The National Chronic Care Management Survey 2015

We recently invited you to participate in The National Chronic Care

Management Survey. We haven't heard from you yet, and we are following up to let you know how important your response is to this effort. The survey results will be reported to the Centers for Medicare and Medicaid Services in response to the agency's request for information regarding providers' experience in providing transitional and care management services. If you are an employee of an ambulatory care provider that serves Medicare beneficiaries, we ask for your help in producing this important resource for the nation's provider community by completing the survey. If your organization is providing CCM, it should take about

10 minutes to complete the survey. If not, it will take a few minutes less. If you believe that someone other than yourself in your organization is better qualified to take this survey, please forward this invitation to that

person. All respondents will receive a copy of a report analyzing the survey findings regarding CCM adoption across the US.
