Which Serta products are included in the event?

Consumers qualify to receive a $100 Grocery Gift Card by making a purchase of $650 or more of any combination of the following Serta branded products:

iComfort , iSeries, Bellagio

iSeries, Trump iSeries and Perfect Sleeper mattresses and foundations

iComfort Directions and

iSeries Profiles motion adjustable bed bases

Serta brand pillows, sheets or mattress protectors

Customers can use the $100 Grocery Gift Card to help set the table for holiday entertaining.

Gift Card choices include the Honeybaked Ham store, Omaha Steaks and major grocery store chains.

Note: Limit one gift card per household.

What has to happen for the customer to get the gift card?

Here are the steps(continued):

Customer makes purchase. Store provides Customer a flyer explaining how they will receive their gift card. (see flyer, right). Stores have been sent flyers to be given to customers at time of sale. If, for any reason, flyers are not available, it is possible to go to the Current News page at www.fmgbuyinggroup.com/currentnews.aspx to download and print a copy of the flyer which can be given to the customer. (No password is required to access this page.)

What has to happen for the customer to get the $100 gift card of their choice?

Here are the steps(continued):

2. Record of qualifying sale.At the conclusion of the event, each participating retailer must provide to FMG a listing of all Consumers that have made a qualifying purchase during the event.

This simple spreadsheet requires a modest amount of information--name, address, phone, retailer, branch, date and items purchased.

At the end of the event a copy of this worksheet showing the voucher numbers that have been assigned is to be sent to FMG. FMG will provide Capitol Marketing the list of customers to be sent gift card vouchers.

3. Purchased items are delivered to the Customer. Store notifies FMG using the Dealer Set the Table Worksheet (see prior page) that the Customer should be sent a voucher. FMG notifies Capitol Marketing that the voucher is to be released to the Customer.

3. The fulfilment processor will mail the requested gift card directly to the customer.

FMG and Retailer are notified by update on the Serta Set the Table Worksheet that the sheet set voucher has been processed.

How is the event being supported?

Stores may order vinyl 24” x 62” vinyl banners (no charge), Flyers will be provided to stores at no charge.

POP and other layouts are being produced for members to use. FMG will not be printing these materials.

Additional informationCustomer will receive gift card by mail-in or on-line rebate. Customers will receive a voucher for a grocery gift card upon completion of the sale. The voucher is to be completed by the consumer by mail or on-line. Customer will choose from among a range of gift card choices at that time. Offer valid for purchases made between October 2, 2014 and October 14, 2014 only. Only one gift card may be selected per voucher. Customers purchasing multiple qualifying sets may receive multiple gift cards. Following receipt of the properly completed voucher, the processor will mail gift card directly to the consumer in 4 to 6 weeks. In case of a return or cancellation, the value of the gift card ($100) will be deducted from any refund. Gift cards are completely transferable, but may not be resold under any circumstances. Voucher must submitted (postmarked or entered on-line by customer) on or before December 31, 2014. Stores must notify FMG that a customer is to be issued a gift card not later than Monday, December 15, 2014.
