In line with government initiatives the provision of care must be based around the needs of the local population, make best use of digital technologies and provide a sustainable health and social care system. Transformation is not easy, but is necessary as existing models of care and current pathways come under increasing pressure. Traditional models of care have remained largely unchanged for over 60 years and are no longer sustainable.
