Aim of the CongressThough research in the field of muscle pain is growing rapidly, most of the theories regarding pathophysiology are still not well explained and debatable. This world congress is aimed to review the current basic science research, mechanisms, laboratory findings supporting the theories, human and animal experiment models, assessment and clinical applications, treatment modalities and prognosis of muscle pain. This aim will be accomplished by exploring and elucidating the mechanisms in muscle pain, various muscle nociceptors and their properties, sensitization at different levels, animal studies and human experimental studies, sequelae in brain due to chronicity, assessment and effective treatment of muscle pain. It would provide a much-needed platform for basic science researchers and practicing clinicians to share the current research about muscle pain, eventually leading to improved clinical outcomes in patients with muscle pain.

Learning Objectives of the CongressUpon completion of the congress, the participants will be able to

Understand the current models regarding mechanisms of muscle pain. Learn about various muscle nociceptors. Understand about sensitization of nociceptors, modulation in muscle spindle activity, sensitization at spinal cord and higher level. Understand the neurophysiology and role of biochemical mediators in muscle pain. Understand the neural and psychobiological mechanisms of muscle pain. Know about animal models and laboratory studies on muscle pain. Understand muscle hyperalgesia, referral patterns, muscle tone and strength in relation to muscle pain, muscle fatigue and its role in

muscle pain. Understand the relation between exercise and muscle pain. Understand the changes in brain due to chronic muscle pain. Know about various standardized, valid physical and subjective measurement tools that are used in muscle pain. Know about various novel treatment approaches in management of muscle pain and thereby design an appropriate treatment

    FASCIAL MANIPULATION FOR LOCOMOTOR SYSTEM

HALL 1, ITC GARDENIA 08 Hours

PROGRAM OBJECTIVES: Learn anatomy and physiology of the fascial system. Learn how to evaluate myofascial planes: upper and lower

extremities, spine. Determine functional testing of areas of complaint. Develop skill and understanding to assess abnormal functional

findings with fascial points of involvement to restore normal function.

ABOUT THE COURSE:Fascial Manipulation is a manual therapy method that has been developed by Luigi Stecco, an Italian physiotherapist from the north of Italy. This method has evolved over the last 40 years through study and practice in the treatment of a vast caseload of musculoskeletal problems. It focuses on the fascia, in particular the deep muscular fascia, including the epimysium and the retinacula and considers that the myofascial system is a three-dimensional continuum. The fascia is very extensive and so it would be difficult and inappropriate to work over the entire area. The localisation of precise points or key areas can render manipulation more effective. An accurate analysis of the myofascial connections based on an understanding of fascial anatomy can provide indications as to where it is best to intervene. This workshop aims to cover the facial manipulation principles for the locomotor system.

(to your client!) The real function of the hamstrings Why restricted deep lateral rotators may lead to rotator cuff issues When to use a clients hand movement to mobilise their adductors

ABOUT THE COURSE:Active Fascial Release has been developed by James Earls, a structural integration practitioner and Anatomy Trains teacher. It blends his understanding of myofascial continuities and functional movement principles with hands-on manipulation of the soft tissue to create a variety of therapeutic effects.Active Fascial Release combines elements of motor control theory with

a neuro-myo-fascial approach to identify, treat and finally re-educate the clients movement patterns. The blending of guided movement with various soft tissue engagement protocols has created a method that addresses the body as a whole and acknowledges the roles of the bodys various tissues and their interrelationships.Active Fascial Release is a dynamic and thorough method of assessing, treating and re-educating soft tissue. It is designed for the manual therapist who wants to incorporate movement into their treatment. By using AFR you are giving the client the potential to educate themselves on the wonderful interrelationships through their body. The learning of Active Fascial Release will also develop a functional and 4 dimensional understanding of the body an essential aspect of modern manual therapy.

