P03: Directional Drilling & Wellbore Surveying

INSTRUCTIONS TO FILL UP AND SUBMIT REGISTRATON FORM: 1. General: Read the Brochure and Registration Form carefully. Fill up the Registration Form as per guidance

given in the registration form. Ink signature is not required in the registration form. Please choose your batch

as per your convenience. Further select Program as per your choice. Trainees are required to fill up all 10 yel-

low cells to complete Registration Form. Particulars in yellow boxes have been filled up for sample only.

Please replace them by your own particulars. Please do not change font/ font size/ other text/ data in the

Registration form. Please do not deform properties of the excel form. Please do not convert it in PDF. It will

remain in Excel only. Registration Form requires no signature and photograph. Column of particulars starts

from 1 and ends at 10. All entries shall be made in CAPITAL LETTERS except in column number 3 and 4.

2. One Registration Form for One program: Candidates can apply for one or all programs in a batch simulta-

neously. Each program/ course/internship topic shall be applied in separate Registration Forms (Use one reg-

istration form for one program only).

3. How much and how to pay Registration cum Internship Fee: -₹ SIXTEEN THOUSAND NINE HUNDRED only (₹16900.00) per program per trainee if applied individually.

Concessional fee: Program fee (INR): 14900.00 per trainee/ program, if applied in group of 4 or more trainees.

Presently GST is not applicable on any program, will be intimated when applicable.

-Please mention group leader's name in column number 10, if applied in a group. Registration cum Internship

Fee does not include boarding/ lodging charges.

-Remit fee amount by net banking (NEFT/IMPS/ Paytm/ google pay) / bank deposit to “Drilling Mentor Pvt.

Ltd.” vide A/c No.: 006705005419, Account Type: Current Account, Bank Name: ICICI Bank, Ahmedabad, IFSC:

ICIC0000067 in view of the refund rule as mentioned in the brochure. Bank Charges, if any, shall be borne by

applicant.

4. How to submit Registration Form: Please keep three (03) attachments ready-

(a) Completely filled up Registration form

(b) Scanned/snap shot copy of proof of payment i.e., Receipt from net banking (NEFT/IMPS/ Paytm/ google

pay)/ bank deposit to “Drilling Mentor Pvt. Ltd.

- Confirmation of Seat By Drilling Mentor: Drilling Mentor will confirm your seat on First Come First Serve ba-

sis within a week subject to correctness of the submitted application.

APPLICATION PROCEDURE

Incomplete application will stand rejected and no further action shall be taken by Drilling Mentor.

The candidate shall have to enquire about the delay if confirmation is not received within a week

from the date of submission.

Registration/ Program Fee for the program: INR 16900.00 (Rs. Sixteen Thousand Nine Hundred

Only) per program per trainee if applied as an individual.

Concessional Fee for the program: INR 14900.00 (Rs. Fourteen Thousand Nine Hundred Only) per

program per trainee if applied in a group of 4 or more trainees.

Mode of payment: Program Fee to be transferred through internet banking (NEFT/ IMPS/Google

Pay/ Paytm) or Bank Deposit to the account of “Drilling Mentor Pvt. Ltd.” vide Account Type: Cur-

Please note that instructors, topics or period are confirmed at the time of program, however,

circumstances beyond the control of the organizers may necessitate substitutions, alterations or

cancellations of the speakers and/topics or period. As such, Drilling Mentor Private Limited

reserves the right to alter or modify the advertised instructor, topics or period if felt necessary.

Drilling Mentor reserves the right to videotape, film, photograph, and/or record the course sessions

and course participants in any media type and to alter or edit these images for use in its publica-

tions, including website entries without giving any payment to participants or without any permis-

sion of the participants, Drilling Mentor may use participant contact information (email, telephone,

or fax) for the following limited purposes: to provide updated class information and other infor-

mation related to professional development in the petroleum industry, to request help in evalu-

ating Drilling Mentor courses and materials, and to provide information concerning future course

offerings.

