Online Test Request System(sof t l ine) – User Guide

O n l i n e T e s t R e q u e s t S y s t e m

This is a web based interface application. It provides a convenient way to input test request information, check the data accuracy and print the Test Request Form.

The information that you need to provide are the same as that on the paper format.

There are 5 steps:

Step 1: Input General Information

Step 2: Input Details

Step 3: New Test Request Verification

Step 4: Service Agreement

Step 5: TRF Agreement

Onl ine Test Request System


Login

1) Open a web browser and go to http://www.testrequest.bureauveritas.com/.

2) Input your username and password and press Login.

In case you forget your password, you may press Forgot Password? for assistance.

After login



Step 1: Input general information

3) After login, click New Test Request to start from Step 1.

4) Select a Lab location, client name and Sample Type.

5) Press Next.



Step 2: Input Details

6) Input the test details.

7) After inputting, click Next to go to Step 3. If you did not input all mandatory fields, the system will not bring you to Step 3. Message box will pop up to remind you to input all mandatory fields.

8) Click Back to go back to previous step for changing location and client.



Step 3: New Test Request Verification

9) Verify the information that you inputted. You may scroll down the cursor to verify all of them.

• If you want to modify, click Edit and it will bring you to Step 1. Then you can start modification from Step 1.

• If no modification is needed, click Confirm to go to Step 4. After that, information could not be modified anymore.



Step 4: Service Agreement

10) Check the terms and conditions. You may scroll down the cursor to read all. Click Accept to accept the agreement and go to Step 5.



Step 5 : TRF Confirmation

11) To preview and print the TRF, click View & Print TRF. The Test Request Form will

show on another web browser. Click to print the form.

12) To input another New Test Request Form, click Go to Main Menu.

If you found any typo mistake on the form, you could not modify now. You may go to Main Menu and work from Step 1.



Test Request List

This lists the Test Request Forms (TRF) that had been generated successfully from Step 5. You can

select more than 1 criterion to filter the search results.

1) Select the searching criteria.

2) Click Generate.

3) The search results show at the bottom. Click the header for sorting and you can choose more

than 1 sorting criterion.

� Sort by descending order

� Sort by ascending order

Click “+” to add 1 more criteria.

The number besides the arrow indicates the sorting priority.

For example,

The search result first sort by “TRF number” in descending order and then sort by “Lab

Location” in ascending order.



Sample Pickup Request

We provided online request for sample pickup service for Hong Kong submitters to submit

sample to our Kwai Chung Lab.

1. Run Test Request List

2. Click Request Sample Pickup on the right hand side of the TRF

3. Input Remarks for Sample Pick

4. Click Submit to send out Sample Pick Request to BV

5. Click to confirm to submit the sample pickup request

6. The status of the sample will be changed to “Pick Request Issued”.



Logout

When finish using the system, don’t forget to logout.

1. Click Logout. And a message box will pop up for asking your confirmation.

2. Click OK to confirm logout.