DANIS BOIS METHOD FASCIATHERAPY, PSYCHOTONIC TOUCH AND PAIN

HALL 3, ITC GARDENIA 08 Hours

    ABOUT THE COURSE:The objective of this workshop is to present the concept of Psychotonus and Psychotonic Touch as developed by DBM Fasciatherapy. This technique is oriented to fascia, and is a body-mind integrated approach. The aim of this workshop is first to present the theoretical field of DBM Fasciatherapy, and then allow participants to experience this specific touch during hands-on sessions. The hands-on part will be the main purpose and will show to delegates how to evaluate and regulate non-specific disorders of the fascial Psychotonus.Fascia, which is often seen as a packing organ, is generally cleared away during dissections in order to see something, and has been largely neglected in anatomy books. This Cinderella of orthopedic science is now a growing field of research and the number of papers on this organ has been growing exponentially over the last 15 years. The role of fascia in sensitivity and pain awareness has now been generally accepted, and manual therapists think that fascia could be a new paradigm for the understanding of the outcomes of their techniques. The existence of a fascial tonus is now well documented, and the link between modifications of this tonus and suffering pain have been evoqued.DBM Fasciatherapy is a body-mind approach which stimulates the physiological plasticity of the fascia, and in particular the modulation of its tonus. The research carried out on the psychotonic touch of DBM Fasciatherapy shows an action on fascial and vascular stress and anxiety .It also shows a modification of the patients perception of their own body, and a reinforced sense of self.This full day workshop is designed to introduce the theoretical basis of DBM Fasciatherapy to manual and movement therapy practitioners (morning session), and to allow them to experience this body-mind integrated manual approach (afternoon practice). The participants will begin to learn psychotonic touch, and how it can be applied with patients suffering from various forms of musculoskeletal disorders, pain, mental suffering, and symptoms related to stress.Over the course of the workshop, the following will be covered: Presentation of psychotonus as a distinguishing feature of the body-mind connection of the fascial system, and the manifestation of the living dynamics of human fascia; Presentation of the action of psychotonic touch on fascial and vascular stress, and on changes in patients general body perception resulting in a reinforced sense of self;

mechanical stress transfer (traction or/and compression) to a targeted dysfunctional tissue. The outcome is a reciprocal reaction from the body that involves biochemical, signaling, metabolic, and finally physiological responses. This is primarily a tuition process, in the search for a restored homeostasis: recovering range of motion, appropriate tension, strength and, mainly, movements coordination. The term induction relates to the recovery of movement facilitation rather than a passive stretching of the fascial system.

    FASCIAL DISTORTION MODEL

HALL 4, ITC GARDENIA 08 Hours

ABOUT THE COURSE:The Fascial Distortion Model (FDM) is an anatomical perspective, originated by US physician Stephen Typaldos, D.O., in which the underlying etiology of virtually every musculoskeletal injury (and many neurological and medical conditions) is considered to be comprised of one or more of six specific pathological alterations of the bodys connective tissues (fascial bands, ligaments, tendons, retinacula, etc.). As a model, the FDM is an abbreviated interpretation of the pathology of fascial injuries and contemplates the structural consequences of orthopedic, medical, surgical, and manipulative interventions.

Some common injuries that are easily resolved with FDM manual treatments include: pulled muscles and muscle tears, sprained ankles, shin splints, Osgood-Schlatter Disease, whiplash, headaches, shoulder pain, frozen joints, kidney stones, plantar fasciitis, sprains, strains and tendonitis, and low back pain. Chronic pain can also be successfully treated using the FDM, but results typically take longer than for acute injuries. Sciatica, carpal tunnel syndrome and many other injuries can also be successfully treated in this model..

The Structural Integration series of sessions helps people who are suffering from chronic pain (myofacial/connective tissue) problem with no resolution. Structural Integration aligns and balances the body by lengthening and repositioning the fascia. As fascia is lengthened it allows the muscles to move more efficiently. The practitioner will apply pressure to the body, working the entire fascial system in a systematic way. When restricted fascia is released and lengthened the body can return to its structurally optimal position. Structural Integration system of sessions uses slow strokes and deep pressure on contracted muscles, while the client performs small muscle movement to optimally stretch the connective tissue/fascia. Oils or lotions are sometimes used. In each of the sessions, connective tissue is released, realigned, and balanced throughout the whole body. Experiencing the Structural Integration System (10 or more separate sessions where each part of your body will be addressed from head to toe, side to side) enhances your posture and freedom of movement along with improving along with improving your emotional perspective, too. Structural Integration System is made of 10 sessions and are generally divided into 3 distinct sections as follows: Section 1 refers to opening the sleeve sessions, Section 2 refers to the core sessions and Section 3 refers to the integration sessions. When a body is aligned and balanced it moves with greater ease. It requires less energy to function. Good posture is effortless and breathing is easier. The body becomes more flexible, more coordinated and athletic performance improves. This workshop focuses on the structural integration approach to the lower limb.

PROGRAM OBJECTIVES: Manual techniques can be used as the first-line in interventions, prior to other invasive and costly ones.ABOUT THE COURSE:

Flossing is the use of elastic bands which are wound circularly around joints and tissues. The Flossing was originally developed by American physiotherapist and cross fitness coach Dr. Kelly Starrett. The method was later developed by Sascha Seifert and Dominik Suslik in collaboration with renowned speakers from the fields of performance and therapy. The training method is based on compression-based techniques with an elastic latex band, the so-called raft band. This

systematic use is applied here according to therapy principles from osteopathy and physiotherapy, in conjunction with the functional and the athletic training. Vitality Flossing, in addition to muscular dynamics such as pendulum, swing, and fascia training, is particularly important in the areas of joint mechanics and tissue lubrication. The seminars show how to recognize mobility deficiencies and how to work with the flossing technique. According to the motto: Optimize by Compressing!

