Major Achievements in 2013

The Board of Directorsis very excited as weenter the New Year.Alzheimer’s CommunityCare has achieved accreditation by theJoint Commission as ac o m m u n i t y - b a s e dorganization and this isa critical piece in fulfilling our vision. ThisBoard believes that the

organization was created to make a difference within the lives of our families asthey cope with the effects of Neurocognitive Disorders throughout the duration of the disease.

When Alzheimer’s Community Care wasfounded in October 1996, accreditation wasnot envisioned as being possible. However,in today’s world it became evident that if wewere to make a serious impact on the transformation of care that would be genuinely respected and acknowledged,this step was necessary.

This Board is well aware that the accreditationprocess – achieving the “BehavioralHealth” designation – will lead to the nextstep of achieving disease-specific certification,the most challenging step. This undertakingis very ambitious but the Board of Directorsis committed to achieving this standard as abenchmark to be replicated within the healthcare system.

Another major achievement of this past calendar year was the renovation of the oneand only building the organization ownswhich is located in Pahokee. This would nothave happened if it were not for the generosity of the Leo and Anne Albert Charitable Trust under the guidance of itsvery caring trustee, Mr. Gene Pranzo. Thededication ceremony naming the buildingthe Leo and Anne Albert Alzheimer’s Careand Service Center occurred in October2013.

Another major achievement was the dedication ceremony of our new facility onthe campus of Advent Lutheran Church in

Boca Raton on November 14th. The buildingis now named the Phyllis and Julius SiegelAlzheimer’s Care and Service Center. RonSiegel, the son of Phyllis and Julius andlong-time friend and supporter of the organ-ization, is responsible for this significant,caring, and generous gift in memory of hisparents. Ron’s vision is that this center isknown as the provider of choice for administering specialized care throughoutthe duration of the disease. Ceremonies inboth locations were heartwarming and trulyreflective of our community roots. There ismore information on these two significantevents within the pages of this quarterlymagazine.

Public Awareness Projects

The focus of this publication is our publicawareness projects. Asan organization, we advanced our agendaby asking our counties’leadership to presentproclamations fromPalm Beach, Martin,and St. Lucie Counties

acknowledging the presence of our localfamilies who are coping with the effects ofNeurocognitive Disorders. This exercise hasbeen executed for the past 17 years, but forthe past three years we have asked theAlzheimer’s Association Southeast FloridaChapter to join us in the public awarenesscampaign for demonstrating the necessityof specialized care and research.

This year we asked four specific local governments to issue proclamations acknowledging our work and mission.Alzheimer’s Community Care continues toexpand our resources within those communities in order to accommodate morefamilies. The municipalities who were sogracious in issuing proclamations were;

• Palm Beach County• Martin County• St. Lucie County• The City of Boca Raton• The City of Pahokee• The City of Belle Glade• The City of South Bay

The proclamations are on the cover of thismagazine and their wording is highlightedwithin the pages of this magazine.

We are in the process of having all nine ofour Specialized Alzheimer’s Care and Service Centers licensed under the Specialized Alzheimer’s Day Care Act,which was passed by the legislature in2011. At this time we have seven Care andService Centers licensed with two more togo. The remaining Centers will be licensedwithin the first six months of 2014. The Centers that are licensed under the Specialized Alzheimer’s Day Care Act are:

• Leo and Anne Albert Alzheimer’s Care and Service Center

• Phyllis and Julius Siegel Alzheimer’sCare and Service Center

• Ft. Pierce• West Lake Worth• West Palm Beach• Lake Worth• Delray Beach

Why is this license so important? It recognizes the following mandatory standards for administering care and services according to the law, and they are:

1. A nurse is on duty during 75% of theCenter’s hours of operation.

2. All staff must be trained on Neurocogntivie Disorders, and training is updated annually.

3. Must assist caregivers and familieswhen it is necessary for discharge planning.

4. A day care center that advertises Alzheimer’s disease and related disorders care must be licensed. This standard will be fully enforced by January 2015.

At this time there are approximately 20 other centers licensed under the SpecializedAlzheimer’s Day Care Act, and the list isgrowing every day. There are health systems excluded from the law so pleaseask if they are licensed.

