Employee Engagement Survey

Next month the fall administration of the Employee Engagement Survey will take place. We hope you will participate! In preparation, we are taking the time to answer frequently asked questions about the Employee Engagement Survey.

Why do we do the survey?The results of the Employee Engagement Survey are used by building, department, and district leaders as part of our continuous improvement cycle. It is a measure that we use on our district, building, and department scorecards. More importantly, we want to be a great employer. We want feedback from you on how to be a great employer.

October is Breast Cancer Awareness Month: Although you cannot prevent cancer, some habits that can help reduce your risk are:

• Maintain a healthy weight • Stay physically active • Eat fruits and vegetables • Do not smoke • Limit alcohol consumption • Inline image 1

Early detection provides the greatest possibility for successful treatment. When breast cancer is detected early, and is in the localized stage, the 5-year relative survival rate is 100%. An Early Detection Plan includes monthly breast self-exams, regularly visit-ing your doctor for scheduled clinical breast exams, and following your healthcare provider's recommendations for mammograms, which will depend on your age and health history.

Why do we provide Biometrics Screening for our employees?On site biometric screenings provide vital health information, real-time results, FREE of charge. In less than 15 minutes you will be provided with: • feedback about your possible risk factors for diabetes, heart disease or hypertension. • data about yourself that can be used towards setting personal goals • full lipid panel, blood glucose and body composition index • discount on your monthly health insurance premiums You are able to use this important data at your next physical. You also are provided with 4 free EAP sessions that can be used for per-sonal/professional coaching, counseling, OR Health/Wellness coaching.

Wisconsin Deferred CompensationOn October 12th and 13th Wisconsin Deferred Compensation will be on-site to do a presentation on the 457 retirement plan. They will be giving a group presentation on October 12th at 3:45 PM in the Middle School Auditorium. Flu Shots are covered at 100% through the insurance prior to the deductible they are offered at various locations throughout the community and may be run through the insurance. If you choose to have the flu shot at a location that does not process insurance you may submit the claim to BPA for reimbursement by using the Benefit Plan Administrators Claim Form located on the HR Website. The Cardinal Healthy Committee is currently reviewing the possibility of on-site flu clinics at each building and will send more information when available.

Employee Recognition

Jesse Pukrop, 5th Grade Teacher, Southview Years of Service with CFSD: 14 In his spare time Jesse enjoys visiting the local driving range with his daughters in an attempt to improve his struggling golf game and exploring the bike trails of Eau Claire with his family. Jesse continues to make positive connections with his students and their families. He recognizes the value in building relationships with his students so they want to work to meet the high expectations Jesse sets for them. Thank you Jesse for helping to prepare our students for a successful future.

Rebecca Schneider, 1st Grade Teacher, ParkviewYears of Service with CFSD: 21In Rebecca’s spare time she loves to travel, kayak, read and dig in garden dirt! Rebecca’s family loves to bicycle, hike, camp, bake and harvest veggies from their community garden plot. Since, Rebecca is originally from San Diego, CA, she still loves any chance to go exploring on a beach or near a tide pool. Rebecca has recently returned to playing piano. Rebecca and her husband Matt love watching their two daughters develop their interests as Middle Schoolers and most free time is spent at their events. Rebecca is a dedicated educator who continually goes above and beyond for her students and families. Rebecca works with her grade level team, resource teachers, and support staff at Parkview to provide a safe and caring environment that is filled with high expectations and success stories!

Holly Weiland, Administrative Assistant, VoyagersYears of Service with CFSD: 1 In her spare time Holly enjoys spending time with family, watching her boys play sports and watching the Green Bay Packers, Wisconsin Badgers and Milwaukee Brewers. Holly comes to work each day with a positive attitude and she is ready to support staff in any way possible. She provides excellent customer service internally to staff and externally in the areas of Adult Community Education, Family Connections, Mental Health, Volunteering and ATOD Prevention. Holly is very solution focused and is a great resource to the district. She is very appreciated!

Rick Fjelstad, Lead Custodian, Jim Falls Years of Service with CFSD: 8 In his spare time Rick enjoys coaching football and softball and hunting. Rick spent the summer making sure the building looked great for the return of staff and students! Rick works extremely hard to make sure everyone has what they need and never passes on an opportunity to help a co-worker. We are lucky to have him in our building!

Cathy Magee, 8th Grade Special Education Teacher, Middle School Years of Service with CFSD: 6 In her spare time Cathy enjoys biking, swimming, and spending time with her family. Cathy brings great joy to all of us here at Chippewa Falls Middle School. She always has a smile to share while working with students and colleagues. Cathy serves in multiple leadership roles in our school and district and is one of the strongest student advocates we have. She is a true CFMS ROCKSTAR!

Sue Zoromski, Teacher, Chippewa Valley High School Years of Service with CFSD: 20In her spare time Sue enjoys reading, writing, scrap booking, traveling, and her cats. Even though Sue is a veteran English teacher, she continues to keep up with best practice in her content area. This past summer, she spent six weeks at the Greater Madison Writing Project Summer Institute learning about all things related to writing - teaching, learning, assessing, etc. Sue is a great role model in terms of being a lifelong learner and continues to be an asset to the overall pro-gramming at CVHS.

Lynn Bergerson, Assistant Cook, Stillson Years of Service with CFSD: 11In her spare time Lynn enjoys spending time with her grandchildren, and children. Lynn also enjoys fishing, camping, boating and fun in the sun. Lynn is a great person! Lynn is positive, considerate, and friendly to all with an amazing work ethic. Her kindness touches students and staff on a daily basis. Thank you for Being Your Best!!!

Monika Lapoint, Teacher, High School Years of Service with CFSD: 20 In her spare time Monika enjoys most outdoor activities--boating, biking, running, and walking her dog, Teddy. Monika is interested in current events and likes catching up with the news. Monika also likes to read, and enjoys non-fiction books, usually political in nature. Monika is completely dedicated to maximizing student learning and opportunities. She is willing to take risks, to ask the difficult questions that need to be asked, and most importantly, if there is a way to help students, she will do whatever it takes to make that happen. Monika is an outstanding teacher leader and a valuable member of the High School staff!

New Deductible UpdateEffective January 1, 2017 the deductible will be increasing for the family plan to $6,000 per family, however due to a new Affordable Care Act law the deductible will now be $3,000 per individual on the family plan with a combined deductible of $6,000 for the entire family. Also the out of pocket maximum per individual on the family plan will be $3,500 with a maximum out of pocket expense of $7,000 for the entire family. What this means: If one person on your plan accrues $3,000 in medical bills the insurance will begin paying at 90% for that individual until they reach $3,500 in out of pocket expenses and then the insurance will begin paying 100% for that individual for the remainder of the calendar year.

Mandatory Reporting Wisconsin Act 81 (effective December 9, 2011), changed Wisconsin law to require all employees of Wisconsin public school districts to report suspected child abuse and neglect.

In addition, school districts must require all employees to receive training provided by the Department of Public Instruction within six months of initial hiring and at least every five years thereafter. Insurance tidbits ...

Injured?If you or a covered dependent is injured after seeking medical attention, call BPA at (800)236-7789 to expedite the processing of your claims more quickly. This will also prevent you from having to complete a questionnaire in regards to the injury. If you receive the questionnaire in the mail you may call BPA which will assist in processing claims more quickly. Then there

would be no need to complete the questionnaire and return it by snail mail.
