ABOUT THIS REPORT

Hospitals play an important role in the health care system, as institutions that provide medical, surgical, and nursing care around the clock, and also as employers, research institutions, and centers to train the health care workforce. According to Centers for Medicare and Medicaid Services reporting, hospital care accounts for over 30% of all health care spending every year. As Oregon continues its large scale health care transformation efforts, tracking costs and utilization of hospitals is an important part of monitoring the impact of these efforts.

This report monitors and compares financial and utilization information for Oregon's acute care inpatient hospitals. Data are presented quarterly for the previous nine quarters, and the report compares the most recent quarterly data with like quarters from previous years. For example, quarter three 2015 results will be compared to quarter three 2014 and quarter three 2013 results. As hospital trends are affected by seasonal variations, more attention should be placed on longer term trends rather than quarter to quarter variation.

The data source for this report is Databank. Databank is a database of self-reported financial and utilization information. Hospitals submit Databank data to the Oregon Health Authority on a quarterly basis. As Databank data are self-reported, they are not audited and this report is not intended as a substitute for audited financial statements.

Kaiser hospitals do not report financial data to Databank, but they do submit utilization data. They are included in utilization reports. Kaiser Westside opened for business in August of 2013, and Shriner's Children Hospital moved to DRG status in the beginning of 2013. These facilities may not have past year data available for comparison. There have not been any hospital closures in recent years.

Types of Hospitals

In this report, hospital data is presented by four categories: All hospitals, DRG hospitals, Type A hospitals and Type B hospitals.

There are 60 acute care inpatient hospitals in Oregon. These are general care facilities and exclude federal hospitals, long term care and rehabilitation facilities.

There are 28 DRG hospitals in Oregon. These are typically large, urban hospitals that receive payments based on the prospective Diagnostic Related Group (DRG) system.

There are 12 Type A hospitals in Oregon. These hospitals are small - fewer than 50 beds - and are located more than 30 miles from another hospital.

There 20 Type B hospitals in Oregon. These hospitals are small - fewer than 50 beds -and are located within 30 miles of another hospital.

HIGHLIGHTS FROM Q3 2015

In quarter three of 2015, we pass the midpoint of the second year of major health policy reforms enacted under the Affordable Care Act (ACA). The Oregon Health Authority is committed to tracking and reporting on the impacts key policy reforms, such as the creation of the health insurance exchange and expansion of Medicaid eligibility to people with incomes up to 138% of the povertylevel. This report includes financial and utilization data through quarter three 2015.

Medicaid expansion has increased the number of individuals with health care coverage, shifting the payer mix for hospitals and reducing uncompensated care. Continuing trends from 2014, these changes have resulted in positive financial outcomes for Oregon's hospitals. Quarter three poised unique challenges for many hospitals as widespread stock market turmoil took a large bite out of total margins and non-operating revenue sources.

In Q3 2015 median total margin fell 48.1% when compared to Q3 2014. This is likely due to investment losses from poor stock market performance in 2015.

Charity care continued to fall across the state. Compared to Q3 2014, charity care fell by more than $90 million statewide.

More people visited the emergency room in Q3 2015 than in Q3 2014. Rates of newly insured people using the emergency room remain lower than rates of use for people with pre-expansion insurance.

Oregon inpatient hospital discharges rose 1.9% when compared Q3 of 2014. Increases in utilization are likely related to recent population growth as well as Medicaid expansion.

NET PATIENT REVENUE

Median net patient revenue is up 3.75% in quarter three of 2015 when compared to quarter three of 2014. DRG hospitals continue to post the most consistent growth in net patient revenue, gaining 7.3% year to year in quarter three. Type A and Type B hospitals have posted modest growth when compared to their previous quarter three results. Type B hospitals have now posted four straight quarters of net patient revenue gains, growing by a 15% since taking a hit in the fourth quarter of 2014. Net patient revenueremained strong in the face of across the board drops in admissions. Patient utilization will be discussed later in this report.

Net Patient Revenue is the amount a hospital expects to receive for services after accounting for contractual allowances to third party payers and for uncompensated care. This includes payments made for providing health care, such as surgeries, emergency services, lab tests, imaging, etc.

Net patient revenue is calculated by taking the total amount of money billed for patient care (gross patient revenue) and subtracting uncompensated care and contractual amounts.

