SPURS SPORTS & ENTERTAINMENT AUTHORIZATION FORM (Must read and sign for child to participate)

As a condition of child’s participation in the 2011-2012 San Antonio Spurs Jr. Silver Dancers Program (the “Event”), I hereby agree to release, forever discharge, and hold harmless the San Antonio Spurs, 5-6-7-8 DANZ STUDIO, the venue hosts for the Event, the NBA, the AT&T Center, and each of their respective parent or affiliated companies, officers, directors, managers, employees, agents, representatives, successors and assigns, from and against any and all claims, costs, damages, demands, expenses (including attorney’s fees), or liabilities (collectively, “Claims”) arising out of or related in any way to my participation in the above-referenced competition including, without limitation, any Claims for personal injury, illness, death, dismemberment, aggravation to pre-existing conditions, damages to personal property, lost wages, or any other claims. I hereby authorize the Event to act for me accordingly in the best judgment in any emergency requiring medical attention.

Participation constitutes permission to use the voice, name, and likeness of child’s participation in the Event for advertising or other purposes without any compensation or payment to said participants.

In exchange for the opportunity to participate in the 2011-2012 San Antonio Spurs Jr. Silver Dancers Program to appear in a Team production, and without any further consideration, I, on behalf of myself or on behalf of my minor child (the “Participant” listed below) hereby grant to Spurs Sports & Entertainment (the “Team”), 5678 DANZ STUDIO, and NBA Entertainment, Inc. and their respective affiliates, licensees, employees and agents, exclusively, and in perpetuity, a non-revocable right in and the complete and unencumbered right, throughout the world, to exhibit, record, reproduce, broadcast, transmit, publish, sell, distribute, perform, and use for any purpose, in any manner, by any means and in any medium, whether now known or hereafter developed, Participant’s image, voice, and/or attributes, including, without limitation, the perpetual and unlimited right to reproduce (by electrical transcription, tape or other recording process whether now known or hereafter developed) any and all of Participant’s images, voice, and/or attributes. Participant shall not have or claim to have any right, title, or interest in or to any material embodied in any program or other material produced as a result of the consent granted herein.

I have full authority to execute this general release and do so with full knowledge of the facts and circumstances surrounding my performance. I have also read and agree to the terms of the refund policy. I understand that there are no refunds for any reason after February 3, 2012. NO EXCEPTIONS.

A GREAT EXPERIENCEThe San Antonio Spurs invite all girls (ages 5-18) to experience the fun and excitement of becoming a Jr. Silver Dancer. No previous dance experience necessary! The Spurs Jr. Silver Dancers Program provides a vibrant and energetic atmosphere with an upbeat spirit. As a Jr. Silver Dancer, your daughter will develop her dance skills from professionals such as the NBA’s top dance choreographer and members of the San Antonio Spurs Silver Dancers.

Time: Ages 5-9 7:30 AM - 8:00 AM (Check-in) 8:00 AM - 11:30 AM (Clinic) Ages 10-18 12:00 PM - 12:30 PM (Check-in) 12:30 PM - 4:00 PM (Clinic)

PERFORMANCE INFORMATIONPerformance will be assigned based on age group. Everyone (parents and Jr. Silver Dancers) must present a valid Spurs game ticket on day of performance to enter the AT&T Center.

Performance #1: Pre-Game March 2, 2012 (AGES 5-9) Spurs vs. Charlotte Bobcats

Performance #2: Halftime March 2, 2012 (AGES 10-18) Spurs vs. Charlotte Bobcats

Attire: Jr. Silver Dancers t-shirt, performance skort, white socks, and white tennis shoes.

TICKET INFORMATIONEach Jr. Silver Dancer will receive one (1) Spurs game ticket credit valued at $23.00 with an option to upgrade to better seat locations. To upgrade your ticket, use your $23.00 game ticket credit towards the purchase of the upgrade.

APPLICATION PROCESS AND REFUND POLICY

Complete form to the right and return along with your registration tuition. All applications must be received by Feb. 3rd. All money, less a $20 non-refundable administrative fee, will be returned for all cancellations prior to Feb. 3rd. There will be no refunds after Feb. 3rd (including clinic fee and purchased tickets). NO EXCEPTIONS. If paying by check, your cancelled check will be your receipt. Please note that there will be a $30 charge for all returned checks. Please note, Spurs performance game tickets are not guaranteed. Please order all Spurs game tickets upon registration. This includes the participating Jr. Silver Dancer and additional tickets for family/friends. Game tickets are not guaranteed to be seated together.
