This guide and the functionality explained within assume that the patient name entered into the NextGen database is the Legal name of the individual. If/when it is required by 3rd party vendors, the patient name stored in the NextGen database will be submitted with a code indicating this is the Legal name. You must take all necessary steps to ensure that the information stored in the person table is, in fact, the Legal name for each applicable individual if you are using an interface that requires such an indicator.

Purpose The NextGen® EHR Meaningful User Reports Guide assists you in learning the setup and inner workings of the Meaningful Use Reports. This guide helps you achieve better results using NextGen EHR application.

Audience This guide is for the NextGen EHR System Administrators.

Who is a System Administrator? A System Administrator is responsible for ensuring that the system is set up properly and that all pertinent components, including hardware, runs properly. The System Administrator is also the person responsible for ensuring that the application contains all the correct and necessary information so that a day in the office runs smoothly.

The American Recovery and Reinvestment Act (ARRA) includes over $60B, designed to stimulate healthcare IT (HIT) adoption by the medical community over the next few years. Of the total money available, approximately $46B is dedicated to pay-for-performance through Medicare and Medicaid Incentives to doctors & hospitals for "Meaningful Use" of certified EHR applications.

Meaningful Use of certified EHR technology: To take advantage of these incentives you must be able to demonstrate Meaningful Use of Healthcare IT.

The EHR must be certified, and you need to use the EHR for:

ePrescribing

Sharing clinical data with other certified EHR systems (interoperability)

Reporting on specified clinical quality measures

Meaningful Use Crystal Reports NextGen Healthcare is committed to providing the proper guidance, tools, and services to help you demonstrate Meaningful Use and earn stimulus incentive payments. As part of this commitment, NextGen EHR has been enhanced with a series of Crystal Reports designed to enable you meet the Meaningful Use criteria. The following reports have been developed and are available:

MU Summary Report by Provider: generates a summary of all Meaningful Use reports per provider. This report is only available when using NextGen KBM, Version 7.9.2 or higher

MU Orders Entered by Provider: provides the percentage of patients who have had a face-to-face encounter that have a documented active medication and have a medication prescription entered by the provider

MU Patients with a Diagnosis: provides the percentage of patients who have had a face-to-face encounter and have received a diagnosis (code) or an indication the patient has no known problems

MU ERX Non-Narcotic Prescriptions: provides the percentage of all non-narcotic (DEA Schedule 0) prescriptions within the last year that were submitted electronically

MU Patients with Recorded Medication: provides the percentage of patients who have had a face-to-face encounter and have an active medication or No Medication indicator checked

MU Patients with Recorded Allergies: provides the percentage of patients who have had a face-to-face encounter and have an active allergy or No Allergies indicator checked

MU Patients with Recorded Demographics: provides the percentage of patients who have had a face-to-face encounter and have all of the following demographic information documented:

MU Patients with Recorded Vital Signs: provides the percentage of patients over age 2 who have had a face-to-face encounter and have a documented measurement for height, weight and blood pressure

MU Patients with Smoker Status: provides the percentage of patients who are 13 years of age or older and who have had a face-to-face encounter with Smoking Status documented

MU PHR Request Completed in 72 Hours or Less: provides the percentage of patients who have requested an electronic copy of their Personal Health Record (PHR) and had the request completed within 72 hours

MU Patient Clinical Summaries: provides the percentage of patients who have had a face-to-face encounter and received a clinical summary of the visit.

MU Patients with Medication Reconciliation: provides the percentage of patients that have a documented transition of care or who have not been seen in one year that have a documented medication review/reconciliation.

MU Summary of Care Records: provides a list of patients who have a transition of care and a summary of care record sent.

