Contract Temporary Staffing Guidelines
Welcome to Boly:Welch! As your employer on assignment, our team has developed guidelines to ensure your success as a Contract Temporary employee. Your Boly:Welch Recruiter will be your key point of contact. Questions regarding your employment or assignment should be directed to us, while job function or work flow issues can be discussed with your work location supervisor. For example, we suggest that you limit personal calls and internet usage to designated breaks, but norms may differ by client.
Priority is given to candidates who email [email protected] each weekend with name and upcoming weekday availability. No news will be seen as a voluntary deactivation of your employment relationship.
While on Assignment, it’s important to stay in touch with your Boly:Welch Recruiter:
1. Your safety is critical. Exercise caution and good judgment, and follow all workplace safety practices or procedures.
• Notify us of any on-the-job injury to ensure prompt resolution and swift claims processing. • All injuries require drug and alcohol screening upon receipt of medical attention.
2. Contact Boly:Welch if requested to:
• Drive for the client while on assignment • Handle cash or valuables • Change your job duties substantially
3. Punctuality and regular attendance are essential to our mutual success.
• Please arrive 10-15 minutes early for your assignment. • If you are going to be absent, notify us at least two hours prior to your shift. • For late arrival, whenever possible, contact us with your estimated arrival time. • Schedule time off with us two weeks in advance. • “No Call. No Show” is grounds for disciplinary action or termination after one day. • Failure to report to work without calling in for two consecutive workdays is a voluntary termination. • Notify us with any commitments that will interrupt or limit the duration of your assignment.
4. Clothing and appearance should be professional and align with the client-specific dress code. 5. Client Equipment & Property
• Equipment, supplies, identification cards, badges, company keys, phones, computers, and client-supplied materials are for business purposes only, may not be taken or used for other purposes, and must be returned immediately upon assignment completion or termination. Failure to do so could be considered theft and may result in termination as a Boly:Welch employee.
Payroll & Paychecks
• Employees are paid weekly for all time worked on Contract Temporary assignment. • Complete and submit a timecard to our accounting team by 5 pm on the Monday following the
week you worked to help assure prompt receipt of your paycheck. • Workweeks are the seven-day period starting Monday at 12:01 am, ending Sunday at midnight. • Please include only one week per time card with the dates you worked, the company you worked for, and
the last four digits of your SSN. • Your Recruiter will let you know which of the following processes is available for your assignment:
o Email [email protected] — Ask your supervisor to sign off on your hours. o Submit via online timecard portal — Your supervisor will be notified to sign.
• We offer Direct Deposit for all of our employees. Complete and submit the form along with a voided check to enroll. • If you haven’t set up Direct Deposit, your paycheck will be available for pickup at our office by 10 am the Friday after you
submitted your time card. Please remember to bring I.D. with you when you pick up your check. • Paychecks will not be released to anyone other than the Boly:Welch employee without written authorization from you. • Contact our Payroll Department with additional questions!
Benefits: You’re eligible for health care benefits − medical, dental, and vision and FSA − the first of the month following 60 days on assignment. You must be working a minimum of 30-hours-per-week in order to qualify for eligibility. Flexible Spending Account: An FSA Section 125 Plan is offered to support medical expenses with pre-tax dollars. Oregon Sick Pay: Available after 90 days. You accrue one hour for every 30 hours worked to a maximum of 40 hours. Holidays: Employees receive a $50 bonus upon completion of 850 hours of service for the following holidays:

Vacation: Employees receive a $300 vacation bonus upon completion of 1,300 hours of service. 401K Employee Deferral Elections: Upon completion of eligibility requirements, you may elect to contribute a percentage of your compensation to the 401(k) plan each year; contributions are 100 percent vested. 401K Safe Harbor Non-elective Contribution: Boly:Welch will make a contribution equal to three percent of your compensation. You are immediately 100 percent vested in this contribution amount, and it will be made regardless of whether you contribute anything to the plan. Contributions will be made at a minimum annually. Eligibility for both Deferral Elections and Non-Elective Contributions requires that you:
• Be 21 years of age or older. • Complete 1,000 hours of service during the 12-month period beginning on your date of hire or
any plan year beginning after your date of hire. • Entry dates for the contribution are the first day of January and July.
Profit Sharing: Boly:Welch may make a discretionary profit sharing contribution. This contribution is always 100 percent vested. To be eligible for a profit sharing contribution, you must:
• Be 21 years of age or older. • Have 24 months of service. • Complete 1,000 hours of service for every 12-month period beginning on your date of hire or
any plan year beginning after your date of hire. • Entry dates for the contribution are the first day of January and July.

• Quarterly invitations to the Adidas and Columbia employee stores for a discount on market prices. • Discounted membership rates at Portland Metro 24 Hour Fitness locations. • Save 22% on monthly wireless service plans through AT&T.
Referral Bonus Program Referred candidates must have worked the guarantee period for a Direct-Hire or 100 hours on a Contract Temporary assignment. Please submit the tracking form within the first six months of the Direct-Hire placement or Contract Temporary assignment.
• Direct-Hire bonus is $100.00 after the candidate has worked the guarantee period and payment has been received from the client.
• Contract Temporary Assignment bonus is $100 after the referred candidate has worked 100 hours. • Use the Referral Bonus Form.

New Health Insurance Marketplace Coverage Options and Your Health Coverage
PART A: General Information
When key parts of the health care law took effect in 2014, there is a new way to buy health insurance: the Health Insurance Marketplace. To assist you as you evaluate options for you and your family, this notice provides some basic information about the new Marketplace and employment-‐based health coverage offered by your employer.
What is the Health Insurance Marketplace? The Marketplace is designed to help you find health insurance that meets your needs and fits your budget. The Marketplace offers "one-‐stop shopping" to find and compare private health insurance options. You may also be eligible for a new kind of tax credit that lowers your monthly premium right away. Open enrollment for health insurance coverage through the Marketplace is October 15 to December 7 each year. Special Enrollment periods exist throughout the year if you experience a loss of coverage.
Can I Save Money on my Health Insurance Premiums in the Marketplace? You may qualify to save money and lower your monthly premium, but only if your employer does not offer coverage, or offers coverage that doesn't meet certain standards. The savings on your premium that you're eligible for depends on your household income.
Does Employer Health Coverage Affect Eligibility for Premium Savings through the Marketplace? Yes. If you have an offer of health coverage from your employer that meets certain standards, you will not be eligible for a tax credit through the Marketplace and may wish to enroll in your employer's health plan. However, you may be eligible for a tax credit that lowers your monthly premium, or a reduction in certain cost-‐sharing if your employer does not offer coverage to you at all or does not offer coverage that meets certain standards. If the cost of a plan from your employer that would cover you (and not any other members of your family) is more than 9.5% of your household income for the year, or if the coverage your employer provides does not meet the "minimum value" standard set by the Affordable Care Act, you may be eligible for a tax credit.1
Note: If you purchase a health plan through the Marketplace instead of accepting health coverage offered by your employer, then you may lose the employer contribution (if any) to the employer-‐offered coverage. Also, this employer contribution -‐as well as your employee contribution to employer-‐offered coverage-‐ is often excluded from income for Federal and State income tax purposes. Your payments for coverage through the Marketplace are made on an after-‐tax basis.
