On October 17, 2017 the Board of Clark County Commissioners approved the revised General Ground Transportation Automated Vehicle Identification (AVI) System Operating Permit for operations at McCarran International Airport. INSTRUCTIONS: Check each box and submit this checklist with all supporting documentation. Incomplete applications or submittal of incomplete documents will not be accepted for processing, and will be returned to the applicant.

Applicants will be contacted by the Parking Office when the Permit is approved. Please note that the permit approval process may take up to 14 days.

GENERAL GROUND TRANSPORTATION AUTOMATED VEHICLE IDENTIFICATION SYSTEM OPERATING PERMIT

THIS GENERAL GROUND TRANSPORTATION AUTOMATED VEHICLE IDENTIFICATION SYSTEM OPERATING PERMIT (“Permit”) is granted by Clark County, on behalf of its Department of Aviation (“County”), in accordance with Clark County Ordinance 20.04.040 and the Clark County Department of Aviation’s (“Department”) Landside Operations Operating Directive 01-2-R005 (“Operating Directive 01-2-R005”), to:

This permit grants a non-exclusive limited operational privilege to the general ground transportation Operator under the Automated Vehicle Identification System (“AVI”) at McCarran International Airport (“Airport”) predicated upon Operator’s acceptance and compliance with the terms and conditions of this Permit and Operating Directive 01-2-R005, which may be amended from time to time by the Clark County Director of Aviation (“Director”) or designee.

1. ELIGIBILITY: This Permit will be issued based upon Operator’s compliance with all of theterms, conditions, rules and regulations provided in Operating Directive 01-2-R005,including any exhibits to Operating Directive 01-2-R005.

2. TERM: The term of this Permit shall commence upon the date of the Operator’s execution ofthis Permit, and shall continue on a month-to-month basis until December 31, 2022. ThisPermit may be terminated by either party, with or without cause, at any time upon thirty (30)days advance written notice to the other party.

3. GUIDELINES: Operator may use only the designated areas of the Airport for the purposescontained in this Permit and Operating Directive 01-2-R005, including all AVI terms andconditions therein, and for no other purpose. Operator shall not conduct any activity oroperations in or upon Airport property that is not authorized by the Director or designee.Operator acknowledges and agrees that Operator has no exclusive rights to conduct thebusiness described herein and that the Department may arrange with others for similaractivities at the Airport. Operator, its employees, agents, drivers, and/or representativesmust immediately comply with all operating requests received from the Department.

4. FEES AND CHARGES: Operator agrees to pay all applicable trips, dwell fees, and charges as

set forth and as further described in Operating Directive 01-2-R005, as may be modified from

time to time. Operator will be invoiced monthly for the preceding month’s AVI trip and dwell

fees. The Operator will make all payments within thirty (30) days of the date of each

invoice, by check, payable to Clark County Department of Aviation and deliver or mail

payments to Clark County Department of Aviation, Finance Division, at P.O. Box 11005,

Las Vegas, NV 89111-1005 or to such other place as the Department may direct the

Operator in writing.

5. REDETERMINATION OF FEES AND CHARGES: The Director or designee retains the right to

re-determine such fees from time to time pursuant to Operating Directive 01-2-R005 and

Clark County Ordinance 20.04.040, which may also be amended as deemed necessary.

Operator agrees to pay the established rates within thirty (30) days after notice is received

from the Director or designee.

6. NOTICES: Notices intended for County will be addressed to:

instrument in writing, executed by the parties. The parties further understand and agree that

the other party and its agents have made no representations or promises with respect to this

Permit or the making or entry into this Permit, except as in this Permit expressly set forth,

and that no claim or liability for cause for termination shall be asserted by either party against

the other, and such party shall not be liable by reason of the making of any representations

or promises not expressly stated in this Permit or Operating Directive 01-2-R005, any other

written or oral agreement with the other party being expressly waived. To the extent of any

inconsistency or conflict between the terms and conditions of this Permit with the terms and

conditions of Clark County Ordinance 20.04.040 or Operating Directive 01-2-R005, the terms

and conditions of Operating Directive 01-2-R005, including exhibits thereto, shall prevail and

control.
