    Good morning!

    I am writing this proposal letter to serve as a recommendation that has something to do with

    upgrading our Hospital/Organization with purposely touching my own skills and expertise.

    Generally speaking, as a member of a Health Care Team, I would like to decrease problems

    regarding the Food Intake and increase Proper Food preference of any residence of Bongao Doctors

    Hospital in a way of a Health (Nutrition) Education which could be a good help and influence to the

    welfare to become part of coping mechanism towards different diseases.

    Here are some detailed explanations about why I want to pursue this idea.

    As a Public Health Nutritionist, I highly nominate myself to render this type of services and to

    be able for me to practice my own expertise with in my own capacity as a Nutrition Educator on our

    good Hospital.

    Furthermore, the said proposal will be basing on the Annual Calendar of the Department of

    Health year 2011 with the primary goal of Educating Health Publicly. On my very own way, theplanned annual activity of the DOH will be part of my proposal through a Public Health Nutrition.

    PROJECT INFORMATION

    Name of Organization: Bongao Doctors Hospital

    Project Title:

    HEALTH (NUTRITION) EDUCATIONDIET COUNSELING

    Project Summary:

    The Purpose of this proposal is to render a type of services

    which is believed to give help to the welfare/ Hospitals

    residence through Nutrition Education and by conducting a Diet

    Counseling to the concerns of the Bongao Doctors Hospital.

    PROJECT SUMMARY

    This project proposal is intended to the welfare and to the residence of Bongao

    Doctors Hospital. It will serve as a helping hand to those involved during the session by giving

    a Nutrition Education and by conducting a Diet Counseling to the patient individually.

    By doing a Nutrition Education, the Nutrition Educator will be teaching to the staff and

    to other Hospitals residence about Nutrition and sometimes it will be based upon the

    demand and trends. The said session will be held at any area of the Hospital aiming to cater

    the needs of the people in it. On the other hand, Diet Counseling will be done by a Dietitian,

    he/she will explain to the patient of what are the food allowed and to be avoided based from

    the interpreted data given by different department of the Hospital, prescribed diet by the

    attending Physician and problems complained by a certain patient. It will be conducted on

    patients room and can only consume 3-5 minutes of conversation.

    PROJECT BACKGROUND

    Tawi-Tawi lies in the southernmost region of the country, is considered as one of the

    poorest provinces in the Philippines, the province is teeming with abundant and rich marine

    resources but most of the people live in poverty due to unstable economic livelihood. Hence,

    access to quality Health Care is a problem of the people with insufficient health facilities like

    hospital and limited financial resources. In fact the province experience high maternal and

    infant mortality rate. Coupled with poor health condition, is the lack of manpower of the

    different health department in delivering the basic health services and implementing Public

    Health program.

    Public Health is a science and art of preventing diseases, prolonging life and promoting

    health through organized community effort to enable every citizen to realize his birthright of

    health and longevity. Also, it is an effort organized by society to protect, promote and restore

    the peoples health through the application of science, practical skills and collective actions.

    The 2008 National Nutrition Survey(NNS) is the 7th in the series of National Nutrition

    Survey undertaken once every 5 years by the Food and Nutrition Research Institute of the

    Department of Science and Technology ( FNRI-DOST).

    From the outcome Data of the National Nutrition Survey (NNS), a high prevalence of

    underheight-for-age or stunted pre-schooler was mostly observed in MIMAROPA, Bicol and all

    the region in Visayas and Mindanao.(under nutrition among 0-5 years old children, by region).

    Tawi-Tawi was one of the provinces with high magnitude of underweight adolescents. (11-19

    years old adolescents, by region and province). ARMM was also one of the regions with

    highest prevalence of underweight among lactating mothers.

    PROJECT GOALS AND OBJECTIVES

    GOALS A generalized equity among the residence of Bongao Doctors Hospital toward

    Nutrition as an ART and SCIENCE which can be possibly applied by an

    individual for everyday life.

    And to support the Department of Health Annual Calendar by implementingthe planned activity, publicly.

    HEALTH ( NUTRITION) EDUCATION

    OBJECTIVES: To be able for the Hospitals residence ( Doctors, Staff and Patients relative)

    to identify differences of the ideas based on the given topic.

    To be able for the Hospitals residence ( Doctors, Staff and Patients relative)to apply the ideas and principles of nutrition for everyday life.

    To be able for the Hospitals residence ( Doctors, Staff and Patients relative)tobe reminded about the latest and trends topic regarding Nutrition and Health.

    To be able for the Hospitals residence ( Doctors, Staff and Patients relative)to participate during the said session as part of evaluation.

    METHOD. The given topic were individualized and it is not ladderized. Therefore different

    education,publicly.

    ACTION DESIGN.

    First, the Dietitian will approach nurse station to write some vital inputs on the DietList.

    Second, the Dietitian will conduct a Nutritional Assessment and it will be done in aform of an interview by asking the patient about some information that will be

    needed to fill-in on the Diet History Sheet. Third, after an interview, the Dietitian will going to analyze datas and compute the

    diet to come up with a proper counseling regarding the food allowed and to be

    avoided by the patient based from his/her case and can only take for about 3-5

    minutes of conversation.

    CONCLUSION

    This project proposal are intended to the welfare and to any residence of Bongao

    Doctors Hospital. It said to serve as a helping hand to those involved during the session

    through the way of Nutrition Education and by conducting a Diet Counseling to the patient

    targeting one goal which is allowing and adopting the world of Nutrition as an ART and as a

    SCIENCE.
