ONLINE SERVICES FOR TATTOO ESTABLISHMENTS

The following changes may be submitted online through the Online Service option for Tattoo

Establishments:

Change in Ownership

Change of Business Name

Change of Location

CHANGE IN OWNERSHIP 1) Sign In to the Licensing Portal:

https://dphregprograms.iowa.gov/PublicPortal/Iowa/IDPH/common/index.jsp

On the My Profile page your tattoo establishment will be listed on the left side under

Registered User’s Memberships. (Call 855-824-4357 if you don’t see your business.)

2) Click on the name of your tattoo establishment, then click Continue.

3) Click Online Services next to your active Tattoo Establishment License.

4) Click on the drop-down menu and choose “TATF Change in Ownership” then click “Confirm.”

5) Click “Continue” when the form is complete.

Click “OK” on the pop-up.

6) Complete the “Change of Owner” fields and then answer “Public Portal Affirmation.”

7) Click “Continue” when the form is complete.

Click “OK” on the pop-up.

8) On the payment screen the fees for the “Change of Ownership” request will appear.

9) To pay online, click “Pay Now” on this screen, and click “Pay Now” again on the next page to be

taken to the online payment system.

NOTE: Once payment is complete, and you have received your confirmation number and receipt, your

application for ownership change will be submitted to the Tattoo program staff for review. Processing

times vary. Contact the Tattoo program to check the status of your application: (515) 281-5894

CHANGE OF BUSINESS NAME

1) Sign In to the Licensing Portal:

On the My Profile page your tattoo establishment will be listed on the left side under

2) Click on the name of your tattoo establishment, then click Continue.

3) Click Online Services next to your active Tattoo Establishment License.

4) Click on the drop-down menu and choose “TATF Change of Business Name” then click

5) Click “Continue” when the form is

complete. Click “OK” on the pop-up.

6) Complete the “Change of Business Name” fields and then answer “Public Portal Affirmation.”

7) Click “Continue” when the

form is complete.

Click “OK” on the pop-up.

No fees are due for this request and you will be redirected to the “My Programs” page.

To verify that the request was successfully submitted, click on Details.

The “TATF Change of Business Name” will appear under the License/Process section.
