Phase III Update
CROWNWeb Phase III began January 17, 2012.
Over 460 dialysis facilities are submitting their
data directly to CMS during Phase III.
Phase III is allowing CMS to analyze:
o Enhanced security requirements
o Interaction with new user interface
o Proof-of-concept for Health Information Exchange
(HIE) data submission efforts

The QualityNet Identity Management System
(QIMS) supports multiple CMS applications; an
account is required to access CROWNWeb.

Registering for a QIMS Account
To successfully register, users must:
o Complete the QIMS Part A form
o Indicate their CROWNWeb role and scope on the
CROWNWeb Part B form
o Complete CMS’ mandatory Security Awareness
Training (SAT) within the last 12 months, which
can be accessed via:

What is QIMS?
Robust identity management system that replaces
any previously used account registration systems.
Must be used to create and manage CROWNWeb
user accounts.
Allows facilities to manage the account activation
process at a facility level.
Eliminates notarization needs for End User
Managers and regular End Users if in-person
identity proofing can be performed.

QIMS and CROWNWeb are two separate systems
that work together.
Each system contains its own roles.
Users must indicate a QIMS and CROWNWeb
role when completing the QIMS account
registration process.

How is QIMS Structured?
QIMS is based on a hierarchy of roles:

Security Official (SO) – Top level of authority for QIMS – Confirms and manages user ID and activation
End User Manager (EUM) – First level of authority for QIMS – Authorizes what users can do in QIMS
Regular End Users – General system users

Security Officials (SO):
o Perform in-person identity proofing
o Verify completion of users’ Security Awareness Training
o Authorize account activation for users
o Submit applicants’ completed Part A forms to Help Desk
o Maintain users’ account (locked accounts, forgotten
passwords, etc.)
o Perform Annual Recertification process
o Serve as CROWNWeb Facility Administrator (Initial SO)

o Conduct first-level approval of account requests.

Printing the Part A Form
Once you submit the
application, a printable
version of the QIMS
Part A form displays.
Print the form by
clicking the Print
button in your browser
(or by pressing
CTRL+P if using
Internet Explorer).

Security Training
Users must complete CMS’ annual Security
Awareness Training (SAT) as part of the account
registration process.
Navigate to http://iase.disa.mil/eta/iss_icv5/launchpage.htm
to complete the security training, then print and
sign your certificate.
SOs – Mail the SAT certificate to the QualityNet Help
Desk with QIMS Part A and CROWNWeb Part B.
EUMs and regular End Users - Keep SAT certificate
locally for 7.5 years.

Initial Security Official
The steps to register as a facility’s INITIAL
(or ONLY) SO are different from the general
account registration process.
The initial SO should complete the Part B
form with a CROWNWeb Facility
Administrator role request.
The initial SO must mail his/her Part A form,
Part B form, and a copy of the SAT certificate
to the QualityNet Help Desk.

Account Completion
The QualityNet Help Desk will activate a
facility's initial SO’s QIMS account request and
configure his/her CROWNWeb roles and scope.
Once the SO’s account is activated, he/she will
receive four e-mails with instructions on logging
in to QIMS at the e-mail address they specified
during the application process.

3. Search for the facility on the Business Info tab.
4. Select “Master Security Official” and “Master Manager” as the
appointed SO and EUM.
5. Identity the applications to which you will need access, in addition
to the default/required QIMS option.
6. Click the Submit button at the bottom of the page.
7. Review the information on the printable QIMS account form, print
the form, and click Submit once again.
8. Complete the printed Part A form; and check the “Create Security
Official Account” box on the paper form.

Account Completion
The QualityNet Help Desk will authorize a
facility’s initial EUM’s account request once it
receives and reviews the Part A form.
The facility’s SO will activate the initial EUM’s
account and configure the EUM’s CROWNWeb
roles and scope when activating the account.
Once the EUM’s account is activated, he/she
will receive two emails with instructions on
logging in to QIMS at the e-mail address they

9. Take your printed registration form to your local Security Official
for identity verification, obtain the required SO signature, and sign
the form as the “Applicant”.
10. Go to www.qualitynet.org to access the CROWNWeb Part B form
to indicate the desired CROWNWeb role(s) and scope. This form
must be kept locally.
11. Go to http://iase.disa.mil/eta/iss_icv5/launchpage.htm to
complete the SAT.
12. Mail the completed QIMS Part A form to:

Initial EUM Registration Steps
13. The QualityNet Help Desk will route the initial EUM's request to
the Production Identity Management team to review and approve
the application request as the “Master Manager”.
14. The facility/Network/organization’s initial Security Official will
receive a notification of an account pending approval.
15. Once the initial SO activates the account request, the initial End
User Manager will receive TWO email notifications (one with a
QIMS User ID and one with a temporary password).
