On 10 December 2009, history was made at Tygerberg Children’s

Hospital when one of the First Ladies of the country, Ms Nompumelelo Zuma, visited the hospital.

Dr Paul Ciapparelli, Director of Clinical Services, said this clearly showed that Ms Zuma has a passion for children. Dr Ciaparelli continued by saying that the first lady’s contact with the children was touching and he hopes that it’s not the last time she visits the hospital. Speaking during the visit, Ms Zuma said visiting the hospital came from her heart. She wanted to visit and support the children during the festive season. Ms Zuma was satisfied with conditions of the hospital and promised to come back.

COMMUNITY

SERVICE NURSINGThey came from Kimberley, Oudtshoorn, Beaufort West and Brakrivier to report for their community service nursing.

Joining the nursing team of an academic hospital is a scary and hair -raising challenge for a professional freshman.

On day one the bars were set and the first cautious steps were taken on a clinical rotation schedule. During the year, on the crash course in professional survival skills, there were several days of headaches, sore feet and anxiety. The “young ones”

soon became the “tough ones”. They have shown courage, character and perseverance through challenging times.

On 1 January 2010, 23 enthusiastic independently registered professional nurses joined the nursing team as winners who successfully overcame obstacles and setbacks during their community service year.

Welcome at Tygerberg Academic Hospital. May 2010 be the first year of a very prosperous professional career. We salute you!!

Measles is a highly contagious viral infection, spread by respiratory droplets, which can have high

morbidity and mortality in both children and adults. Very young children, as well as those who are malnourished or immuno-suppressed, are particularly at risk of complications. Measles has an incubation period of 10-14days.

The diagnosis of measles is made according to the case definition of measles as stated by the Extended Programme of Immunisation

(EPI) program. A clinical diagnosis is made if there is a history of fever AND a rash AND ONE of cough OR red eyes OR runny nose. Complications include pneumonia, diarrhoea, croup and encephalitis.

Most children do not require admission to hospital but some children with complicated measles may require intensive care. Treatment is mostly supportive with administration of two doses of vitamin A orally and management of complications which may include antibiotics for pneumonia and fluid therapy for diarrhoea.

Measles can be most effectively prevented by immunisation – the standard recommendation in South African-EPI is a vaccination at 9 months and booster at 18months which should afford 95% protection. However in an outbreak setting those at risk include: children less than 9 months who have not yet received their standard immunisation, children who have missed their immunisation, health care workers, older children and adults who may have reduced immunity or have not been immunised as children.

Within Tygerberg the response has included:Encouraging a booster vaccination for all health 1. workers.Vaccinating children > 6 months of age2. who are hospitalized and have not had at least 1 dose of measles vaccine. Administering booster vaccine or immunoglobulin 3.

to any child in contact with a case.Opening a 30 bedded measles ward in G10 for 4. children with measles requiring hospitalisation. This opened on Friday 26 February and has required additional medical and nursing staff as well as assistance from pharmacy and administration to keep the contagious cases separate from the rest of the hospital. All children from Metro East who attend our local clinics with complications of measles are sent here for evaluation. The ward is currently full with a high turnover.

Children or adults with symptoms of measles should ideally be managed at home to limit spread at clinics and in the community, but should they be unwell with inadequate fluid intake, significant diarrhoea or respiratory symptoms or any other concerns they should be seen at the local clinic or Community Health Centre where a decision will be made if they require referral for evaluation at Tygerberg.

The National Immunisation Campaign is scheduled for 12 April until 28 May 2010. This campaign will stretch across the province as follows:Round 1: 12-23 April 2010Polio (drops)will be given to children 0-59 monthsMeasles (injection)all children 9 months to under 15 yearsRound 2: 24-28 May 2010Polio (drops)will be given to children 0-59 monthsDeworming tablet (Albendazole)to children 12-59 monthsVitamin A (drops) to children 12-59 months

EVENTS

On 28 January 2010 the new therapy room in G7 , was officially opened. The opening was the culmination

of a one year project initiated by the Occupational Therapy Department. The final product is a dedicated and well- resourced in-ward therapy room. Financed by the Tygerberg Children’s Hospital Trust, the room has beautiful murals, designed and painted by Mrs Anne Wells, a local artist, and her team of volunteers. The initiation would not have come to fruition without the support of Dr Rabie and Dr Nel, as well as all the staff and therapists of G7. This room will provide an invaluable work space for use by Occupational Therapist, Physiotherapist, Speech Therapist and Social worker, improving the efficiency and quality of their treatment.