MU Lab Order with Results: provides the percentage of patients who have had a face-to-face encounter and have lab results entered in their chart as structured date

MU Patients Reminders: provides the percentage of patients who are 5 years of age or younger or 65 years and older who had reminders sent to them

MU Patient Education Provided: provides the percentage of patients who have had a face-to-face encounter and have been given specific patient education

Requirements In order to use Meaningful Use Crystal Reports, you must have the following components installed and/or properly set up:

NextGen EHR, Version 5.6 SP1 or higher

NextGen System Administrator

NextGen KBM Meaningful Use compliant version:

NextGen KBM, Version 7.6.10 or higher NextGen KBM, Version 7.7.5 or higher NextGen KBM, Version 7.8.5 or higher NextGen KBM, Version 7.9.2 or higher Note: NextGen KBM, Version 7.9.x is the preferred solution, but minimum reporting requirements can be met with the indicated service packs to NextGen KBM, Versions 7.6, 7.7, or 7.8)

Availability of Meaningful Use Crystal Reports

You are able to obtain all of Meaningful Use Crystal Reports by downloading the reports package from the Customer Resource Center (CRC) of the NextGen website. To set up Meaningful Use Crystal Reports from a downloaded package, access the directory containing the package, extract the report reports and follow the steps to set up Meaningful Use Crystal Reports (see "Steps to Set Up Meaningful Use Crystal Reports" on page 8) starting with Step 2.

Caution: If necessary, any updates to the Meaningful Use Crystal Reports will be distributed through the download package from the CRC. You must make sure to have the latest available copy of the reports. If you download Meaningful Use Crystal Reports, extract from the package and follow the steps to set up Meaningful Use Crystal Reports starting with Step 2.

Steps to Set up Meaningful Use Crystal Reports

In order to be able to run the Meaningful Use Crystal Reports, you must be using NextGen EHR, Version 5.6 SP1. Prior to using the MU Crystal Reports, you must properly set these up.

To set up Meaningful Use Crystal Reports: 1 Log on to NextGen System Administrator. 2 From the Main Menu bar, select View > Universal Preferences. 3 Select the Reports folder to view the existing report paths. 4 Check the location specified in the Custom Report Path Universal Preference. 5 Copy the Meaningful Use reports to the location specified in the Custom Report Path. 6 Set up each Meaningful Use Report in the NextGen EHR. 7 If necessary, set up NextGen Report Server. 8 Test the report. 9 Verify the reports generate correctly.

Verify Custom Report Path Prior to using the Meaningful Use Reports, you must verify the Custom Report Path Universal Preference.

To verify Custom Report Path Universal Preference:

1 Double-click on the NextGen Desktop shortcut, or click on the Windows Start button, go to the Programs selection, and then choose NextGen. NextGen Application Launcher displays.

2 Click System Administrator.

Copy Meaningful Use Reports Locally To copy the Meaningful Use Reports: 1 Locate the the Distributed Reports folder on the server. 2 Open the Crystal Reports sub-folder and then Meaningful Use sub-folder.

3 From the Main Menu, select Edit > Select All. 4 From the Main Menu, select Edit > Copy. 5 Locate your C:\NextGen directory and navigate to the directory specified in the

Custom Reports Path Universal Preference. 6 From the Main Menu, select Edit > Paste.

All of the copied report files are moved to the Custom Report directory.

Setting Up Meaningful Use Crystal Reports

In order to generate Meaningful Use Crystal Reports you are required to set up each report and assign it to a printer.

To set up a Meaningful Use Crystal Report:

1 Double-click on the NextGen Desktop shortcut, or click on the Windows Start button, go to the Programs selection, and then choose NextGen. The NextGen Application Launcher displays.

2 Click EHR.

Note: Only reports already placed in the directory specified in the Custom Report Path (see "Verify Custom Report Path" on page 9) Universal Preference are displayed.

9 Select All Encounters for All Patients from Report Type drop-down list.

Caution: Selecting any other value for the Report Type will prevent Meaningful Use Crystal Reports from being generated.

Report Server The NextGen Report Server relieves your NextGen 5.x database server from the processing burden associated with generating NextGen EPM and NextGen EHR reports.

Report Server Installation To use the Report Server, you must replicate your database on a regular basis. The Report Server then uses the replicated database to generate reports. Database replication must occur regularly whenever an upgrade or version has been applied.

