Reminder: The State of Connecticut requires each pupil to
have a complete health examination within one year before
entering Grade 10. This physical must be done after August
25, 2015 for entry into the 2016-2017 school year.
The physical form must be filled out completely –
parent/guardian fills out the front and the doctor fills out the
back. Doctor must include hemoglobin/hematocrit, vision,
hearing, and all areas as mandated (starred*) under
Connecticut State law.
