Spondylosis is a common term used for neck pain radiating in one or both the hand and sciatica is a pain of low back

radiating to legs. The pain in both the condi-tions occurs due to herniated disc or disc pro-lapse which will irritate the nerve coming out from the spinal cord leading to pain.

What are the causes?Any conditions which leads to excessive

mechanical pressure on the disc will lead to uneven distribution of the pressure leading to herniation. Any excessive physical activity, trauma, disc degeneration, poor posture or in old age due to degeneration can cause the disc to herniate. Cervical spondylosis is very com-mon in young age people working on computers continuously and in those who use cell phones continuously with head bent down.

What are symptoms?Symptoms of herniated disc occurs when the

herniated part of the disc will compress the nerve root present there. The compressed nerve root will be inflamed due to constant irritation with the herniated disc. The patient will have pain in the neck, arms, hands and also in the shoulders in case of cervical spondylosis and in case of sciatica, pain will be in low back radiat-ing to one or both the legs. Sometimes the pain

may be present only in the neck and may not radiate. The pain will be burning or electric shock like and can be associated with tingling and numbness. When a large portion of disc have herniated out it may also cause weakness of the hands and legs in which the patient may feel very weak to hold any object or may find it difficult to walk and stand. Many times it is just an accidental finding and the person may not have any pain at all.

hoW can We diagnose it?The diagnosis of this condition is made on

the symptoms, clinical examination of the patient and MRI of spine. Classical pain which is burning or electric shock like, radiating to the arm or hands and legs, tingling numbness makes the diagnosis towards herniated disc. The diagnosis is confirmed by examining the patient and Undergoing a MRI of spine. Doing a MRI spine in cases of herniated disc is very important not only to confirm the diagnosis but

also to rule out other complications like any tumour or any major injury like involvement of spinal cord. X-RAYS are of no importance in diagnosing a herniated disc.

What are the complications of herniated disc?

Increase in pain, weakness of the hands and also the legs in late stages, involvement of bowel and bladder leading to loss of control of bladder and bowel function, that is unable to control urine and stool. If the herniated disc is big it will start compressing the spinal cord leading to weakness of hands and legs and may also lead to paralysis.

When should you see a doctor? Any burning pain or electric shock like pain,

or any tingling numbness in hands and legs, is a sign of pain involving nerves and it is compul-sory to visit the pain physician.

Complications like weakness of hands and legs, loss of urine and bowel control, pain asso-ciated with fever, pain increasing in intensity and not relived with medications, pain started after any major trauma. These complications

are called RED FLAGS and it becomes manda-tory to see the pain physician.

What are the treatment options available?

There are multiple treatment available both surgical and non surgical. The treatment depends on the symptoms of the patient.

Drugs- Patients with only pain can be treated with anti neuropathic drugs. These drugs act on the nerves which are inflamed and are causing pain. These drugs are to be taken under the supervision of the treating doctor.

Nerve root blocks – it is a Non-surgical treatment for patients who are already taking antineuropathic drugs with no pain relief. The nerves involved are identified and then the nerve root blocks are done in operation theatre using fluoroscopy machine. Patient will have

immediate pain relief following which patient is explained about the do’s and don’t’s. The patient is observed for two hours in the hospital and then discharged. Patient can resume their work the same day.

Surgical treatments- surgical treatment for disc prolapse is required when required when there are complications like severe pain not relieved with medications, weakness of limbs, unable to control urine and motion.