As an organization we have been instrumental in achieving the following legislation on behalf of our Alzheimer’s families:

• Alzheimer’s Medicaid Waiver (2005 - 2010)

• Florida Silver Alert Act (2010)• Specialized Alzheimer’s Day

Care Act (2011)

The legislative initiatives are having a positive impact on the care and well-beingof our patients and caregivers. In addition, Ihad the honor of serving on the Florida Purple Ribbon Task Force, which was responsible for creating a three-to-five yearAlzheimer’s plan for the state of Florida.The final plan is listed on the Florida Department of Elder Affairs website.

Former-President of the Florida SenateMike Haridopolos appointed me to theFlorida Purple Ribbon Task Force becauseof my experience with the Baker Act. Representative Matt Hudson, using our recommendations, is creating a legislativeappropriations request to fund a FloridaAlzheimer’s Plan.

Alzheimer’s Community Care wants the following areas funded through this appropriations request:

• More services funded for families copingwith Alzheimer’s diseases and related disorders.• Fund the technology to find someonewithin minutes, as well as more training forstate law enforcement agencies that addresses persons who become lost onfoot. • Disaster preparedness for local communityorganizations like Emergency OperationsCenters and the Health Department so theycan be prepared to assist families who areat-risk because of the disease process. • An alternative to the Baker Act that provides stabilization services within thecommunities. Initially, funds would be usedfor a pilot program that gathers data andmeasures its effectiveness. • Community emergency intervention program that diminishes the need for institutionalization when a caregiver becomes incapacitated.

There are other viable recommendationsthat are stated within the Purple RibbonTask Force’s Report, and perhaps they willbe incorporated within Representative Hudson’s appropriations request.

Our Florida Legislators are very supportiveof the organization’s work and mission, andthat is due to our annual advocacy trip. The2014 advocacy trip will be from February 11-13. The Affordable Care Act was signed into law in March, 2010.We are all aware of the many debates about this comprehensivelaw, however, there are many provisions that provide new andinnovative benefits for our patients and families. The overallgoals of the Act include:

1. Expanding access to health insurance for all Americans2. Increasing protections for consumers3. Emphasizing wellness and preventive care4. Improving quality of health care5. Increasing the health care work force6. Curbing the rise in health care costs

Let’s look at each of these goals and how the Affordable Care Act addresses them.

1. Expanding access to health insurance• Expands Medicaid (in states that agree to

expansion) to cover those with incomes below 133% of federal poverty guidelines

• Requires employers (with 50 or more employees) to cover workers, or pay penalties

• Creates state-based insurance exchanges to help individuals and small businesses purchase insurance

• Eliminates the “pre-existing condition” barrier to purchasing affordable insurance

2. Increasing protections for consumers• Eliminates lifetime caps on insurance coverage• Prohibits insurance plans from cancelling coverage• Establishes state-based reviews for unreasonable

insurance premium increases• Links Medicare and Medicaid payments to medical

providers to quality of care• Creates a national process for developing health

care quality initiatives3. Emphasizing wellness and preventive care

• Establishes a national prevention and health promotion strategy

• Increases access to preventive care• Provides for Medicare coverage of annual wellness

visits with NO deductible or copayments• Awards grants for agencies promoting community

health• Provides funding to examine best

prevention practices

designed to improve efficiency of health care and health care outcomes

5. Increasing the health care work force• Provides for grants to strengthen nursing education

and improve retention of nurses and nursing assistants

• Encourages innovation in training, recruitment and retention of health care workers

• Establishes a national commission to review the health care workforce and project workforce needs

• Establishes grants to enable states to engage in comprehensive workforce planning and to develop new strategies

• Expands training programs for family medicine, general internal medicine, long term care, geriatric care

6. Curbing the rise in health care costs• Increases oversight of health insurance premiums• Fosters competition between insurance carriers• Provides for new payment structures for Medicare

and Medicaid (as above, tied to outcomes)• Emphasizes preventive care• Reduces health care fraud and abuse

There are extensive additional provisions in the AffordableCare Act and its emphasis on prevention and qualitypaves the way for cost-effective care.

