OPT is temporary employment authorization allowing F-1 students the opportunity to gain practical experience in their field of study. The OPT application is a two-step process; the first requires an application to the IO, and the second the completed application must be submitted to USCIS. Complete this form along with the documentation listed on the Checklist to turn into the International Office during walk-in hours.

Program Information: To be completed by the Academic Advisor or DeanImmigration regulations require that Optional Practical Training be used by students for employment related to the student's field of study, usually after completion of all degree requirements have been met. Please return the completed form to the student for submission to the International Office. After a student's I-20 end date, they are no longer eligible to work on-campus without OPT approval.

The OPT start date can be no earlier than the day after your I-20 expiration date or later than 60 days after your program completion date. These dates cannot be changed once the OPT application has been submitted to USCIS.

By signing this form, you agree to adhere to all F1 regulations to maintain your status while you are on OPT. You also agree to report your employment status and personal information throughout your period of OPT, and acknowledge that failure to report may result in termination of your F-1 record. Your signature confirms that you have reviewed all of your application materials for accuracy and agree to mail your application within 15 days of the International Office recommending you for OPT.

*PhD students who are ABD are eligible to apply for OPT.
