Integrated, Sustainable Network Components

• Sustainability strategies including:– Business plan with clear strategies

– Contracts and/or other means to secure and expand sustainable financing

– Infrastructure in place to receive reimbursement

• Involvement of entities at state/local level (aging, disability, public health, etc.)

• Strategic partnerships with other organizations involved in health promotion program delivery (including organizations that can embed programs)
