9:00am – 11:00am PCA/HCCN/NCA Networking Session: Aligning the Cooperative Agreements California

HRSA has released the 2017-20 competitive funding opportunity announcement for NCAs, S/RPCAs, and HCCNs. All three T/TA groups will be on the same funding cycle. One key requirement in the FOA is increased coordination and collaboration across and among all three T/TA groups. This session will offer HRSA a chance to share more details about this collaboration expectation. It will also provide attending NCAs, S/RPCAs, and HCCNs an opportunity to initiate conversations around collaboration opportunities and identify levels of training and technical assistance coordination across grant applications. This special session will also provide updates on how NACHC is planning to work with all NCAs, S/RPCAs, and HCCNs. Further, all NCAs will be provided time to work with PCAs and HCCNs to formulate partnerships to enhance the program.

Choral Performance: Urban Voices Project, Los Angeles, CA

The Urban Voices Project, is an endeavor to provide healing through music for homeless and

disenfranchised individuals on Skid Row. Composed of artists and performers from the Skid Row neighborhood in Downtown Los Angeles, this project is presented by The Colburn School and John Wesley Health Centers (JWCH Institute) to bring music, health and well-being, and community to one of the largest homeless neighborhoods in the United States. The Urban Voices Project: A Skid Row Choir serves as a “bridge” to the Wesley’s health and wellness services for our homeless. Through music education, musical workshops, and a performing ensemble, the Urban Voices Project continues to share music and its healing power with many individuals and audiences inside and outside the neighborhood of Skid Row. Leeav Sofer, Colburn School teaching artist, directs this project and is assisted by Christopher Mack and Kate Richards Geller. The Urban Voices Project has found recognition on the front page of the LA Times and was featured on NPR’s All Things Considered. The Urban Voices Project has had the privilege to collaborate with Street Symphony’s Messiah Project in December 2015, as well as LA Opera’s Community Festival Play of Daniel in March 2016.

Dreamland: The True Tale of America’s Opiate Epidemic

Sam Quinones, Journalist and Author

Sam Quinones is a Los Angeles-based freelance journalist and author of three books of narrative nonfiction. His latest book Dreamland: The True Tale of America’s Opiate Epidemic (Bloomsbury, 2015) recounts twin stories of drug marketing in the 21st Century: A pharmaceutical corporation flogs its legal new opiate prescription painkiller as non-addictive. Meanwhile, immigrants from a small town in Nayarit, Mexico devise a method for retailing black-tar heroin like pills in the United States, and take that system nationwide, riding a wave of addiction to prescription pills from coast to coast.

The collision of those two forces has led to America’s deadliest drug scourge in modern times.

Sam Quinones will sell and sign copies of his book immediately following this session in the International Foyer.

The Community Care Coordination Program is an essential element of Population Health Management involving effective care management of patients with more than two chronic diseases. Utilizing health coaching principles and mutual goal setting strategies, Care Coordinators help patients self-manage their chronic disease resulting in improved outcomes related to quality of life, longevity, and healthcare cost reduction. A successful Care Coordination program relies on effective partnerships with local colleagues and key leaders to ensure that available community resources align effectively with patient needs. Capturing social determinants of health data is becoming an increasingly important part of care delivery. The Protocol for Responding to and Assessing Patient Assets, Risks, and Experiences (PRAPARE) is one tool that health centers can use to identify and address patient social risk factors. This session will focus on the role of PCAs and HCCNs in aggregating the data collected by the tools and bringing the model to scale.

10:00am – 11:30am HTuC1 Can We Improve Health by Collaborating Across Systems? Pacific

Across the country, access to specialty behavioral health services is inadequate and many individuals and healthcare entities are looking to health centers to fill the void. For individuals receiving mental health and primary care services at separate locations, the ability of their providers to share information and coordinate care is challenging at best. Furthermore, providers are often concerned they are making treatment decisions with incomplete or inaccurate information. In this session, findings from two San Diego pilot programs, Shared Treatment Pilot and Transition Visit Pilot, will be reviewed – focusing on the response of the providers involved in the project. The rationale, purpose, and goals of the pilot programs will be explained, highlighting the reactions of mental health clients who were transitioned to primary care for ongoing care. Methods used in the projects to “work around” the current barriers to communication and coordination of care between the mental health and primary care providers will also be discussed. Participants will also explore how PCAs and HCCNs can support their health centers to take advantage of value-based payment opportunities to serve these patients efficiently and effectively.

According to a recent NACHC report, as many as 95% of health centers reported at least one clinical vacancy. The Association of Clinicians for the Underserved (ACU) STAR² Center has created individual recruitment and retention profiles for each health center program grantee and FCHC Look-A-Like across the country and has also worked with PCAs to create workforce strategic plans. The California Primary Care Association also recently released a report, Horizon 2030, Meeting California’s Primary Care Workforce Needs, that highlights the tremendous shortage of primary care providers in California, but also lays out a roadmap of policy, regulatory, and program solutions to address the issue. In this session, participants will learn about strategies for addressing workforce challenges and creating a workforce program. A representative from Western Governors University will also discuss ideas for addressing these persistent workforce issues health centers face.

4:00pm – 5:30pm HTuB3 Medicare Shared Savings Program Best Practices California

With the advent of accountable care and value-based pay, health centers face a myriad of options that need to be thoroughly analyzed to assure smart business decisions are made. The challenge with many of these new platforms is they are generally state-based and sometimes even localized, making it very hard to compare and contrast what might be the best path forward. However, the Medicare Shared Savings Program (MSSP) levels the playing field across the country and provides significant strategic alignment for health centers focused on the Triple Aim. Not only can health centers create local or state-driven clinically integrated networks, they can look at MSSP platforms anywhere in the country to see how others are solving issues and creating successful care management models. This session will look at health center networks from across the country using different models and how they have led to success. Further, this session will explore how these networks intend to use the MSSP to strategically move toward other payers such as Medicaid and commercial markets.

The refresher on key shortage designation concepts includes a look at some of the programs that use shortage designations, the types of Health Professional Shortage Area (HPSA) designations, and the HPSA designation and scoring criteria.
