Infectious Diseases aiming to achieve a disease free community.
a. Assists and monitors in the mass treatment of:
- Filariasis - Schistosomiasis
b. Refers identified cases of:- Rabies
- Malaria- Filariasis
- Schistosomias c. Actively participates in the detection of TB cases in the assigned area
(including sputum smearing).
d. Identifies records monitors & refers TB-MDR atients e. Re orts and monitors cases of notifiable diseases: - Den ue - Others Pls. s ecif 2. Provides nursing care services in line with the implementation of the the
following DOH Programs:
I. MATERNAL, NEONATAL and CHILD HEALTH and NUTRITION
MNCHN A. Maternal Health
a. Performs Prenatal Care b. Tetanus Toxoid Immunization c. Follow-up /Post Natal Care
d. Refers Women with Danger Signs of Pregnancy B. Facilit Based Deliveries If rotated at BEMONC Facilit a. Handles Assists deliveries
b. Performs Cord and Neonatal Carec. Performs Intra artum & Post arum Care
C. Child Health a. Conducts Immunization to under 1 ear old children. b. Performs Deworming & Vitamin A Supplementation ( 1-5 y.o) c. Assesses malnutrition in 1-5 .o children d. Mana es common childhood illnesses

healthy programs of DOH or the LGU. c. Effectively provides health teachings and promotes proper health
practices upon client interactiond. Advocates and promotes healthy lifestyle activities to clients and
communit . V. COMMUNITY HEALTH TEAM:
Assists in the supervision and monitoring of the Community Health Team
(CHT) members a. Actively monitors the CHT Members in conducting the health survey
amon the NHTS-PR uantile 3 Familiesb. Involves/assists updating the Q1 & Q2 health data in thei area.
c. Submits monthly CHT reports.
3. Caters to nursing health care needs of the Indigenous People (NHTS-PR &
4Ps without re udice4. Supervises and assists activities of barangay health workers in their
catchment areas.5. Collaborates and coordinates health activities/plans with the other health
workers, barangay officials and/or other government & non-government
agencies.6. Supports the implementation of new DOH Programs:
a. Adolescent Immunizationb. Pneunomoccal Vaccine
c. MR-OPV7. Performs recording and reporting; TCL, FHSIS,; natality & mortality; Hilot
& BHW re orts8. Submits monthly accomplishment report and DTRs to CHD noted by the
Mun. Health Officer and/or Public Health Nurse describing the highlights of
their activities, targets and accomplishment with learning insights,
issues/concerns and recommendation.9. Carry-out other tasks as maybe assigned.
