  c. Refers patients w/ OB-Gyn complications  III. HEALTH EMERGENCY MANAGEMENT  a. Assists in the conduct of endemic disease surveillance and refers

emergency cases per assessment; Acts as field responders in times of

disasters and calamities.  b. Provides first aid intervention to emergency cases with prompt referral

to hi her health facilit  IV. HEALTH EDUCATION & PROMOTION  a. Conducts health education activities(mothers class, infodrive, etc.)  b. Facilitates/conducts health program trainings, workshops on various

healthy programs of DOH or the LGU.  c. Effectively provides health teachings and promotes proper health

practices upon client interactiond. Advocates and promotes healthy lifestyle activities to clients and

communit .  V. COMMUNITY HEALTH TEAM:

 Assists in the supervision and monitoring of the Community Health Team

(CHT) members  a. Actively monitors the CHT Members in conducting the health survey

amon the NHTS-PR uantile 3 Familiesb. Involves/assists updating the Q1 & Q2 health data in thei area.

  c. Submits monthly CHT reports.

3. Caters to nursing health care needs of the Indigenous People (NHTS-PR &

4Ps without re udice4. Supervises and assists activities of barangay health workers in their

catchment areas.5. Collaborates and coordinates health activities/plans with the other health

workers, barangay officials and/or other government & non-government

agencies.6. Supports the implementation of new DOH Programs:

a. Adolescent Immunizationb. Pneunomoccal Vaccine

  c. MR-OPV7. Performs recording and reporting; TCL, FHSIS,; natality & mortality; Hilot

& BHW re orts8. Submits monthly accomplishment report and DTRs to CHD noted by the

Mun. Health Officer and/or Public Health Nurse describing the highlights of

their activities, targets and accomplishment with learning insights,

issues/concerns and recommendation.9. Carry-out other tasks as maybe assigned.