“I’m a retired teacher so sometimes I like to go backand substitute, so that I can mingle with the peoplethat I’m use to mingling with for the past 40 years.On those days I’d always wonder, ‘Where’s mother,how is she doing?’ ”

Mrs. Williams worried because her mother oftenwalked down the alleyway by their house, leavingMrs. Williams to try to communicate to her that people were not the same anymore. The decisionto bring her mother to the Leo and Anne AlbertAlzheimer’s Care and Service Center has left themboth smiling.

“When my mother leaves this place she leaves witha smile, she kisses everybody here,” said Mrs. Williams.

Alzheimer’s Community Care started its community-based, dementia specific outreach inthe Glades Community in 1993. In 2004, the Pahokee Women’s Club donated their property toAlzheimer’s Community Care which now serves asthe Leo and Anne Albert Alzheimer’s Care andService Center, providing therapeutic Day Care forpatients and respite for family caregivers. In 2011,Gene Pranzo visited the Pahokee facility with thegoal of making the care center a pillar of the community. Mr. Pranzo worked tirelessly to ensurethat the architectural changes were done to Leoand Anne Albert’s standards. The Leo and Anne Albert Alzheimer’s Care and Service Center illustrates the love Leo and Anne Albert had for theGlades Community.

Mr. Pranzo’s efforts cumulated in a celebration thatwas held on October 17, 2013. The ribbon cuttingbrought together distinguished guests includingMrs. Williams and her mother, former President ofthe Florida Senate Ken Pruitt, Senator JosephAbruzzo, Pahokee Mayor Colin Walkes, ReverendJohn Mericantante, Paul Razza, Upendo-Shabazz-Phillips and Pastor Patti Aupperlee.

The ceremony was filled with emotional speechesdescribing the impact of Alzheimer’s disease onthe Glades Community. Providing an environmentwhere people like Mrs. Williams’ mother can remainin the community with safety and dignity is the reason we dedicate 90 cents of every dollar donated to our care centers. As a 501 (c) 3 non-profit we rely on the goodwill of trustees, donorsand community leaders.

“Alzheimer’s Community Care has made a permanent investment in the Glades communitywith this building, and that’s something that cannotbe minimized,” said Senator Abruzzo.

Phyllis and Julius Siegel Dedication Ceremony 2013 was a wonderful year as we not only held aceremony in Pahokee, but also in Boca Raton,Florida.

On June 27, 2013 the Alzheimer’s CommunityCare Board of Directors voted to accept a$250,000 donation in memory of Phyllis and JuliusSiegel to rename the newly renovated Boca Ratoncare and service center in their honor. Alzheimer’sCommunity Care’s relationship with AdventLutheran dates back to 1987, and this longstandingpartnership gave the organization the opportunityto renovate the building owned by the church. Thedonation was made by their son, Ronald M. Siegel,a social worker and former employee ofAlzheimer’s Community Care. The renovations resulted in a beautiful home-like care center forAlzheimer’s and Neurocognitive Disorder patients.

“We are so fortunate to have Ron Siegel. Here is aselfless man who is committed to helping others,and who volunteers to lead a men’s only supportgroup for Alzheimer’s patients, a safety net thatprovides the patients with much needed support,”said Mary M. Barnes, President and CEO.

Mr. Siegel is the first to admit that the community-based care model is one of the reasons he tirelessly supports Alzheimer’s Community Care.

“I worked at this center as an employee and I havenow been volunteering there for several years. Ihave never failed to be impressed by the staff andthe quality of care and dignity that is shown to theprogram’s participants and their caregivers. I haveseen this with my own eyes week in and week out.It brings me great pride that this center will carrymy parents’ names.”

The ribbon cutting dedication ceremony was heldon November 14, 2013, and featured speechesfrom Representative Bill Hager and City of BocaRaton Deputy Mayor Susan Haynie. With a tremen-dous outpouring of support, audience memberswere treated to stories and a tour of the new facility.

Mr. Siegel concluded the ceremony by explaininghow he couldn’t think of a more fitting cause for a donation.

