W hy are we excited about your ACP member-ship? ACP continues to offer more and more perks for your membership. If you think ACP only pro-vides the Annals of Internal Medicine and MKSAP as perks, allow me to update you. ►What a Great Fall 2013 Scientific Session! Kudos to our 2013 program chair, Shouvik Chakrabarty MD, FACP for scheduling great scholarly presentations. If you came, you ex-perienced an excellent CME experience. If you missed this meeting, I am disappointed for you. We are working hard to provide you with this ACP perk.

►On the Advocacy front, our ACP team is con-tinuously working in Washington DC on repeal of the SGR. The Affordable Care Act still re-mains a mystery, but it will not likely go away. There will be a great deal of change and com-promise as the Affordable Care Act rolls out. Issues of enrollment, public confidence, and the promise of not losing you and your pa-tient’s existing coverage still loom as major problems for President Obama. We are still pressing Congress to eliminate se-questration and fund the CDC, NIH, and gradu

ate medical education to name a few. Where will our new medical school graduates go if we run out of post graduate spots. ►Smart Medicine has been released by ACP as competition for “Up To Date”. This newsletter includes a link to help you connect to this new perk. This is initially designed for the outpa-tient internist for a quick reference during a busy clinic. Our hope is that Smart Medicine will soon have complete, timely information and references for our Internal Medicine hos-pitalist.

►Internal Medicine 2014 in Orlando repre-sents the premiere CME for internist regard-less of what you do. There will be plenty to see and do. Dates for the meeting are April 10—12, 2014. We will again have a chance to honor our Okla-homa national award winners- John Kalbfleisch MD, MACP, Michael Weisz MD, MACP, and Loveland Award Winner The Cherokee Nation. Stay tuned for our reception time and place in Orlando.

Our annual meeting this year in Oklahoma City saw another great collection of research

and clinical vignette presentations, as well as a few thoughtful review pieces, submitted by

Associate residents and fellows from across the state. Here are a few of the highlights:

Dr. Parker presented a patient with shortness of breath and hypoxia, found to have a pul-monary arteriovenous malformation.

Dr. Farrukh presented a hypokalemic periodic paralysis patient who presented with a con-comitant renal tubular acidosis.

Dr. Nguyen presented a review of the frequency of drug-induced thrombocytopenia in pa-tients younger than 18 years of age, a group frequently ignored.

A case of pulmonary Langerhans cell histiocytosis presenting with generalized seizure was presented by Dr. Khawar who also presented “ A case of cutaneous polyarteritis nodosa”.

Several authors were involved in presenting a case of prostate adenocarcinoma with thy-roid metastases, of which only six have been reported in the literature to date.

An exceptionally rare case of Fahr’s disease was presented by Dr. Sprawls, as was an equally rare case of Scimitar syndrome, by Dr. Iftikhar.

Candida endocarditis was presented by Dr. Hamilton, as was a case of neuromyelitis optica uniquely associated with Sjögren’s syndrome by Dr. George.

Dr. Jaiswal presented a novel approach to treating refractory venom-induced thrombocy-topenia, a case of autoimmune NMDA psychosis, and an oral presentation surveying resi-dents’ knowledge of USPTF hepatitis C screening guidelines.

Reviews of common diseases and therapeutics were present, including an oral presenta-tion reviewing trends of the use of anti-thrombotic agents and outcomes in NSTEMI by Dr. Wayangankar,

a poster reviewing fecal microbiota transplantation for Clostridium difficile infection by Dr. Austin,

AAcase of encephalopathy and profound hyponatremia after administration of Borte-zomib by Drs. Bhutta and Stuber,

An oral presentation reviewing our current understanding of inflammatory contributions to sleep disorders by Dr. Choe.

A retrospective analysis of trends in management of prediabetes was presented by Dr. Gupta.

A quite different but much appreciated oral presentation was given by Dr. Mahmood re-garding work/life balance during residency.

All four oral presentors received commendation. Winners of the poster presentations included:

First place, research: Dr. Choe and his aforementioned inflammatory sleep review; First place, clinical: Dr. White with a cryotococcal bone marrow infection case, Second place, clinical: tie between Dr. George for his presentation of NMO and Dr.

Jaiswal for her venom-induced thrombocytopenia case. Overall, quite a successful competition was had during the Associates’ presentations, highlighting yet again the importance of scholarly work in the armamentarium of the well-rounded physician.

to approve the bipartisan, bicameral SGR repeal and Beneficiary Access Improve-ment Act of 2013 which is a critical step to-ward enactment of legislation to permanently repeal the SGR and ensure beneficiary access to care. We believe that this bill achieves sev-eral critically important objectives:

1. It repeals the Medicare SGR, once and for all, and all future SGR-related sched-uled cuts. 2. It consolidates that separate Medicare reporting programs into one Value-based Payment (VPB) program, cancels the exist-ing penalties scheduled for 2016 under the current reporting payers. 3. It creaprograms, and requires a GAO study for harmonizing measures used by Medicare and private tes multiple path-ways—including medical homes and regis-tries--for physicians to qualify for higher annual payments by successfully partici-pating in the new VBP program or in an al-ternative payment (APM) model. 4. It expressly creates positive incentives for Primary Care Patient-Centered Medical Homes and Patient-Centered Medical Home Specialty practices, under both the VBP program and as an APM. 5. It provides financial support to smaller practices in all geographic areas to help them be successful under the VBP and APM programs. 6. It doubles federal funding for perform-ance measures several years of transition-ing to APMs and VBPs is necessary, which is a feature of the bipartisan Energy and Commerce committee bill that was unani-mously reported out in July. We believe that an affirmative vote now for the SGR

Repeal and Medicare Beneficiary Access Improvement Act of 2013 is a critically im-portant step to achieving repeal of the SGR, ensuring beneficiary access to care, and making other needed reforms in Medi-care physician payments.

Senate And House To Take Up Per-manent SGR Repeal. The AP (12/11, Alonso-Zaldivar) reports that both Democrats and Republicans now favor a permanent change to “Medicare’s broken physician payment policy,” known as the “sustainable growth rate” (SGR) and are working towards a new system that would contain “financial carrots and sticks for doctors to provide quality, cost-effective care” for Medicare patients, with “up to 10 percent of an individual physi-cian’s pay” connected to “quality indica-tors.” This week, the Senate Finance Com-mittee is expected to take up “a broad bi-partisan framework” on the matter which will also be taken up by the House Ways and Means Committee. Yet, Congress will still have to adopt a “temporary patch” for early 2014, as no one expects agreement on a plan before the beginning of the year. The biggest obstacle to that is the need to find “offsets” to the cost of the temporary fix. As to the permanent fix, the AMA is said to be “reserving judgment.”

….Brief facts for you

Are You Performing Tests that Require a License?

If you are performing tests such as dipstick uri-nalysis, fecal occult blood, pregnancy and ovu-lation tests, and many other common tests without a waiver, you may be violating CLIA laws.

Check the FDA website to make sure your tests are being performed in conjunction with CLIA regulations and find out how to obtain a CLIA certificate if you are in violation.

Already Have a Waiver but Want to Ensure the Accuracy of Your Tests? Over 3,000 labs around the world depend on MLE's proficiency testing program for user-friendly individual and bundle testing pack-ages. Manage risk, maintain the highest stan-dards of quality and enhance patient safety by including MLE's proficiency testing as part of your ongoing quality assurance checks.
