WORD CHECK

Nursing is one of the several health care professions that mainly deals with caring for patients and working with doctors and other medical staff. However, a nurse’s job doesn’t start nor end with taking care of a patient. From the time a patient seeks medical advice on the onset of symptoms to the time he or she receives diagnosis and prescriptions, nurses are there to assist. When patients are advised to undergo laboratory tests such as urinalysis, fecalysis, or blood tests, nurses still communicate with the doctor, the patient and other healthcare professionals. When a person needs in-patient services, the nursing staff takes care of him and his needs. Simply put, nurses play a big role in helping patients deal with matters concerning their health.

DIALOGUE

Dr. Kim:

Mr. Smith:

Dr. Kim:

Good morning, Mr. Smith. Aside from stomach pain, do you feel other symptoms like nausea, headache, or frequent visits to the bathroom?

No, I don’t have headaches at all. I started to feel stomach pain after I ate some seafood. But it disappears after several minutes. For the last 3 days, I think it occurred seven times already.

Well then, Mr. Smith, I want you to undergo a series of tests for me to give an accurate diagnosis about your illness. I will give you a laboratory request and the nurse will accompany you to the laboratory department of our hospital. For the meantime, I will give a prescription to help you alleviate the stomach pain. Let Nurse Anna take you to the laboratory department. She will assist you until you finish all the necessary tests.

WORD CHECK

Physical examination is the process of evaluating objective anatomic findings through the use of observation and a series of other steps. It is a procedure done by nurses and doctors. The information obtained must be thoughtfully integrated with the patient’s history and pathophysiology. Moreover, it is a unique situation in which both patient and physician understand that the interaction is intended to be diagnostic and therapeutic. Generally, it follows the following procedure:

1. visual inspection 2. palpation 3. percussing 4. ausculation

The physical examination, thoughtfully performed, should yield 20% of the data necessary for patient diagnosis and management.

Like other parts of the physicalexam, it begins with inspection, and then proceeds to palpation. It requiresthe use of several special instruments in order to inspect the eyes and specialtechniques to assess their special sensory function. The nose is examined forany obstruction in the airway and any problem with a person’s sense of smell. The throat is examined by looking at the oral cavity and checking for any sores, bleeding gums or lesions.

Mr. Smith:

Dr. Kim:

Let’s now start with your neck and throat assessment. We need tocheck whether there are lumps or nodes present in your neck. Did you feel any pain in your throat and neck these past few days?

No, I didn’t, except that my throat felt very itchy when I had allergies.

Alright. Well, the movements are smooth and there is a good control of range of motion movements.

What are the deviations from normal?

Asymmetrical head position, masses, scars and swelling are indications of cancer, or enlarged thyroid or inflamed lymph nodes.

Rigidity, stiffness, and pain are manifestations of muscle spasms, inflammation, meningitis, and cervical arthritis.

Do you have recent changes in your vision like spots, halos, and rings?

No, I don’t, but I’m wearing contact lenses.

Okay. Let’s test your visual acuity. Normal findings show a 20/20 vision. Now, please sit still as I check your nose. Now, let’s proceed to your chest and lung assessment. I will palpate using my fingertips and palmar surfaces in an organized manner. There should be no vibrations or pulsations in your aortic, pulmonic, and tricuspid area. That’s it. Everything looks normal.

Whew, I’m so glad. Thank you, Dr. Kim.

WORD CHECK

Both the examination of the thorax and the abdomen follow a certain procedure. When the physician examines the chest area, palpation, confirmed by percussion, assesses for tenderness and degree of chest expansion. Auscultation, a more sensitive process, confirms earlier findings and may help to identify specific pathologic processes not previously recognized. On the other hand, during an abdominal examination, auscultation of the abdomen is performed for detection of altered bowel sounds, rubs, or vascular bruits. Palpation is the examination for crepitus of the abdominal wall, for any abdominal tenderness, or for abdominal masses. Next, inspection consists of visual examination of the abdomen with note made of the shape of the abdomen, skin abnormalities, abdominal masses, and the movement of the abdominal wall with respiration.

Each person on your hospital team has a unique role. The main members of your team will usually include:

Doctors. One doctor may lead the hospital team. Often this is a doctor who specializes in hospital care, called a hospitalist. If you’ve had surgery, your surgeon will lead your team.

Nurses. You’ll have a team of nurses taking care of you around the clock. They keep your care coordinated among the different hospital team members. They also keep you informed of your daily care plan.

Pharmacists. Working with your doctors and nurses, the pharmacist pays careful attention to your medication needs, as well as providing you the prescriptions you’ll need when you leave the hospital.

Care managers. The care manager arranges for smooth transitions, including any equipment or at-home services you may need after you leave the hospital.

You and your family members. As the patient, you are the most important member of your team. Be sure to ask questions and share your concerns about any part of your care.

Perioperative nurses help plan, carry out, and assess treatment for patients undergoing surgery. Working in hospital surgical departments, ambulatory surgery units, clinics, or physicians’ offices, these RNs are involved in the care of patients before, during, and after surgery.

Types of perioperative nurses include the following:

A scrub nurse is sterile and is responsible for choosing and handling instruments and supplies used during surgery.

A circulating nurse includes developing a patient plan of care, reviewing preoperative assessments with patients, and managing activities in the operating room.

The RN first assistant (RNFA) assists during the operation by helping with surgical activities such as keeping bleeding under control, exposing wounds, and suturing, all under the direction of the surgeon.

A PACU (Post Anesthesia Care Unit) RN cares for patients immediately after surgical procedures and anesthesia.

WORD CHECK

When you leave a hospital after treatment, you go through a process called hospital discharge. A hospital will discharge you when you no longer need to receive inpatient care and can go home. Or, a hospital will discharge you to send you to another type of facility.

Many hospitals have a discharge planner. This person helps coordinate the information and care you’ll need after you leave. You’ll need to understand your injury or illness. You’ll need to know the next steps to take. This may include taking medicine and caring for a bandage. Make sure you know who to contact if you have a question or a problem. The discharge planner and your healthcare provider will answer your questions.

After you leave the hospital, you will need to make sure to take care of yourself as instructed. This will help ensure you don’t need to go back into the hospital.
