New participant plan format
Improved participant plan format released November 2018.
New format gives:
• Greater emphasis on individual circumstances
• Clear links between supports and participant goals
• Simpler layout and clearer definitions
• Clear information on where to get help.
![Page 20: NDIS Update - hsnet.nsw.gov.au NDIS...Specialist Disability Accommodation (SDA) Note: Figures as at 30 June 2018 To build a clearer picture the SDA market, the NDIA is publishing data](https://reader034.vdocuments.us/reader034/viewer/2022051901/5ff0ab4db9dba44625690cd0/html5/thumbnails/20.jpg)
Complex Support Needs Pathway
• The need for Complex Support Pathway identified in the 2017 Participant Pathway Review.
• When working with participants with complex support needs, the NDIA sees the participant as an individual that is part of a broader system of supports.
• It is not always an individual person or their disability that determines ‘complexity’ in the NDIS but the person’s situation and the combination of systems and supports they use.
• An NDIA Decision Tool has been developed and will feature a template of criterion and considerations to assist in decision making for delegates.
![Page 21: NDIS Update - hsnet.nsw.gov.au NDIS...Specialist Disability Accommodation (SDA) Note: Figures as at 30 June 2018 To build a clearer picture the SDA market, the NDIA is publishing data](https://reader034.vdocuments.us/reader034/viewer/2022051901/5ff0ab4db9dba44625690cd0/html5/thumbnails/21.jpg)
Complex Support Needs Pathway
• Key features of the Complex Support Needs Pathway include
– Pre-access, local engagement and liaison
– Access, detailed handovers and connections
– Specialised planning and Complex Support Practice Leads
– Skilled Support Coordinators and effective plan implementation
– Ongoing monitoring and evaluation
– Review outcomes and progress
