will signify your response to that item. In the case of certain items like Native District,you respond by writing a code in the box or boxes provided.

3. SUPPORTIVE DOCUMENTS: -

Documents / Certificates are required in support of the data furnished by the candidate forthe following :- Date of Birth, Community Certificate, if applicable, marks in qualifyingexamination, Nativity Certificate, if applicable and claim for Special Category seats, ifapplicable, first graduate if applicable.