“My parents were always interested in helpingthose most in need. Their parents came to thiscountry with nothing, and through the assistanceof others were able to build a foundation that allowed our family to achieve success.”

Alzheimer’s Community Care is eternally thankfulfor all of those who have helped us along the way.We truly enjoy providing a safe, dignified and compassionate environment for those who need itmost.

Half-Day Care PlanSince 1996, Alzheimer’s Community Care has prioritized specialized, quality, and compassionate carefor patients and caregivers. The organization is always self-evaluating and soliciting feedback from itscaregivers. This feedback uncovered a need that would provide financial relief, an option for caregiversto bring patients to the Specialized Care and Service Centers for half-a-day.

“One of our caregivers is so grateful for this option because she was previously paying for a full-daywhen she came at 1 p.m.,” said Denise Douglas, Program Manager for the Phyllis & Julius SiegelAlzheimer’s Care and Service Center in Boca Raton, Fla.

Program Manager’s meet with families, and assess their individual needs. The Program Manager thenrecommends a plan that works best for their schedule and budget. Beginning Aug. 7, 2013, Alzheimer’sCommunity Care began offering half-day care plans for $40. The half-day can be utilized in the morningor afternoon, depending on the family’s needs.

“Caregiver’s need support too, and it’s important that Alzheimer’s Community Care meets not only thepatient’s needs, but also those of our caregivers,” said Mark Cornett, COO.

It is extremely important for caregivers to take personal time to run errands, socialize or just relax. AtAlzheimer’s Community Care a place exists where patients receive cognitive stimulation with others ina similar situation.

“We’ve had this request for years,” said Cornett.

Alzheimer’s Community Care and Joyful Melodies

Tino Negri arrives with maracas, tambourines,American flags, and most importantly, smiles. Buthe isn’t the only one smiling. The patients atAlzheimer’s Community Care’s Specialized Careand Service Centers are full of smiles becauseNegri cares so deeply for them.

“My biggest highlight is when I am able to walk intoa community and pull someone out of a bad state,make them smile and be happy,” said Negri.

Negri starts by going around and personally greetingeach patient. The personalized touch allows him toconnect with patients before they break into song.The one-on-one interaction doesn’t end there because Negri sings with each patient individually,and even has a few of them join him for dancing.

“He brings out the very best in our patients. Whenhe comes they just sing and smile, it’s a wonderfulthing,” explains Kathy Peterson, Alzheimer’s Community Care Program Manager for North Stuart.

Joyful Melodies began in February 2013 whenComForcare, Negri’s company, was searching for amusician to create a music program. As a

percussionist Negri began working on incorporatingrhythm into an engaging and interactive sing-a-longprogram. He developed an activity-based musictherapy program that emphasizes positive interaction, not only between the musician and thepatients, but also between the patients themselves.

“When they clap their hands and tap their feet tothe rhythm while shaking the maracas I give them,it has multiple benefits. It gets people to exercisejust by keeping them moving, singing, and maybeeven getting up to dance,” said Negri.

Exercise is a major benefit of Joyful Melodies, butremembering lyrics and bringing back memories isthe core of the program. Negri chooses songs thatare familiar to the patients, and is always happy totake special requests.

“The patients really love him, he knows what to doand how to react to each individual patient and theirspecific needs,” adds Peterson.

Joyful Melodies continues to be one of the mostpopular therapeutic activities offered at Alzheimer’sCommunity Care. Cognitive stimulation is vital forpatients with Neurocognitive Disorders. Florida hasmore residents over the age of 65 than any otherstate in the U.S, and approximately half a millionFloridians are affected by Alzheimer’s disease according to the Florida Department of Elder Affairs. As the Baby Boomer population ages, thenumber of Florida residents with Alzheimer’s disease is projected to rise by nearly one-third between 2010 and 2025.

On March 12-13, 2014, Alzheimer’s Community Care COO Mark Cornett and Grants Administrator Kathleen Herd will travel to San Diego to receive the 2013 Rosalinde Gilbert Innovations inAlzheimer’s Disease Caregiving Legacy Award on behalf of the organization. The award will be presented at the 2014 Annual Conference of the American Society on Aging and recognizes outstandingand innovative care practices, specifically, for the organization’s “Diverse/Multicultural Communities”work and mission within the Glades’ Communities.