Adhesive Capsulitis (AC) is an enigma wrapped in a mystery! Why does it affect more woman than men? Why only in the over 40 age group? It is so severe, so why does it never return in the same shoulder? Why is it self-limiting to 30 months? Why is it 10 times more common in diabetics?

Like all enigmas, once the code has been cracked, AC offers us a deeper insight into the inner workings of the shoulder, musculoskeletal and nervous system. Simeon will explore AC from an Osteopathic perspective, demonstrate his trigger point based approach to treatment and management and offer a novel concept for trigger points theory.

A STRUCTURAL INTEGRATION APPROACH TO THE SPINE

HALL 1, ITC GARDENIA 3 Hours

ABOUT THE COURSE:This course teaches the important treatment techniques (informed by current research) necessary to fully rehabilitate those with chronic or acute myofascial pain and fibromyalgia. Low Back, buttocks and hip pain causes untold hundreds of millions of dollars of lost productivity in the world each year. Athletes, exercisers, office workers are all impacted by myofascial problems and they can be prevented from participating in activities. There are 12 muscles that, when harboring trigger points, can refer pain to, and cause dysfunction in this area. There are also a large number of perpetuating factors that need to be assessed for in order to remediate mechanical, structural, biochemical factors that are creating or making the myofascial problems worse. This class, with the use of digital video media, and live-hands- on participation, will show dynamic and innovative treatment techniques for effective and lasting relief of pain. The quadratus lumborum can be a trick muscle to get at, we will show a fully stretched yet supported position to effectively treat this muscle. The spinal erectors, when treated in a standing torso-flexed position, or flexed and rotated, can illicit exquisite and amazing referral patterns. These dynamic positions allow for functional movement during and right after treatment techniques are applied and allow full access to all forms of manual therapy techniques including but not limited to: contract/relax/ further stretch, myofascial release, trigger point pressure release, fascial stretching, Lewitt technique, breath coordinated stretch. Billions of dollars are spent on costly medical interventions to the hip and low back. When the cause was simply myofascial trigger points, the pain often returns and the patient is labeled as a malinger or a failed case. Why not address the myofascial causes first, rule them out, then if necessary, continue

MYOFASCIAL FORCE TRANSMISSION IN UPPER QUADRANT. EVIDENCE FROM UN-EMBALMED CADAVERS DISSECTIONS

HALL 4, ITC GARDENIA 3 Hours

ABOUT THE COURSE:The concept of myofascial force transmission implies any kind of transmission from the full surface of a myofibril, excluding the direct participation in the myotendinous/myoaponeurosis unit, where force is transmitted in the aponeurosis/tendon and myo-myonal continuum. The findings, from the un-embalmed cadavers dissection, provide an understanding of body movements as the global response following the muscular fibers contractile input. Also, allow the improvement of clinical observations and decisions making, regarding the pathology patterns and synergisms of movement. The Fascial System based body movement model, may allow the developing of new and effective approaches to treat muscle injuries and diseases, such as muscular dystrophies, repetitive strain injury and spasticity. EFFECTS OF FOCUSED EXTRACORPOREAL SHOCKWAVE THERAPY ON MYOFASCIAL TISSUE

HALL 2, ITC GARDENIA 90 Minutes

Focused extracorporeal shockwave therapy (ESWT) could be a very helpful tool for the treatment and even for the diagnosis of myofascial pain. Focused ESWT are an acoustic energy, that can penetrates oft tissue even in the deeper layers (up to 15 cm). The use of focused ESWT for the treatment of many musculoskeletal pathologies and pain condition is considered to be very efficient, and there are numerous high level clinical studies available. With low to middle energy levels no tissue damage is done (high and highest energy is used for the destruction of kidney stones), but specific induction of healing processes and reduction of vaso-nociceptive- active substances (CGRP, substance P, bradykinin, etc.) have been found in basic scientific studies. The exact application in terms of location and dosage of focused ESWT allows the examiner of myofascial pain to collect reliable and reproducible data with the use of a medical apparatus. This will bring the specific myofascial examination to a higher level. Over the last years, soft tissue and muscle has become of interest for the use of focused ESWT. Direct muscle relaxation effects have been found for the treatment of spasticity. Recent basic studies show positive effects for the specific regeneration of muscle tissue. Soon, focused ESWT will play a more important role for the specific diagnosis and treatment of myofascial pain.
