These may include changes in contact information and/or personal information. Changes to personal information may include: changes in immigration status (expiration or change in temporary work permit), changes in family composition due to marriage, birth of a child, change of custody of a child, divorce/separation, death, etc., changes in employment, changes related to your Express Entry profile (eligibility or Comprehensive Ranking System score), and changes to your language test results or other program criteria. You may also use this form to request a correction of an error made while completing your online profile and/or application in the OINP e-Filing Portal. Please note that only data entry errors (i.e. spelling mistakes) will be accepted. F.31 (“FIPPA”). All information collected on this form is necessary for the Ontario Immigrant Nominee Program’s (the “Program”) proper administration. The Program is authorized under the Ministry of Citizenship and Culture Act, R.S.O. 1990, c. M.18, as well as by the Canada- Ontario Agreement on Provincial Nominees, executed under the authority of the Ministry of Citizenship and Culture Act, R.S.O. 1990, c. M.18 and O.C. 220/2015 executed under the Executive Council Act, R.S.O. 1990, c. E.25. Any personal information collected by MCI in connection with this form will be used for the purposes of ensuring the Program’s proper administration, as well as for Program integrity, Program evaluation, and statistical purposes. In this form, “personal information” has the same meaning as under FIPPA.