Each year, the Family Caregiver Alliance National Center on Caregivers conducts a national solicitationfor competitive proposals from non-profit organizations and governmental agencies. Three organizationsare then selected to receive awards of $20,000 each, recognizing unique programs that address theneeds of Alzheimer’s caregivers.

“I am very proud of what we stand for and for our achievements. Together we continue to make thoseimportance differences that have long lasting effects on our communities, our families and society,” statesMary Barnes, President and CEO at Alzheimer’s Community Care.

Preparing for aMedical Emergency

A trip to the emergency room can be a traumatic event. To make these visits less traumatic, plan aheadand have an emergency packet readily accessible at all times.

This packet should include, at minimum, the following for both you and your loved one:

Be sure that your emergency contacts are aware that you’ve listed them, and that they know of any specific requests you may have. If you have a home health agency that is assisting you in the home, ora long term care policy, include this information as well. You may have met with a home health agencythat has your information, however, in-home care may not have begun. It is a good idea here, to alsoinclude the agency’s name and #. This will make the process smoother if in-home care needs to be initiated quickly, and your loved one will need the in-home care if you are the patient. As with any legaldocuments, give a copy to someone listed as an emergency contact(s), or named as DPOA and/orhealth care surrogate.

In the event your loved one is being taken to the ER, alert hospital staff of a memory loss disease; besure to give as much information about your loved one that will assist in their care. Some hospitals mayrequire a private sitter for your loved one if you are unable to stay with them; be sure to ask.

1. A list of all current medications (prescribed and over-the-counter, including vitamins).

2. Allergies to food or medication.

3. Name and # of physicians.

4. Emergency contact information.

5. Copies of legal documents (Living Will, DPOA, Do Not Resuscitate Order (DNR).

6. If you have a cell phone, program I-C-E into it (in case of emergency) with the name and phone # of someone who knows you, your loved one, and your medical histories.

Mariam KhanWest Lake Worth/Program Nurse

Mariam started with the organization as a Per Diem Nurse in August 2012, she was promoted to a full-time Program Nurse in October 2012.

Mariam was nominated by her peers for the Employee of the Quarter honor for the following reasons:

• Her accomplishments since becoming the Program Nurse. Mariam has an eagerness to learnand ask questions about the organization, not being content for basics and wanted to learn more aboutthe services, patients and caregivers that we served. Also she is able to fill in and assume a larger job responsibility when the Program Manger has been out of the building at meetings.

Mariam has a great work ethic and is always willing to help out, always cheerful with a smile and neverexpresses a negative mood or comment. She can do the hula and get back to the care plan in 5 notes,and the patients and caregivers love her.

Marilyn SilvestriFamily Nurse Consultant Assistant - Treasure Coast

Marilyn started employment with the organization on June 29, 2013.She originally started as the Activities Coordinator at Stuart DayCenter. In December 2012, Marilyn was promoted to the role ofFamily Nurse Consultant Assistant.

Marilyn was nominated by her peers for the Employee of the Quarter honor for the following reasons:

• Her ability to grow and flourish while performing her current job responsibilities. Her Commitment to team work and always showing willingness to assist others and work in unison when aproject is presented to her.

Marilyn is a true professional and is a plus to the organization. She has been facilitating Support Groupsand is enjoying her role and listening to caregivers while directing them to staff and resources that aremost appropriate to meet their concerns.

ACC salutes Marilyn Silvestri for her excellent job performance and for being EMPLOYEE OF THE 2rd QUARTER 2013.

Volunteer Department

In the words of Robert “Sargent” Shriver, creator of the Peace Corps, father of Maria Shriver, whodied of complications from Alzheimer’s disease at age 95.

Alzheimer’s Community Care is currently recruiting for volunteers to serve in the following capacities: General office/ clerical assistants, day care assistants for all Specialized Adult DayCenter programs, Support Group Facilitators and co-facilitators, special events and health fair representation.