The use of any recording device (audio or video) by participants during a Drilling Mentor program is

strictly prohibited. The unauthorized use of a recording device during a Drilling Mentor course/

presentation shall be grounds to remove the participant and confiscate or destroy the related

recording. No portion of any Drilling Mentor's course may be recorded digitally, on film, video tape,

audio tape or other recording device, or be reproduced photographically or by any sight or sound

device without the explicit written permission of Drilling Mentor. All Drilling Mentor course

presentations are the sole property of Drilling Mentor. We are the exclusive owner of the copyright

of all course materials.

Any dispute arisen during the pendency of the Program/Course/ Internship shall be solved in the

court of law within the jurisdiction of Ahmedabad District of Gujarat State.

TERMS OF PAYMENTS

INCOMPLETE/ REJECTED APPLICATIONS

PROGRAM CHANGE POLICY & STSTUTORY NORMS

Drilling Mentor reserves the right to cancel a course if less than eight (8) participants have signed

up, one (01) week prior to the date of commencement of the course or can merge such student in

other contemporary program on mutual understanding.

Hundred percent (100%) course fee will be refunded within one (1) weeks from the date of com-

mencement of course, if course cancelled by Drilling Mentor.

Cancellation by buyer will be charged one hundred percent (100%) of the fee.

No refund is possible once the seat is confirmed.

Drilling Mentor Private Limited is not responsible for any loss or damage as a result of a

substitution, alteration or cancellation/postponement of an event.

Drilling Mentor Private Limited shall assume no liability whatsoever in the event this training course

is cancelled, rescheduled or postponed due to a fortuitous event, Act of God, unforeseen occurrence

or any other event that renders performance of this training course impracticable or impossible. For

purposes of this clause, a fortuitous event shall include, but not be limited to: war, fire, strike, Bund,

extreme weather or another emergency.

Participants will have to follow the instruction given in confirmation letter/ brochure/ Registration

Form, failing which candidature will be cancelled.

JOINING THE PROGRAM

COURSE CANCELLATION: LIABILITY, RESPONSIBILITY & REFUND POLICY

Course activities will run during working days as per government calendar.

The Course syllabus shall be covered as per time plan.

The Course can be completed in shorter duration if participants accept the time management. No

shortcut shall be accepted by Drilling Mentor to cut short the Course.

Daily time of joining & dismissal of session shall be watched from the clock of the Centre. Only an

allowance of ±5 minute shall be considered for joining the session.

Registration fee shall be non-refundable once the seat is confirmed.

Seats are very limited. Please contact the Drilling Mentor Team at mobile number +91 6351 728

908 before making payment for program or booking return ticket for the purpose of the program.

Participants during the period of training, will have to maintain optimum discipline at the venue of

training as per rule applicable for educational institutions and public place failing which his/her

candidature shall be cancelled and will be expelled from the running session without any

compensation and can be liable to face consequences of local law and order.

Boarding/lodging: Drilling Mentor does not operate any Paying Guest (PG)/ Guest House/Hostel or

other accommodation; however, it can arrange economical PG on request. Interns willing to hire PG

can search Hotel/ PG in Motera/ Chandkheda on JUST DIAL.

Separate Boys’ & Girls PG accommodations are available at nearby locations at reasonable rates

subject to booking them in advance.

HSE, Security & Drug Abuse Undertaking is included in the registration form. As per following wordings:

““Food, lodging, any kind of insurance/ expense or compensation with respect to the course/internship,

life, health, safety, security, environment, violation of smoking & drug abuse and law & order of the place

of the internship are all at own risk and responsibility of the applicant/trainee and for the same Drilling

Mentor Pvt. Ltd. shall not be held responsible. I will maintain optimum discipline at the venue of training

as per rule applicable for educational institutions and public place failing which his/her candidature shall

be cancelled and will be expelled from the running session without any compensation and can be liable to

face consequences of local law and order to recover any loss caused by me. Based on above, may please