OPERATING MARGIN

Statewide operating margins were down 26% in quarter three of 2015 when compared to quarter three of 2014. DRG and Type A hospitals took the biggest hits, dropping 40.6% and 114.6% respectively . Operating margins are down as a result of operatingexpenses increasing 1.18% from last quarter and operating revenue falling 2.78% from last quarter. Since net patient revenue is up slightly, the losses in revenue come from hospital operations not directly related to patient care. These sources, Databank's "other operating revenue" category, include things such as cafeteria sales, revenue from classes or events, conference room rentals andother sources of income the hospital directly controls. Statewide, hospitals other operating incomes were down 28.4% in quarter 3 when compared to quarter 2 of 2015.

Operating margins show little growth year to date 2015.

Operating Margin is calculated as a ratio of operating revenue minus operating expenses divided by operating revenue. Operating revenue includes net patient revenue as well as revenue from other non-patient hospital services, such as cafeteria sales. Operating expenses are the expenses associated with running a hospital, such as supplies, salaries, rentals, professional fees, and insurance.

A positive operating margin means that the hospital is making a profit, while a negative margin means the hospital is operating at a loss.

TOTAL MARGIN

Statewide median total margin fell 48.1% in quarter three of 2015 when compared to quarter three of 2014. This drop in total margin, and posting of total margins that are lower than operation margins is due to non-operating revenue losses in quarter three. Non-operating revenue primarily consists of investment income, donations and gifts. In quarter 3 of 2015 there was significant stock market downturn, with all major indexes posting sharp losses in August. This resulted in more than half of Oregon's hospitals reporting non-operating revenue losses. DRG hospitals are more affected by stock market volatility because they generally are the hospitals that have the size and budget to be able to make extensive investments.

Total Margin compares revenue and expenses from all sources, including investments, tax subsidies, and operations. It is calculated by dividing all sources of income by all expenses.

PAYER MIX

Statewide payer mix is holding consistent after redistribution as a result of the Affordable Care Act. Medicaid and self-pay payments remain steady state in quarter three of 2015 when compared to quarter three of 2014. Commercial payments remain lower than pre-ACA levels, but are showing signs of slowly gaining ground. Commercial payments have increased 2.9% from the beginning of 2015 through quarter three.

Statewide payer mixes have remained consistent throughout 2015.Percent of total patient payments

Payer mix shows the share of hospital revenue from different payer sources. It is calculated by dividing a payer's total charges by the total charges for all payers. A payer is an entity that assumes the majority of the financial responsibility for the hospital bill.

This report includes four broad payers: Medicare, Medicaid, commercial and self-pay. Since each payer reimburses hospitals at different rates, the payer mix can provide insight to the hospitals larger financial performance.

CHARITY CARE

Statewide median charity care continued to decrease in quarter three of 2015. Median charity care fell15.3% when compared to quarter three of 2014. Total charity care is down $92.6 million compared to the same time period of 2014.

This dramatic fall in charity care can be directly attributed to the expansion of Medicaid under the provisions of the Affordable Care Act. Medicaid expansion resulted in significant drops in uninsured hospitalizations and as a result, reduced the need for charity care to be offered by hospitals.

Median charity care remains at historically low levels in Q3 2015.Percent of gross patient revenue

Charity Care includes services that are provided for free or at a reduced cost to eligible patients. Each hospital or health system determines its own criteria for providing charity care based on factors such as income, employment status, or alternative sources of funds. The value of charity care is based on a hospital's full established charges and may be equal to the full charge or any portion of it the hospital elects to provide for free.

The total dollar amount a hospital waives may be expressed as a percent of its gross patient revenue for comparison purposes.

BAD DEBT

Statewide median bad debt has increased throughout 2015, but still remains lower than quarter three of 2014. Type B hospitalshave had their bad debt percentage steadily increase for the past four quarters and are close to approaching pre-ACA levels. While all hospitals saw an uptick in bad debt in quarter three, it is not immediately apparent why type B hospitals have been more affected, and for a longer period of time. Despite this uptick, hospitals have still combined to have $51.7 million less in bad debt year to date in 2015 than in the same time period of 2014.

Bad Debt occurs when patients do not pay all or part of their bill for services. It is an unpaid bill for care, based on a hospital's full established charges, for which a hospital expects payment but is unable to collect. Bad debt can include unpaid components of the total bill such as co-payments and co-insurance. Like charity care, bad debt is expressed as a percent of gross patient revenue.

UNCOMPENSATED CARE

Statewide median uncompensated is down 20.6% in quarter three of 2015 when compared to quarter three of 2014. While down year to year, all hospital types show small increases in uncompensated care, driven by increases in bad debt all hospital types saw. Though the first three quarters of 2015, hospitals have combined to have $144.4 Million less in uncompensated care when compared to the same time period of 2014. Total Statewide uncompensated care levels remain at the lowest they have been in the last ten years.

