WHAT DO I DO ONCE MY BOXES ARRIVE?
1. Locate District Box One and find the district and school packing lists.
2. Check to be sure you have received all materials the packing lists indicate you have been sent:
a. Contact the vendor to inform them that you did not receive all the materials either by email at [email protected] or by fax at 217-398-5798.
b. Attach a copy of the email or fax to the 2005 – 2006 Documentations of Materials not returned form.
3. Check that you have received a sufficient amount of material. If not, you may request additional materials via the Additional Materials Order form. Fax the form to MetriTech, Inc. at 217-398-5798.
4. Divide material into packages for each school.
5. Create sign-in and sign-out forms for test materials. A sample is posted on our the Bilingual web site. You will need this to answer any security questions.

DIRECTIONS: The School Test Coordinator must make sure that the test administrator has signed the Agreement to Maintain Confidentiality before issuing secure test materials. The test administrator must date and sign this form when secure test materials are issued. The School Test Coordinator must sign this form when secure test materials are returned.
NOTE: The School Test Coordinator should keep one copy of all completed forms and return the original to the District Test Coordinator with the secure test materials.
