BackgroundBackground

• Free seasonal influenza vaccination provided to all residents of Residential Care Homes (RCHs) for the elderly (RCHEs) since 1998

• The SIV vaccination programme extended to healthcare workers in RCHs and RCH for persons with disabilities (RCHDs) in Hong Kong

• Since 2009, free pneumococcal vaccination (23vPPV) were also provided to RCHE and those residing in RCHDs of age 65 years or above, who have not received pneumococcal vaccine before.

• RCHs invite Visiting Medical Officers (VMOs) to deliver seasonal influenza vaccines (SIV) and pneumococcal vaccine (23-valent pneumococcal polysaccharide vaccine, 23vPPV) and to eligible residents and healthcare workers in RCHs

• The Government will reimburse VMO $50 per vaccine injection

• To enhance public-private partnership in medical care to residents in RCHs

• A total of 380 VMOs were enrolled under RVP 2012/13

– Seasonal influenza• From 1 Nov 2013 (preferably provide vaccination

to recipients before 31 Dec 2013 for better protection of clients)

– Pneumococcal vaccination• Ongoing since commencement in October 2009

– Seasonal influenza vaccine• Vaxigrip(Sanofi Pasteur): Trivalent Inactivated Vaccine

(TIV)• Vaccine Composition

– an A/California/7/2009 (H1N1)-like virus – an A(H3N2) virus antigenically like the cell-propagated

prototype virus A/Victoria/361/2011– a B/Massachusetts/2/2012-like virus

– Pneumococcal vaccine• Pneumo-23 (Sanofi Pasteur): 23-valent pneumococcal

polysaccharide vaccine (23vPPV)

RVP 2013/14RVP 2013/14Eligibility for Vaccination: same as last year

• Seasonal Influenza vaccine – ALL residents and ALL healthcare workers in RCHs

• Pneumococcal (23vPPV) vaccine:– ALL residents in RCHEs who have never received before; and– Residents aged 65 or above in RCHDs who have never

received before– Healthcare workers in RCHs are not eligible

• Vaccination given to non eligible persons will not be reimbursed.

• Consent by eligible residents and healthcare workers

• DH supplies and delivers vaccines, consent forms, sharp boxes and immunization cards to RCHs before the scheduled vaccination

• Injection fee same HK$50 per vaccine injection

• Vaccination Office will arrange collection of clinical waste (sharp box) and un-used vaccines in RCHs by professional company

• In-charge of RCHs confirmed that the guardian of the resident cannot be contacted and signed on the consent form

• For the benefit of the residents without consent form, VMO can provide vaccination to them after assessment and considers the vaccination is in the best interest of the them

• Other relatives of residents (if any) will be encouraged to sign on the consent form to acknowledge that VMO will provide vaccination to the residents.(Optional)

• Preparation

– Ensure enrolled successfully prior to service

– Collect all consent forms from RCH 10 days before vaccination date

– Should perform prior checking of vaccination history for all recipients in eHealth System (eHS) and create account for new eligible clients

– Date and time for vaccination

– Adequate number of vaccines are ready and properly stored in RCH

– Availability of necessary materials/ equipment for administration of

vaccines

• Responsibility of VMO to ensure safe and quality delivery of vaccination to recipients

– Ensure the recipients’ vaccination record have been checked through eHS, vaccination card or history from recipients or relatives

– Oversee the whole vaccination process and ensure personnel involved in vaccination are qualified/trained to perform vaccination duties

– Ensure the personnel involved in vaccination are qualified and trained in emergency management of severe immediate reactions and are equipped to do so

– Check the consent form, identify of the recipients and the medical conditions of the recipients before vaccination

– Ensure correct vaccines to be given to recipients and the vaccines are in good condition

– Record in vaccination card

– Check and sign on the consent list

• VMO to supervise and stand-by for emergency

Code of Professional ConductCode of Professional Conduct

• All VMOs should follow the Code of Professional Conduct issued by the Medical Council of Hong Kong

• To provide quality and safe care health service• Fail to do so may be subject to administrative

sanctions• Referral to Medical Council of Hong Kong if

necessary

• Prior checking of vaccination history in eHealth System, vaccination cards and history from recipients or relatives, especially for those who are newly-admitted to RCHs

• For recipients whose vaccination history have not been checked through eHS, another day for vaccination should be arranged

Scientific Committee on Vaccine Preventable Diseases: DO NOT recommend anyone to receive more than two doses of 23vPPV

• Separate injection for SIV and 23vPPV

• Double check name of vaccine and expiry date before administration

• Report vaccination incidents to Vaccination Office as soon as possible

• Scheme equipment eg authentication token for eHS operation- If lost or damaged, administration charge will be made

• Late claim- Claim for vaccination fee not made within 7 days, the Government shall have the absolute discretion to refuse payment

- All late claims submitted after 90 days will not be considered.

• Vaccination fee are not allowed to share with RCH or in-charge person of RCH.

• Prior checking the eligibility of clients for vaccination (duly signed consent form Vs eHealth system)

• Provide vaccination to recipients during the RVP 2013/14 vaccination period (on or after 1 November 2013)

• Any claim for reimbursement not made within seven calendar days considered as a LATE CLAIM, and the Government shall have the absolute discretion to refuse payment of any vaccination fee to a VMO

• All late claims submitted after 90 days will not be considered

• Make Claims under RVP instead of Vaccination Subsidy Scheme (VSS)

• Arrange mop up vaccination within vaccination period