Uncompensated Care is the total of charity care and bad debt charges. It measures the total value of care, based on established charges, that a hospital provides without receiving payment. Because it represents care provided without reimbursement, uncompensated care can directly affect revenue and margin numbers.

Utilization Trends

INPATIENT DISCHARGESInpatient discharges dropped in quarter three of 2015, but still remain 1.9% higher than a year ago. Overall year to year increases in inpatient discharges can likely be attributed to several years of sustained population growth in Oregon, and the Portland metropolitan area in particular. Population data references are provided in the appendix. Quarter to quarter decrease experienced in quarter three is likely explained by seasonal factors. Inpatient discharges typically decrease in the summer months, when common hospitalization causes such as pneumonia and influenza are less common.

Inpatient discharge occurs when a patient is formally released from the hospital after being admitted and lodged for at least 24 hours. This report represents Inpatient Discharge is a count of hospitalizations. It is not a count of unique individuals, however, as a single person can be admitted to the hospital multiple times a year.

Inpatient discharges are acute care only and do not count rehabilitation, long term care or specialized psychiatric care.

Utilization Trends

EMERGENCY DEPARTMENT VISITS

Similar to inpatient trends, statewide emergency department visits are up year to year, but down quarter to quarter. Emergency visits have increased 1.9% compared to quarter three of 2014, but have decreased 2.2% from quarter two 2015. The drop from quarter two is likely explained by seasonal differences in hospital admissions. Like inpatient discharges, overall year to year growth is likely due to overall population growth in Oregon as well as the affects of Medicaid expansion.

Hospitals have seen 60,000 more ED visits in 2015.Total emergency room visits through the third quarterEmergency Department Visits are reported as the

number of visits to a hospital emergency department that do not result in a hospital admission. A person that visits the emergency department and is then admitted is counted as an inpatient discharge.

This report focuses on the count of ED visits, but cannot provide information about the rate of ED use - i.e. how frequently individuals in a given population visit the ED.

Utilization Trends

OUTPATIENT SURGERIES

Statewide outpatient surgeries have continued to show fluctuations. After increasing 8.3% in quarter two, statewide surgeries fell again by 4.5%. Year to year, outpatient surgeries are down 6.7% compared to quarter three of 2014. The elective and non-emergency nature of most outpatient surgeries can help explain why there is such variability in surgery numbers. Free Standing Ambulatory Surgical Centers (ASCs) are likely also playing a role, as they expand their market presence and take business away from hospitals. This is not necessarily a negative, as many health systems form business partnerships with ASCs, or even outright own ASC facilities.

Hospital outpatient surgeries continue to show quarter to quarter volatility in Q3 2015.Total count of quarterly outpatient surgeries

Outpatient Surgeries include surgeries performed at the hospital that do not require an overnight stay. These discharges consist primarily of non-emergency, planned services in which the patient is treated and released in the same day. Outpatient surgery numbers do not include standard lab work, clinic visits or most diagnostic imaging work.

The following appendix pages contain individual hospital results for the measures included in this report. Data are provided as year to date results through the second quarter for the past five years, from 2011 to 2015.

Kaiser hospitals are not included in financial measures, as Kaiser does not submit financial data for individual hospitals. Someinformation is not available for Kaiser Westside, as it recently opened, and for Shriner's Children's Hospital, as it recently became a DRG hospital.

As previously described, hospitals in Oregon are divided into three categories based on size and distance to other hospitals. Inaddition to these categories, there are two other key designations a hospital might have that can impact its financial and utilization outcomes. Those are health district and critical access hospitals.

A health district hospital is a one that is under the control of a formal health district. In most cases the controlling entity is the local county government. Being a part of a health district allows the hospitals access to additional funds from tax sources to contribute to operations. This access to tax funding allows many hospitals to continue to operate in rural areas when they otherwise could not afford to. Health districts may also provide funding to other types of clinics and providers.

A critical access hospital is designated by the Centers for Medicare & Medicaid Services (CMS). This designation impacts the reimbursement the hospital receives from Medicare. There are a number of specific criteria a hospital must meet to be considered a critical access hospital, but in general a hospital must be located in a rural area with limited access to otherhospitals. In exchange for providing additional services that the hospital might not otherwise provide due to the cost, Medicarewill reimburse the hospital at a higher rate than other hospitals receive for the same service. These services mostly relate toexpanded emergency services such as a 24 hour emergency room and ambulance transportation.

In the following pages health district hospitals are indicated with an asterisk (*) and critical access hospitals are designated with a Greek letter (ᶲ).