If you are licensed for NextGen EPM or NextGen EHR, the Report Server client installs automatically when the NextGen applications are installed. You can run the client from the NextGen workstation, but the duplicate database used with the NextGen Report Server should be set up on a separate machine. This duplicate database should be configured by your NextGen installer.

The ngconfig.ini file must have the following information: [DATA SOURCES] Source1=(production database name) ReportSource1=(replicated database name)

[REPORT_SERVER] DSN=(replicated database name) ReportServerSite=True

The specifications required for the duplicate database system depend on the size of your NextGen database. Because Report Server only generates and prints reports, the duplicate database computer does not require the same capabilities as the computer housing the primary NextGen database.

The Report Server workstation does experience some performance degradation due to the amount of data required to generate the reports; running other software applications on the workstation worsens the performance even further.

Replicating Your Database Your replicated database must be current to the original database so that the reports can be accurate. Once the Report Server is installed, configure your backup process to place a duplicated database on the Report Server's duplicate database system on a regular (nightly) basis. This process saves you from having to do it every night.

When you log onto the Report Server, two database connections are established - one to the Report Server and one to the NextGen main database. Memorized reports created or modified in the Report Server are saved to the main database. The Report Server can then recall the memorized reports at any time because they are retrieved from the main database.

Qualifying Office Visits Several of the Meaningful Use Crystal Reports use either a unique patient or office visit for the denominator. The reports reference a list of visit codes to determine if the patient is a unique patient, or if the patient was seen in the office. The codes are stored on the system template ngkbm_custom_dbpicklists,which enables you to modify the codes being referenced. A unique patient is one that has been seen in the past 3 years and has another unique visit during the reporting period. The purpose of identifying a unique patient is to ensure that their records are maintained by the electronic health record.

MU Patients with Medication Reconciliation

Requirement Perform medication reconciliation for more than 50% of transitions of care in which the patient is transitioned into the care of the provider.

Caution: There is a Crystal Report for NextGen KBM, Version 7.6.x through 7.8.x and a different Crystal Report for NextGen KBM, Version 7.9.x to accommodate the additional field that was added in the NextGen KBM, Version 7.9.x on the med rec pop up.

NextGen Analysis The Denominator is querying for a patient that has had a face to face encounter during

the reporting period that has not been seen in at least one year, or has had a transition in care documented using Interim History pop up (in addition, NextGen KBM, Version 7.9.2 or higher, has an additional field to document transition of care on the medication reconciliation pop up).

The numerator is querying for either a medication reconciliation completed (master_im_.chk_medreviewed) OR eyemasterHx_.no_systemic_meds AND eyemasterHx_.no_ocular_meds OR No Active Medications check box in the Medications Module).

Suggested Workflow (NextGen KBM, Version 7.6.x through 7.8.x) When a patient has not been seen in over one year, or the patient has a transition of care (documented on the Interim History pop up) such as a hospitalization, respite care, etc., perform medication reconciliation and ensure master_im_.chk_medreviewed check box is selected. If patient is not taking any active medications document using the master_im_.chk_nomed check box or master_im_chk_nomed or No Active Medications check box in the Medications Module.

Suggested Workflow (NextGen KBM, Version 7.9.x) When a patient has not been seen in over one year, or the patient has a transition of care (documented on the Interim History pop up) or a transition of care documented on the medication reconciliation pop up perform medication reconciliation. MU Timely Access Requirement The Denominator is querying for a unique patient seen during the reporting period

(each encounter).

The Numerator is querying the encounter date and verifying a document upload or CCD upload to NextMD (Patient Portal) for the encounter and for lab results signed off on in the PAQ (uploads are within 4 days excluding weekends).

This report requires you to set up the ngkbm_custom_dbpicklists system template to include documents for lab results. This set up enables you to determine which documents include lab results. When a lab result is accepted/signed off in the PAQ, a document that is listed in this table or a CCD document should be uploaded to NextMD (Patient Portal) within 4 days of the result being accepted.

Note: A SQL script is available to populate the ngkbm_custom_dbpicklists system template with the documents LabResults and LabResults_All.
