Office Hours

Our office hours are Monday through Friday from 8:00 a.m. to 4:30 p.m., except during company holidays. AFTER HOURS: We have a nurse on call 24 hours a day, 7 days a week to ensure that you receive necessary hospice and home health services. Our work week begins 12:00am Sat and ends Friday 11:59p

Educate all patients, families, caregivers and facility staff to call us when:

1. A change in condition including Increased pain

Any new or worsening symptom or complaint

Nausea/Vomiting

Increased restlessness

Decrease in level of alertness

Breathing becomes difficult

2. Questions about medications or treatments

3. Prior to ordering any new medication, equipment, medical transportation, lab services, seeking emergent care or medical services including hospital, rehabilitation or long-term care

Company Benefits

Employees are paid on a Bi-weekly Pay cycle. The Established payday is Friday

Medical, Dental and Vision Insurance

Suncrest Home Health and Hospice offers medical insurance coverage to their Full-time employees, beginning on the first day of the month following 30 days of the full-time start date. For example, if the full-time start date is March 15th, insurance benefits will be effective on May 1St

An employee will have 30 days, from start date, to elect either to participate or waive medical coverage. After the initial 30 days, an employee will have to wait until the next benefit open enrollment period to enroll into the health insurance, unless there is a qualifying event.

Examples of qualifying events include:

Adding spouse due to marriage

Spouse loss of job/medical insurance coverage

Divorce

Government Directed

Newborns/Adoptions: Need to be added within the first 30 days of life

401(k) Retirement Savings Plan

The Company offers a 401(k) Retirement Savings Plan to employees. The terms and conditions of the 401(k) Retirement Savings Plan are controlled by the applicable plan document.

Employer Match Plan Specifics:

1. Full-time employees are eligible for company match 90 days after their start date.

2. Employees are 100% vested in employer match, 90 days after start date.

3. Employees can enroll into 401(k) benefit plan after 90 days of employment. Employees must remain on plan for a minimum of 90 days. If employee leaves plan, after 90 days, then a mandatory 90 day waiting period will be enforced before being eligible to re-enroll.

Absenteeism & Tardiness

Excessive absenteeism or tardiness, excused or unexcused, can result in discipline, up to and including termination. If you need to make any changes to your schedule, including cancellation or absenteeism, you must personally notify your supervisor. Notification should occur, at least, four (4) hours before the beginning of the workday, so that proper arrangements can be made to handle work during your absence. Of course, some situations may arise in which prior notice cannot be given. In those circumstances, you are expected to notify your supervisor, or the office Administrative Assistant, as soon as possible, but no later than 30 minutes after your scheduled start time. If you are required to leave work early, you must personally contact your supervisor and obtain his/her permission. When absence is due to illness, the Company may require appropriate medical documentation. The Company will not hold the position of an employee who does not appear for work or call in to explain his/her absence for three or more scheduled workdays and will consider this to be job abandonment. Exceptions may be made for emergencies..

Gift and GratuitiesEmployees shall not seek or accept for themselves or for others any gifts, favors, entertainment, meals or payments nor shall they seek or accept personal loans, from any persons (patients, families or caregivers) or business organizations that seek to do business with, or are a competitor of the Company.

Damage to PropertyWe have made an investment in our building and equipment in order to better serve our customers and to make your job easier. Deliberate or careless damage to the Company’s property, as well as damage to your co-workers’ or customers’ property may result in discipline.

Workers’ Compensation Insurance

If you get an injury or illness on the job, your employer is required by law to provide workers’ compensation benefits. The Company pays the entire amount of the Workers’ Compensation insurance premium, which provides benefits to employees who experience injury or illness that arises out of the course and scope of your employment.

Benefit entitlements are governed by law, but it is essential that you report all work related accidents, injuries, and Illnesses Immediately To Human Resources. In the event of an injury or illness on the job the Company will require the employee to complete an Employee Incident form and comply with the Worker’s Compensation procedures.

Driving Company, Personal or Other Vehicles You must hold a valid state driver’s license for the class of vehicle you are driving. Further, you may

never use a motorcycle to conduct either business or provide transportation for a customer or fellow employee. All passengers are required to use their seatbelts. Not using seatbelts while driving on Company business may lead to disciplinary action, up to and including termination. Only people authorized by your supervisor can be passengers in Company and personal vehicles, while performing work duties. Permitting unauthorized passengers may lead to disciplinary action, up to and including termination.

You must notify the Company immediately of any change in the status of your driving record. Any employee whose duties include the operation of vehicles and is convicted for D.U.I. or for any other serious moving violation will be considered to have an unacceptable driving record and his or her continued employment will be subject to review. Any employee whose duties include the operation of vehicles who becomes uninsurable under the Company’s liability policy will be considered to have an unacceptable driving record and his or her continued employment will be subject to review.

If an employee receives a traffic citation while operating a Company or customer vehicle, or their personal vehicle while on duty, the employee will be responsible for paying any fine or penalty. If an employee is involved in a traffic accident while operating a Company or customer vehicle, or their personal vehicle while on duty, the employee is required to call a police officer to the scene of the accident. The employee must report the accident immediately. Do not attempt to render medical care or assistance beyond your ability.

Mileage

Keep Insurance up to date

Employees who use a personal vehicle in the performance of their duties must keep their personal vehicle in good working order and comply with the state insurance laws governing liability, property damage, and bodily injury. Proof of current personal vehicle insurance must be submitted and will be kept in the personnel file.

When new insurance cards are issued please update the office.

What is a Commute? For an employee working in the office, a commute

is defined as the travel from home to the office and from the office back home, at the end of the day. This will not be reimbursable mileage.

For an employee who works out in the field, a commute is defined as the travel from home to the first visit of the day and from the last visit of the day back home.

ADP mileage entries are due by 1:00 on the Monday Before Payroll.

For those employees that work out in the field –

If your first or last visit location is further from your home than the office, you will be able to get the difference in mileage reimbursed. Suncrest Home Health and Hospice will reimburse mileageonly for the miles between the office and the first or last visit. You will not get paid for the time it takes you to get to the first visit of the day or from the last visit of the day back to your home. For example if the office is 25 miles away from my home but my first visit of the day is 40 miles away, I would be reimbursed for 15 miles of travel. (40miles – 25 miles = 15 miles). If your first or last visit is the same or shorter distance from your home than it is considered a commute and you would not be reimbursed for the mileage or be paid for the time.

Safety

It is our policy to promote safety on the job. The health and well-being of our employees is foremost among our concerns. For this reason, you are urged to follow common sense safety practices and correct or report any unsafe condition to your Supervisor.

Each employee is expected to assist Suncrest Home Health and Hospice in maintaining safe working conditions. Safety is a state of mind and requires constant vigilance and common sense. Safety is everyone’s responsibility. Remember: SAFETY FIRST.

All accidents -- including those which do not involve serious injury and those involving customers -- must be reported immediately to your Supervisor It is only through full knowledge of every accident that the Company can become a safer, healthier place to work for everyone.
